The tumor results from an overproduction of Schwann cells small sheet-like cells that normally wrap around nerve fibers like onion skin and help support the nerves. When growth is abnormally excessive, Schwann cells bunch together, pressing against the hearing and balance nerves, often causing gradual hearing loss, tinnitus (ringing in the ears), and dizziness. If the tumor becomes large, it can interfere with the facial nerve, causing partial paralysis, and eventually press against nearby brain structures, becoming life-threatening. The early diagnosis of an acoustic neuroma is the key to preventing its serious consequences. Unfortunately, early detection of the tumor is sometimes difficult because the symptoms may be subtle and may not appear in the beginning stages of growth. Once the symptoms do appear, a thorough ear examination and hearing test are essential for diagnosis. Computerized tomography (CT) scans and magnetic resonance imaging (MRI) are helpful in determining the location and size of a tumor and also in planning its microsurgical removal. If an acoustic neuroma is surgically removed when it is still very small, hearing may be preserved and accompanying symptoms may go away. As the tumor grows larger, surgical removal is often more complicated because the tumor may become firmly attached to the nerves that control facial movement, hearing, and balance.

What is a vestibular schwannoma (acoustic neurinoma)?
A vestibular schwannoma (also known as acoustic neurinoma , acoustic neurinoma, or acoustic neurilemoma) is a benign, usually slow-growing tumor that develops from the balance and hearing nerves supplying the inner ear. The tumor comes from an overproduction of Schwann cellsthe cells that normally wrap around nerve fibers like onion skin to help support and insulate nerves. As the vestibular schwannoma grows, it presses against the hearing and balance nerves, usually causing unilateral (one-sided) or asymmetric hearing loss, tinnitus (ringing in the ear), and dizziness/loss of balance. As the tumor grows, it can interfere with the face sensation nerve (the trigeminal nerve), causing facial numbness. Vestibular schwannomas can also press on the facial nerve (for the muscles of the face) causing facial weakness or paralysis on the side of the tumor. If the tumor becomes large, it will eventually press against nearby brain structures (such as the brainstem and the cerebellum), becoming life-threatening.

Description
This book is a detailed description of the approach to patients with acoustic neuromas, including diagnosis and the choice of conservative care, radiation or surgical treatment. While there is considerable emphasis on quality of life as well as the philosophy and results underlying all forms of intervention, the main thrust is the surgical anatomy and technique.
The volume is based on the surgical techniques for total microsurgical removal of 600 acoustic neuromas. Multiple stereoscopic photographic pairs in color were taken through the microscope of each of the 600 cases, and 130 stereo pairs have been included to illustrate the various operative approaches and the anatomic variations, each with a rather detailed description. There are also 30 black and white non-stereo photographs of instruments, positioning and operating room arrangements, as well as 30 X-ray copies.
The book is excellent documentation for the future, a highly valued example of the consistent work of a master surgeon and master physician.

Acoustic Neuroma Acoustic neuroma is a benign (noncancerous) tumor that starts in the cells that wrap around the auditory (hearing) nerve in the head. This type of tumor accounts for about 7% of all skull tumors. Early symptoms may include loss of hearing, ringing in the ears, dizziness and vertigo. Diagnosis When the condition is detected early, doctors may order a magnetic resonance imaging (MRI) scan and conduct hearing tests, which may include a special technique to test nerve impulses as they travel to the brain.
