The term "TMJ" is the acronym for "TemperoMandibular Joint". This is the ball and socket joint that allows the lower jaw to open, close and move sideways when chewing and speaking. Everyone, of course, has two of them, and they are located about one centimeter in front of the ears. They are the only joints in the head, and if something goes wrong with either one of them, you can have real problems. MPD vs. Intra-articular Disease
One of the most important concepts to understand is the basic difference between true interarticular disease and the myofascial pain complex of symptoms which the lay press tends to erroneously label as TMJ. These two entities can, and often do, coexist but they are not synonymous. In patients where both are present it is almost always helpful and desireable to treat the MPD first and then address the interarticular pathology. The most notable exception to that treatment sequence is the onset of acute interarticular disease in a previously healthy joint.
Surgical vs. Non-Surgical Treatment
For MPD the majority of treatment options are non-surgical however, some consider brisement used for trismus secondary to immobilization syndrome a surgical procedure. Warmth, massage, physical therapy including range of motion exercises, non-steroidal anti-inflammatories reduction in parafunctional activity and patient education form the bulk of the treatment protocols. Many practicioners believe in the construction of intraoral "splints" of a variety of types to treat parafunction. Some purport to treat interarticular disease with these devices. The ability of such devices to treat interarticular disease is not supported by the scientific and orthopedic literature. Their efficacy in the treatment of muscle spasm/myofascial pain is perhaps a little higher but often no better than what can be obtained through the utilization of over the counter rubber mouthpieces such as are provided for high school athletes in contact sports.

open Any disruption or disorder to these joints could lead to a medical and dental problem known as TMJ dysfunction.
There are a wide variety of signs and symptoms indicative of TMJ dysfunction. Typically, a TMJ dysfunction diagnosis is usually made after the patient complains of tenderness in the jaw muscles, multiple headaches, and/or dull, aching pain in the face, but symptoms may also include pain in the sinuses, ears, eyes, teeth, neck muscles, and shoulders. Clicking and grating in the jaw joints, inability to open or close the mouth freely, and difficulty in chewing and swallowing are also often present. All of these pain and dysfunction symptoms can be temporary, chronic (continuing for longer than six months), or intractable (never-ending). TMJ dysfunction can have a variety of causes
and is believed to result when the chewing muscles and jaw joints
do not work together correctly.

If you do, you're hardly alone. The American Dental Association estimates that 10 million Americans suffer from the condition, whose symptoms may range from mild to debilitating. The epicenters of TMJ disease are the body's two temporomandibular joints, located just in front of the ears. The joints connect the temporal bone of the skull and the lower jaw, also known as the mandible. Dentists regard the temporomandibular joints as the most complex in the body. They enable the jaw to move in multiple directions, opening and closing like a hinge and gliding back and forth like a sliding door. Healthy joints are extraordinarily flexibility and strong enough to withstand the enormous pressure and wear and tear of chewing food. TMJ disease, which is considered a form of arthritis, can develop when the joints move out of alignment. The most common cause is a "malocclusion," or bad bite. When teeth are crooked or don't mesh, they cannot grind food smoothly. This misalignment puts extra pressure on the joints and, not surprisingly, many people with TMJ disease have difficulty eating or feel pain when chewing.     Introduction It is estimated that approximately 70 to 90% of patients with facial pain have either a primary or secondary muscle disorder and that pain in these patients can be alleviated with biofeedback(1). Facial pain usually results from acute overstretching of muscles, whiplash, muscle fatigue, oral surgery or immobilization of muscle or as a consequence of a muscle-bracing oral habit, such as clenching and/or grinding and malocclusion. The pain experienced most often is described as a dull deep ache that is usually constant and active for months, sometimes for years. The pain is usually more intense on awakening, and is frequently unilateral or one-sided. Often, clicking or popping sounds originating from the temporomandibular joint (TMJ) area are reported. Mandibular movements are limited because patients have difficulty opening their mouth. The jaw often deviates to the affected and painful side of the face. The pain may radiate into the temple muscles and down into the sternocleidomastoid muscle and splenius capitis muscles as well as to the zygomatic process.
