On the face of it, there seems to be little reason to question the ethics behind transplanting organs. Apparently one of the greatest achievements of modern surgery, tens of thousands of people are given a new lease on life through the selfless altruism of others who choose to find hope in the midst of tragedy, literally giving of their own bodies in the effort to save others. But this really is more appearance than fact, because right under the surface lurks a morass of ethical dilemmas and controversies which have threatened to undermine the entire practice of transplanting organs. These problems have only grown in scope as new medical advances have been made in recent years, with little prospect of an easy resolution any time soon. What are the sources of organs used in transplantation? How can we make the procurement system more efficient? Should we pay for organs? Should someone who has already received one transplant be allowed a second? Should alcoholics be given liver transplants? Are transplants really worth the tremendous costs?

Web posted at: 8:33 p.m. EDT (0033 GMT) From Reporter Louise Schiavone WASHINGTON (CNN) An experimental procedure to use umbilical cord blood instead of bone marrow to treat immune diseases is gaining attention from doctors and patients. Derek Kuhs, 7, has spent much of his life in the hospital due to a rare immune system genetic disorder called chronic granulomatous disease, or CGD. "His white cells can't kick certain types of bacteria and fungi, so he's always open to life-threatening infections," said his mother, Gloria Kuhs. She and her husband are pinning their hopes on a transplant of umbilical cord blood. The cord blood, about 3 to 4 ounces taken from a mother's placenta shortly after childbirth, contains stem cells, the building blocks of all blood cells.

Hematopoietic Stem Cell Transplantation Stem cells are the way the body replaces cells that have been damaged or killed. Blood-forming stem cells can be collected from the blood or bone marrow of volunteer donors, frozen, then given to a recipient by intravenous catheter. Hematopoietic stem cell transplantation (HSCT) is the only known cure for many disorders, including certain leukemias and cancers, bone marrow failure syndromes, and genetic diseases. Some evidence suggests the transplanted donor cells may even help directly fight the disease-causing cells. Prior to the stem cell infusion, the recipient is treated with a conditioning regimen (chemotherapy and radiation) to prepare the body to accept the stem cells. Newer conditioning regimens with lower chemotherapy doses allow these procedures to be performed in the outpatient setting, compared to conventional regimens requiring several weeks on an inpatient myelosuppression unit, and often the toxicity is reduced. Your hematologist-oncologist typically will make the initial referral to a stem cell transplant specialist to find out if HSCT is appropriate treatment for your medical condition.
