General Information It should come as no surprise that bladder control is a topic very few people want to discuss with their physicians. Perhaps more surprising is the reality that many doctors would also prefer not to talk about urinary incontinence (UI). While patients suffering in silence anxiously wait for their doctors to bring up the topic, some doctors remain committed to a "Don't ask Don't tell" mentality. Unfortunately, this lack of communication can lead to more serious medical and psychosocial problems. More than fifty percent of the general female population has some form of urinary incontinence. Women are more commonly afflicted than men but male incidence increases with age and is more of a problem than most people are aware. The Agency for Healthcare Research and Quality estimates that one in four women, ages 30 to 59, has experienced urinary incontinence. If this percentage stays constant, the number of cases will rise dramatically as baby boomers age. Incontinence, which usually can be cured or improved, causes unnecessary social isolation, expense and secondary medical conditions.

Coincident with its 10th anniversary in1998, the Department of Urology has reaffirmed its commitment to Urological excellence in the form of the new Urology Centre, formally opened in December 1998.
The Urology Centre represents the Department's vision for a new approach to patient care and service. The goal is to enable patients to have access to all the services they require, within one location. This emphasis on a smooth passage along a continuum of care is evident in the design of the 2-storey centre. The first level of the Urology Centre houses the outpatient clinics, urodynamic evaluation rooms, cystoscopy suites and the ESWL units. To make the centre a truly one-stop evaluation service for urological problems, all the urological services and the most frequently used other supporting services are physically housed in the centre. A test station for urine and blood tests and a room for counselling patients for erectile dysfunction are but some of the facilities avaliable so that the patients would not have to make multiple visits at various hospital service points. In addition, operation theatres and a full patient recovery ward on the second level allow for the provision of ambulatory surgery and inpatient surgery service. Adjacent to the Operating Theatre Suite is a 40-seat Coonference Room, where meetings and teaching can be held. This room was the venue of the recently concluded 4th Asian Congress in Urology Oncology Workshop where participants were able to watch to live operating demonstration; cabled in from the operating theatres.

Although urology is classified as a surgical subspecialty, because of the wide variety of clinical problems encountered, a knowledge of internal medicine, pediatrics, gynecology, and other specialties is required by the urologist. In recognition of the wide scope of urology, the American Urological Association has identified eight subspecialty areas:

Urology is the branch of medicine concerned with the urinary tract in both genders, and with the genital tract or reproductive system in the male. Urogenital is a word that refers to the urinary and genital organs. The medical specialty of Obstetrics and Gynecology specializes in the reproduction (genital) system of females. Nephrology is the medical specialty concerned with the kidneys. According to the National Kidney Foundation, kidney and urologic diseases affect more than 20 million people. More than 70,000 US adults die each year from kidney failure. The number of people affected by these diseases is expected to grow as the populations of older adults and racial and ethnic minorities, groups disproportionately affected by the diseases, increase. The most serious and debilitating of kidney and urinary tract diseases includes end-stage renal disease (ESRD); kidney stone disease; urinary incontinence; benign prostatic hyperplasia (BPH); interstitial cystitis; urinary tract infection; and polycystic kidney disease.

    Residency Training
The Department of Urology of the Stanford University School of Medicine offers a postgraduate residency training program designed to prepare selected physicians to evaluate, understand, and manage medical and surgical aspects of genitourinary disorders. In addition to providing a rigorous clinical training program, from its inception the Urology program at Stanford has fostered a unique atmosphere of scientific curiosity and endeavor. Through the years, this has manifested itself in ground-breaking basic and clinical research in renovascular hypertension, adult and pediatric urinary tract infection, treatment of urinary incontinence, neurourology, urinary tract physiology, anatomy and cellular biology of the prostate gland, and genitourinary oncology. The large number of Stanford alumni currently holding academic faculty positions and Chairmanships in Urology is testimony to this history. The most important manifestation of this academic atmosphere is, however, the attention to detail and scientific approach to Urology that characterizes the Stanford Urologist, whatever the location and nature of practice.

The urology residency begins with one year of general surgery training followed by four years of clinical urology under the direction of five full-time and three clinical faculty. During the first year, three months are spent rotating on urology and one month is dedicated to research. The second through fifth years are solely devoted to urology. The major focus of the program is the clinical practice of urology and the development of research interests. The full-time faculty includes a fellowship-trained pediatric urologist, a fellowship-trained endoscopist, a fellowship-trained female urologist, a general urologist and a urologic oncologist. These faculty members perform complex adult and pediatric reconstructive cases including bladder reconstruction and continent diversion. There are also many adult and pediatric laparoscopic surgical procedures being performed. There are now procedures being performed via robotic assisted surgery on the da Vinci system. The program has developed an advanced didactic curriculum that includes the use of a state of the art surgical skills laboratory. Residents utilize the laboratory to learn basic surgical, endoscopic and laparoscopic skills. Advanced laboratory skills development follows the basic training. Conferences are held weekly for four hours. Topics include morbidity and mortality, x-ray, uropathology, pre-op, research, and Campbell's review.
