Key findings from this new set of research include: Current TANF proposals that are pending in Congress would do little to address most of these challenges. When the Senate takes up TANF reauthorization, it should take these recent research findings into account and ensure that states have the resources and flexibility to address these issues.

October 16, 1998 RealAudio 2.0 14.4 Part of the MPR Welfare to Work Series VILMA MARTINEZ THOUGHT SHE HAD A FULL-TIME JOB raising five boys. For 10 years she was out of the workforce. Then came welfare reform. Vilma says she needs two jobs to make ends meet. It's 5:00 pm in downtown Sioux Falls - people are heading home after work. But tucked between two buildings on Main Street about a hundred people line up for a meal at the local soup kitchen called "The Banquet." Some are homeless, many are on welfare but are working under the new welfare reform program. However, they say they're still finding it hard to make ends meet. That's why they're here for a meal. They can also get a free flu shot. It's a preventative measure city and county officials believe is important particularly for low income people. The Centers for Disease Control does not list low income as an at risk factor for the shot. But the city does. When asked, people in line all have health care problems and more over problems with paying bills. Roxanne has four children and with recent changes to the state Medicaid program, her children now qualify. She says that and county services help. But when it comes to the hospitals, she's not so favorable.

Gov. Pawlenty has begun using the term "welfare health care" when referring to state-subsidized health insurance programs. Some say he's doing it to sway public opinion away from those programs. (MPR Photo/Mark Zdechlik) In the weeks leading up to his budget announcement, Gov. Pawlenty spoke often of his desire to rein in spending on health care programs. And in doing so, he started referring to the state's subsidized health insurance programs as "welfare health care." In the plan released Tuesday, Pawlenty recommended limiting eligibility for MinnesotaCare, a state subsidized health insurance program for the working poor. Those who are on the program and advocate on behalf of people with low incomes call Pawlenty's "welfare" characterization unfortunate and wrong.
"It is not fair to the rest of the obligations and responsibilities we have to stand by and say, 'It's OK to grow welfare health care at 27 percent, when we're also trying to fund K-12 education and transportation, and so many other priorities that I know Minnesotans want funded as well," Pawlenty said.

Passage of the FIA was the culmination of nearly a year and a half of public debate including an extensive community input process which involved local community, advocates, businesses, and media groups as well as welfare beneficiaries. More than 900 individuals participated in the public meetings and more than 175 came forward to state their concerns. The Family Independence Act is based on the principle that children who receive assistance should never be worse off than they were before reform. The Family Independence Program (FIP), implemented according to law under the new FIA on May 1, 1997, has several components which help to differentiate it from the old program including:
  • A five-year time limit on cash assistance for adults. Children may continue to receive benefits. Work requirements with work supports as follows:
    - Preparation for workforce
    - Child care assistance without time limits
    - Health care coverage without time limits
    - Earned income disregards, which allow

Governors complain that federal law is overly prescriptive and are willing to take less federal money in return for more flexibility.     L. Whitfield Health Service Journal, vol.115, July 14th 2005, p.14-15 Discusses the development of a pan-European public health policy, which is being actively encouraged by the UK government. EU health ministers adopted a public health programme in 2002, which aims to improve public health information and knowledge, to enhance Europe's capacity to respond to health threats, and to address how other policies and activities affect people's health. The EU Commission in 2004 proposed a new health and consumer protection programme to run to 2008. Practically, this should relate to action plans for mental health, alcohol, diet and nutrition.

    The future of the Welfare State is being hotly debated both within the UK and abroad. Social research institutions, think tanks, pressure groups, charities and professional associations have contributed to the debate alongside government opposition politicians and the media. Comment and analysis is found in a wide range of research reports, government consultation and policy papers, academic and trade journals and quality newspapers. Welfare Reform Digest aims to help social policy practitioners and researchers to keep up to date with this flood of literature. We systematically scan the quality press, the British Library's intake of social science books and government publications, and a wide range of trade and academic journals to identify relevant material. We then produce full bibliographic references with detailed abstracts to give both the flavour of the cut and thrust of the debate and an overview of the research literature. The first, pilot, issue covered material scanned from mid June to mid July 1998. The second issue comprised material scanned in March 1999, and new data is now added on a monthly basis.

    Statistics from a Federal government report (Federal Interagency Forum, 2002) indicate a positive trend in poverty reduction. The child poverty rate in 1999 and 2000 was 16 percent, down from a high of 22 percent in 1993, with the decline steepest for African American children in female-householder families. Slightly less than one percent of children resided in households reporting child hunger, although 18 percent lived with "food insecurity," percentages also lower than in previous years. PUBLIC AND FAMILY RESOURCES TO SUPPORT CHILD DEVELOPMENT School Readiness OUT-OF-SCHOOL ACTIVITIES Participation in after-school and summer programs enhances student academic achievement, supports the development of a wide range of competencies, and limits opportunities for delinquent behavior. However, even before their parents were required to work, poor children and adolescents were likely to spend less time engaged in structured activities than other children because their parents lacked the funds for private enrichment programs and poor communities lacked sufficient affordable public programs. Traditionally, they spent more time on their own, frequently watching television, a situation related to lower academic performance.
    ACADEMIC ACHIEVEMENT Information on the school performance of students whose families are transitioning to work results primarily from studies using data collected from parent surveys. Thus, while comprehensive statistics on important indicators are not now known, some potentially useful information is available.

These dramatic caseload reductions bring welfare caseloads to the lowest level since 1968 and the proportion of the total U.S. population on welfare is down to 2.1 percent, the lowest in 37 years.     Although America's antipoverty efforts were rooted in concerns about conditions in rural areas, virtually all current policy debates center on the urban poor. Yet, poverty rates are higher in rural areas, unemployment is greater, and there is less earnings growth than in urban labor markets.
    Two recent conferences hosted by JCPR highlight the effects of welfare reform in rural areas. Although urban and rural areas have seen similar declines in cash assistance caseloads, rural residents face different hurdles in making the transition from welfare to work. The rural economy offers fewer job opportunities and jobs are often less rewarding. The need to tailor the current "make work pay" policies becomes critical.
    "Many low-income people in rural areas live in small towns and work at low-skill, low-wage jobs in manufacturing and service industries," said Greg Duncan, JCPR deputy director. "Opportunities for work are more limited and less financially rewarding than in urban areas."

    We investigate the relationship between welfare reform and health insurance, health care utilization, and self-reported measures of health status for women aged 20-45, using nationally representative data from the Behavioral Risk Factor Surveillance System. We present estimates from both difference-in-difference models (applied to single women and single women with children) and difference-in-difference-in-difference models (using married women and single women without children as comparison groups). We find that welfare reform is associated with reductions in health insurance coverage and specific measures of health care utilization, as well as an increase in the likelihood of needing care but finding it unaffordable. We find no statistically significant effects of reform on health status. Overall, effects are somewhat larger for Hispanics compared to blacks and low educated women.     Welfare reform has resulted in a dramatic decline in welfare caseloads and some have claimed that a significant number of low-income women may be without health insurance as a result. The loss of insurance may reduce low-income, pregnant women's health care utilization, and this may adversely affect infant health. Welfare reform also may affect healthcare utilization and health of pregnant women and infants because of welfare-induced changes in family disposable income, time available for health investments, and levels of stress. In this paper we examine the effect of welfare reform on prenatal care utilization and birth weight of low-educated women and their infants. We find that a 50 percent reduction in the caseload, which is similar to that which occurred in the 1990s, is associated with a zero to seven percent decrease in first trimester prenatal care; a zero to five percent decrease in the number of prenatal care visits; and a zero to 10 percent increase in low birth weight.
