Neural stem cells and their application to multiple sclerosis.
1. AIDS dementia complex (ADC): cellular and viral pathogenesis. ADC pathogenesis is poorly understood. Evidence exists for the importance of both the virus and the host. Our group is working on the importance of certain parts of HIV that may be preferentially able to cause damage to neurons. We are assessing this by using neurotropic strains of HIV and determining the extent of neuronal damage in a human neuronal tissue culture system. We are also examining the importance of the host in the pathogenesis of ADC by determining whether there is any relationship between ADC and certain chemokine receptor mutations. Lastly we are examining the clinical significance of a variety of antiretroviral drug resistance mutations in the cerebrospinal fluid in determining the presence of ADC and its response to therapy. AIDS-Related Psychopathology A large number of psychiatric symptoms and diagnoses are now recognized as being associated with the human immunodeficiency virus (HIV) infection, acquired immunodeficiency syndrome and AIDS-related complex (ARC). According to Vomvouras, AIDS-related psychopathology can be divided into two broad areas: nonorganic conditions (or functional and reactive disorders) and organic neuropsychiatric conditions. There is considerable overlap between these two categories. The reactive disorders develop in response to stressors of the disease, while the organic conditions are psychiatric manifestations of either the primary HIV infection or its complications.

The era of highly active antiretroviral therapy (HAART) has led to a considerable decline in the HIV disease progression rates and HIV-1-related opportunistic infections especially in developed countries. Unfortunately, antiretroviral treatment for almost 90 per cent of the HIV-infected population is not available because of cost concerns. Although a number of studies have shown uniform impact of HAART on disease progression, its effect on treating HIV infection of the brain and its manifestations, such as AIDS dementia complex (ADC), remains unclear. Along with the reasons why AIDS dementia complex continues to be a problem in the era of HAART, this review also discusses the changes in ADC patterns with HAART and its relevance in developing countries such as India. In addition, an overview of various biological, molecular and therapeutic aspects that may influence HIV dementia (HIV-D) is provided.

Dementia is a brain disorder that affects a person's ability to think clearly and can impact his or her daily activities. AIDS dementia complex (ADC) dementia caused by HIV infection is a complicated syndrome made up of different nervous system and mental symptoms. These symptoms are somewhat common in people with HIV disease. The frequency of ADC increases with advancing HIV disease and as CD4+ cell counts decrease. It is fairly uncommon in people with early HIV disease, but it's more common in people with severely weakened immune systems and symptoms of advanced disease. Severe ADC is almost exclusively seen only in people with advanced HIV disease. ADC consists of many conditions that can be of varying degrees and may progressively worsen. These conditions can easily be mistaken for symptoms of other common HIV-associated problems including depression, drug side effects or opportunistic infections that affect the brain like toxoplasmosis or lymphoma. Symptoms of ADC may include poor concentration, forgetfulness, loss of short- or long-term memory, social withdrawal, slowed thinking, short attention span, irritability, apathy (lack of caring or concern for oneself or others), weakness, poor coordination, impaired judgment, problems with vision and personality change.

    A degenerative neurological condition attributed to HIV infection, characterized by a group of clinical presentations including loss of coordination, mood swings and loss of inhibitions, and widespread cognitive dysfunctions. It is the most common central nervous system complication of HIV infection. Characteristically, it manifests itself after the patient develops major opportunistic infections or AIDS-related cancers. However, patients can also have this syndrome before these major systemic complications occur. The cause of ADC has not been determined exactly, but it may result from HIV infection of cells or inflammatory reactions to such infections.
  • AIDS Dementia Complex (ADC, HIV-Associated Cognitive/Motor Complex): SFAF
    mental impairment with an organic cause that is due to HIV infection (i.e., not an opportunistic pathogen). Symptoms may include cognitive impairment (e.g., difficulty concentrating, memory loss), disorientation, mood and personality changes, speech and vision difficulties, psychomotor dysfunction (lack of coordination, incontinence) and paralysis. AIDS-related dementia typically affects people in the later stages of HIV disease. Early stages of the syndrome are referred to as HIV-associated minor cognitive/motor disorder.

Sponsors and Collaborators: National Institute of Allergy and Infectious Diseases (NIAID) Bristol-Myers Squibb Glaxo Wellcome Information provided by: National Institute of Allergy and Infectious Diseases (NIAID) ClinicalTrials.gov Identifier: Purpose To compare the safety and effectiveness of orally administered didanosine (ddI) with high dose orally administered zidovudine (AZT) in patients who develop or exhibit progression of the AIDS dementia complex (ADC) and who have not previously been intolerant to AZT at doses of up to 1000 mg/day. HIV-infected or AIDS patients may develop ADC which causes damage to the nervous system. ADC may be caused by some action of the AIDS virus on the nervous system, although similar problems can be caused by other infections because the AIDS virus lowers the body's ability to fight other infections. It is important to determine whether symptoms are due to ADC or to some other infection since treatment varies for different conditions. AZT has been shown to be beneficial to people with ADC although its effectiveness has only been studied in a small number of patients.
