André Strohl Description: The commonest form of acquired neuropathy affecting both sexes, most frequently affecting young adults and believed to be a form of autoimmune disease with a delayed hypersensitivity reaction. Commences with progressive muscular weakness of extremities that may lead to paralysis. It spreads rapidly, ascending to involve the cranial nerves. Commences distally but spreads proximally and may involve the bulbar region and the diaphragm. Clinical features are pain in the back and the legs and weakness beginning in the feet and legs and progressing upwards. The respiratory muscles are affected in about half the cases, and this puts the patient in danger. The tendon reflexes are lost.

Overview
The pathologic hallmark of the disease is loss of myelin in peripheral nerves due to an acute and progressive inflammation of unknown cause. It is suggested that it is an autoimmune disease, in which the sufferer's immune system is triggered into damaging the nerve covering. There is some support for this in that half of all cases occur soon after a microbial infection or respiratory or gastrointestinal viral infection. Many cases developed in people who received the swine flu vaccine Peripheral nerves originate in the spinal cord and proceed to their target tissues (mainly muscle, skin and all internal organs). Their most proximal parts emerging from the spinal cord are called nerve roots and the inflammation in most (but not all) typical Guillain-Barr© syndrome cases starts in these roots. Guillain-Barré Syndrome (GBS) is a rare inflammation of the nerves, caused by the patient's body producing antibodies against the peripheral nerves. The syndrome affects each patient differently and so the course of the disease differs for each patient.
How and why one contracts GBS is as yet unknown. The syndrome is typically observed after an infection in the lungs, diarrhoea, surgery, vaccination or childbirth.
The inflammation damages portions of the nerve cell, resulting in muscle weakness, paralysis or sensory disturbances. The damage usually involves loss of the nerve's myelin sheath (demyelination), which slows the conduction of impulses through the nerve.

DESCRIPTION: A group of predominately demyelinating diseases causing acute progressive weakness, usually an ascending paralysis. 30% have respiratory paralysis requiring mechanical ventilation, but complete or substantial recovery is the rule.
  • GBS divided into two disease forms:
    • Demyelinating
      • AIDP (acute inflammatory demyelinating polyradiculoneuropathy): more than 90% cases in Western country, 30% patients have non-specific elevated anti-GM I antibodies
      • Miller-Fisher syndrome: ophthalmoplegia, areflexia, and ataxia of limbs with relatively preserved muscle strength; 90% patients have elevated anti-GQ1b antibodies
    • Axonal loss (poor prognosis)
      • AMAN (acute motor axonal neuropathy): 5% cases in Western country, more than 50% in northern China. Predominant motor involvement, recent Campylobacter jejuni gastroenteritis.
      • AMSAN (acute motor-sensory axonal neuropathy): occurs mostly in adults

A 62-year-old man presented with an acute onset of symmetric upper and lower extremity paresthesias. Neurological examination showed normal motor strength throughout with areflexia and reduced vibratory and proprioceptive sense in all four extremities. A lumbar puncture performed 6 weeks after onset demonstrated elevated cerebrospinal fluid protein with no cells. Electrodiagnostic testing 7 weeks after onset revealed a primarily sensory neuropathy with normal motor conduction and needle electromyographic studies. Symptoms stabilized within 3 weeks of onset and improvement began within 2 months. Except for the absence of motor disturbance, this case appears to resemble Guillain-Barr© syndrome. Guillain-Barr© syndrome (GBS) is typically characterized by an acute onset of bilateral, symmetric, rapidly progressive weakness of the limbs, and sometimes the trunk and cranial innervated muscles, associated with absent or reduced muscle stretch reflexes and an elevated cerebrospinal fluid (CSF) protein without an increase in cells.
