What is a toothache?
"Toothache" usually refers to pain around the teeth or jaws. In most instances, toothaches are caused by tooth or jaw problems, such as a dental cavity, a cracked tooth, an exposed tooth root, gum disease, disease of the jaw joint (temporo- mandibular joint), or spasms of the muscles used for chewing. The severity of a toothache can range from chronic and mild to sharp and excruciating. The pain may be aggravated by chewing or by cold or heat. A thorough oral examination, which includes dental x-rays, can help determine the cause, whether the toothache is coming from a tooth or jaw problem. Sometimes, a toothache may be caused by a problem not originating from a tooth or the jaw. Pain around the teeth and the jaws can be symptoms of diseases of the heart (such as angina or heart attack ), ears (such as inner or external ear infections), and sinuses (air passages of the cheek bones). For example, the pain of angina (inadequate supply of oxygenated blood to the heart muscle because of narrowing of the arteries to the heart) is usually located in the chest or the arm. However, in some patients with angina, a toothache or jaw pain are the only symptoms of their heart problem. Infections and diseases of the ears and sinuses can also cause pain around the teeth and jaws. Therefore, evaluations by both dentists and doctors are sometimes necessary to diagnose medical illnesses causing "toothache."

Causes, incidence, and risk factors: Many TMJ-related symptoms are caused by the effects of physical and emotional stress on the structures around the joint. These structures include the muscles of the jaw, face, and neck; the teeth; the cartilage disc at the joint; and nearby ligaments, blood vessels, and nerves. For example, daily stress can lead you to clench and grind your teeth, both during the day and at night while you sleep. Clenching means you tightly clamp your top and bottom teeth together, especially the back teeth. The stressful force of clenching causes pressure on the muscles, tissues, and other structures around your jaw.

Treatment: Simple, gentle therapies are usually recommended first. If those don't work, mouth guards and more aggressive treatments may be considered. Surgery is generally considered a last resort. Fortunately, there are many steps you can take at home long before that point. Try massaging the various muscles that may be involved. Probe all of the muscles of the face, shoulders, and back of the neck. (Avoid the area around the throat.) Press on the muscles to identify extremely painful points. Massage the painful spot with hard, slow, short strokes. Do this several times a day until the muscle is no longer painful when pressed. To massage the masseter muscles on each side of your jaw, place your thumb inside your mouth and squeeze the thick muscle in your cheek (toward the back of your mouth) with your fingers. To get at the harder-to-reach jaw muscles inside your mouth, use your index finger to probe for tender areas behind the teeth, and use the finger to massage these spots. Here are some other steps to consider:
  • Maintain good posture while working at a computer, watching TV, and reading. Take frequent breaks to relieve stressed muscles.

Although TMJ is difficult to cure, there are many things you can do to help alleviate the symptoms. Steps: Don't overuse your jaw. Avoid hard foods. Try over-the-counter anti-inflammatory medications. Some TMJ sufferers find ibuprofen especially effective. Take a stress management course or listen to relaxation tapes. TMJ is sometimes caused by and always aggravated by stress. Practice stretching exercises designed especially for the shoulders and neck. This will help reduce some of the tension in your upper body. Stand tall. Good posture helps your jaw to sit properly. Avoid spending hours over the computer. Holding your neck stiff aggravates TMJ.

How to Use Shiatsu for TMJ
Temporo-Mandibular Joint Syndrome (TMJ) is a collection of symptoms that includes a clicking or grinding sound when you open and close your jaw and is caused by a malfunction of the joint where the mandible (lower jaw) meets the skull. Shiatsu assists and promotes the healing process by releasing tension and muscle spasms in the jaw and provides an overall stress release as it calms the nervous system. Steps: Feel the tension begin to release as you move your jaw from side to side, clench your teeth and open wide to loosen the muscle. Begin by pressing both sides of your jaw with the heels of your hands. Press the same point again and begin with your mouth open so as to position the pad of your index finger in the middle of the muscle. Close your mouth and press firmly until you feel a tolerable pain and breathe deeply to promote relaxation. Relieve facial spasm and jaw pain with the pressure point found in a hollow just below the earlobe. Use your middle finger and press toward your jaw.

Temporomandibular joint disorder is an acute or chronic inflammation of the temporomandibular joint (TMJ) the joint connecting the lower jaw to the skull). The disorder and resultant dysfunction results in significant pain and impairment for many. There have been multi-factorial approaches gleaned out of thousands of clinical studies. These have been based mostly on empirical evidence. Attempts in the last decade to suggest surgical treatment modalities based on MRI and CAT scan parameters now receive less attention. These techniques are reserved for the most recalcitrant cases where other therapeutic modalities have changed. Exercise protocols, habit control, splinting, or more recently neuromuscular dentistry should be the first line of approach, leaving oral surgery as a last resort. Certainly a focus on other possible causes of facial pain and jaw immobility and dysfunction should be the initial consideration of the examining oral surgeon or well-trained health professional.
