An adjustment disorder is a debilitating reaction, usually lasting less than six months, to a stressful event or situation. It is not the same thing as post-traumatic stress disorder (PTSD), which usually occurs in reaction to a life-threatening event and can be longer lasting. Description
An adjustment disorder usually begins within three months of a stressful event, and ends within six months after the stressor stops. There are many different subtypes of adjustment disorders, including adjustment disorder with:
  • Depression Anxiety Mixed anxiety and depression Conduct disturbances Mixed disturbance of emotions and conduct Unspecified.
Adjustment disorders are very common and can affect anyone, regardless of gender, age, race, or lifestyle. By definition, an adjustment disorder is short-lived, unless a person is faced with a chronic recurring crisis (such as a child who is repeatedly abused). In such cases, the adjustment disorder may last more than six months.
An adjustment disorder occurs when a person can't cope with a stressful event and develops emotional or behavioral symptoms. The stressful event can be anything: it might be just one isolated incident, or a string of problems that wears the person down. The stress might be anything from a car accident or illness, to a divorce, or even a certain time of year (such as Christmas or summer).

What is an adjustment disorder?
An adjustment disorder is defined as an emotional or behavioral reaction to an identifiable stressful event or change in a person's life that is considered maladaptive or somehow not an expected healthy response to the event or change. The reaction must occur within three months of the identified stressful event or change happening. The identifiable stressful event or change in the life of a child or adolescent may be a family move, parental divorce or separation, the loss of a pet, birth of a brother or sister, to name a few.

Adjustment disorders are a reaction to stress. There is not a single direct cause between the stressful event and the reaction. Children and adolescents vary in their temperament, past experiences, vulnerability, and coping skills. Their developmental stage and the capacity of their support system to meet their specific needs related to the stress are factors that may contribute to their reaction to a particular stress. Stressors also vary in duration, intensity, and effect. No evidence is available to suggest a specific biological factor that causes adjustment disorders.

In psychology adjustment disorder refers to a psychological disturbance that lasts six months or less. Adjustment disorders are caused by specific sources of stress , such as severe personal crisis (divorce, death of loved one, recent abuse) or major unexpected negative events (tornado or fire destroys a person's home). The usual symptoms mimic depression anxiety , or sleep disorder ; however the disturbance disorder is short-term and can usually be treated with counseling or mild short-term medication. If the problem persists past six months, the person may have a more permanent problem, such as a genuine mood or sleep disorder. edit

Adjustment Disorders arise in response to one or more stressors that occur within three months of the onset of the patient's symptoms. The symptoms can be emotional and/or behavioral. If the symptoms last longer than six months due to an enduring stressor (one that continues to occur or whose consequences continue to occur), they are referred to as chronic. If the symptoms last less than six months, the disorder is referred to as acute. There are adjustment disorders with anxiety, with depressed mood and adjustment disorders with a mixture of anxiety and depressed mood. With some adjustment disorders, there is a disturbance of conduct in which the patient becomes rebellious or reckless, and, obviously, some adjustment disorders are characterized by changes in both emotions and conduct.

        Studies examining the prevalence of mental disorders in cancer show that most cancer patients do not meet the diagnostic criteria for any specific mental disorder; however, many patients do experience a variety of difficult emotional responses. In order to effectively match patient needs and treatment interventions, health care professionals must be able to distinguish the periodic difficulties that characterize normal adjustment from more serious mental disorders. Psychosocial distress exists on a continuum (see figure below) ranging from normal adjustment issues through the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) Adjustment Disorders; to a level close to, but below, the threshold (i.e., meets some diagnostic criteria but not all) of diagnosable mental disorders; to syndromes that meet the full diagnostic criteria for a mental disorder (e.g., major depressive disorder).     Adjustment Disorders Adjustment Disorders are conditions with clinically significant emotional or behavioral symptoms which are in response to identifiable psychosocial stress. Examples could include the ending of a romantic relationship, going off to school, retirement, becoming a parent, financial difficulties, or marital problems. The stressor can recur periodically or be continuous. These include seasonal business downturns or living in a high crime area. The stressor might effect only one individual, a particular family, or an entire community as with a natural disaster. The symptoms of Adjustment Disorders develop within 3 months of the onset of the stress. The reaction is characterized by distress in excess of that that would be expected given the nature of the stressor, or by significant impairment in social or occupational functioning. The use of the diagnosis Adjustment Disorder is generally reserved for conditions lasting less than six months from the onset of the stress; however if the stressor or its consequences are enduring the diagnosis is used for longer periods. Adjustment Disorders are subdivided into six subtypes according to the symptoms displayed.

Youth who experience distress in excess of what is expected as a response to a stressor may even experience significant impairment in normal daily functioning and activities ( Institute for Health, Health Care Policy and Aging Research, 2002). Adjustment disorders in children are created by factors similar to those found in adults. In order to be considered and diagnosed as an adjustment disorder, the child’s reaction must occur within three months of the identified event (The Medical Center Online, 2002). Typically, the symptoms do not last more than six months, and the majority of the children quickly return to normal functioning (United Behavioral Health, 2002). Adjustment disorders differ from post-traumatic stress disorder (PTSD) in that PTSD usually occurs in reaction to a life-threatening event and may be longer-lasting (Access Med Health Library, 2002). In 1997, the U.S. Department of Health and Human Services, the Substance Abuse and Mental Health Service Administration and Center for Mental Health Services conducted a Client/Patient sample survey of 8,000 children in mental health facilities.
