Vitiligo (vit-ill-eye-go) is a pigmentation disorder in which melanocytes (the cells that make pigment) in the skin, the tissues that line the inside of the mouth and nose and genital and rectal areas (mucous membranes), and the retina of the eyes are destroyed. As a result, white patches of skin appear on different parts of the body. The hair that grows in areas affected by vitiligo may turn white. The cause of vitiligo is not known, but doctors and researchers have several different theories. One theory is that people develop antibodies that destroy the melanocytes in their own bodies. Another theory is that melanocytes destroy themselves. Finally, some people have reported that a single event such as sunburn or emotional distress triggered vitiligo; however, these events have not been scientifically proven to cause vitiligo. It is most dramatic when it occurs in a black individual because of the contrast of sheet white with their normal skin tone. There is often mild symmetry to the disorder such that scattered patches may appear on the backs of both hands or around both eyes. It can occur anywhere to just about any extent without explanation. Current thinking is that it is autoimmune and somewhat genetic (30% are familial). Occasionally, it is part of a syndrome that includes dysfunction of glandular organs like the thyroid. Diagnosis The stark white appearance is self-evident. A Wood's lamp exam by the doctor can confirm the loss of pigment, which may be difficult to discern in fair individuals. A biopsy, which is seldom necessary, should show loss of the pigment-producing cells known as melanocytes. Treatment Treatment is cumbersome and not altogether successful. Chemicals that stimulate the pigment-producing melanocyte are used, namely the psoralens. Specific treatment protocols have been developed because of potential for severe sunburns.

VITILIGO At the Derm Update meeting in Montreal last weekend CMV / Vitiligo was discussed several times, and it is not known whether CMV is playing a role in vitiligo and alopecia areata, or is just an "innocent bystander", developing in tissue which is infiltrated with T-cells rather than CAUSING the infiltration. CMV likes to live in proliferating T-cells, and this is one reason why it may be an incidental finding and not the pathogen in vitiligo and alopecia areata. I expect we'll have an answer in a year or two. I have several patients with vitiligo and alopecia areata trying 2.5% cytovene in Moisturel cream and / or famcyclovir 250 mg bid - too early to tell if this will help the skin conditions, and no way to know if this would be a cure or just a maintenance treatment if it works. The CMV / vitiligo reference is: Dr. Pearl Grimes [a respected vitiligo researcher] in a talk to the American Dermatological Association [not published yet] - CMV in 12 of 13 biopsy specimens demonstrated on PCR. Det jag hade var en ovanligare form av psoriasis som inte bildar stora sjok vid leder - vilket är vanligast. Det jag fick kallas för droppformig psoriasis och yttrar sig som små prickar över hela kroppen. Tur i oturen eftersom det ser trevligare ut än vanlig psoriasis. Jag hade bara psoriasis i några månader. Det var en ovanligt stark stress som utlöste det och när allt lugnade ned sig försvann det. Har jag tur så kommer det aldrig tillbaks . . . peppar, peppar.
