Opioid Analogue May Slow Spread of HIV in Brain
By Neil Swan , NIDA NOTES Staff Writer Synthetic compounds, or analogues, related to the opiates morphine and heroin can limit the growth of HIV - the virus that causes AIDS - in the brain, according to NIDA-funded research findings. The study with human cell culture suggests that a synthetic compound that binds to the brain's kappa opioid receptors (KOR) may curtail a virus subtype called HIV-1 from spreading in the brains of people infected with the AIDS virus. A drug analogue is a chemical compound that differs slightly from another drug. Although the study was performed in cell culture, the results suggest the possibility that a medication could be developed to prevent a particularly destructive aspect of AIDS - the loss of intellectual capacity. This research is part of a broad range of NIDA-supported studies of the complex relationship between drugs of abuse and AIDS, including studies of the effects of drug use on the body's immune system and, subsequently, on infection and disease progression. The study also provides an example of how research on drugs of abuse can have benefits in other areas of medicine.

Dementia is not a disease itself, but rather a group of symptoms that are caused by various diseases or conditions. Symptoms can also include changes in personality, mood, and behavior. In some cases, the dementia can be treated and cured because the cause is treatable. Examples of this include dementia caused by drugs or alcohol, or hormone or vitamin imbalances. In some cases, although the person may appear to have dementia, a severe depression can be causing the symptoms. This is known as pseudo-dementia (false dementia) and is highly treatable. In most cases, however, dementia cannot be cured.

AIDS dementia complex: A brain disorder that occurs in people with AIDS that causes the loss of cognitive capacity, affecting the ability to function in a social or occupational setting. The exact mechanism by which HIV triggers the AIDS dementia complex has not been determined, but it may result from HIV infection of cells in the brain or from an inflammatory reaction to such an infection. Significant Increase in Prevalence of AIDS Dementia Complex Accompanies Increase in Survival in HAART Era
Results of a number of clinical studies show that HAART has significantly slowed the progression of HIV disease. HAART has also produced significant declines in the incidence of HIV-related opportunistic infections. Finally, HAART has significantly increased survival rates in people with AIDS. Despite the impact of HAART in these areas, there are few data available concerning the natural history of AIDS dementia complex (ADC) in the era of HAART. Although incidence of ADC has declined since HAART became widely available, this decrease could be less significant than for other AIDS-associated OI and even could lead to an increased incidence of ADC. The current study, conducted in multiple centers in Australia, evaluates the impact of HAART on survival following ADC and the prevalence of ADC at a population level through examination of routine AIDS surveillance data in Australia in the period 1993-2000.
Australian AIDS notification data in the period 1993-2000 were examined. In order to examine the impact of HAART, two periods of AIDS diagnoses were chosen: pre-HAART (1993-1995) and HAART (1996-2000). Median survival was based on Kaplan-Meier estimates, with examination of factors influencing survival in a Cox proportional hazards model.
