Why Does TMJ Occur?
The temporo-mandibular joint is the joint where your lower jaw bone is connected to the temporal bone of your skull. It is covered with a thin layer of cartilage and separated by a small disk. This joint is almost constantly in use as you eat, speak and swallow. Pain is experienced in this joint due to wear and tear; stress that expresses itself in grinding or clenching the teeth; misalignment of the upper and lower jaws; and, sometimes, arthritis.

What are the most important symptoms I should be concerned about?
Pain, and or Jaw "locking" episodes.
What is a "locking" episode?
A "locking" episode can occur during opening or closing movement. What happens is that the patient experiences an interruption of jaw movement - a "catch" or a "stop", and in order to complete the movement must jiggle, or somehow, self manipulate the jaw.
Why does it happen?
Referring back to my anatomy lesson, and in the simplest of terms, what is happening within the Joint is that the Articular Disk which rides on top of the Condyle Head is getting stuck in the wrong place and is preventing the Condyle Head from moving.
If I can jiggle my jaw and reduce the dislocation, why should I be concerned?
Because each time it happens more damage is occuring to the tissues in the Joint, and the tissues controlling the Articular Disk. As a consequence there is the risk that if the problem is not addressed by appropriate treatment, one day you will be unable to reduce the dislocation yourself, and you will require an emergency visit to a TMJ practitioner, if one is available in your community, or an oral surgeon. In severe cases, reduction can only be accomplished under general anesthesia.

TMJ Overview
Temporomandibular joint (TMJ) syndrome or TMJ joint disorders are medical problems related to the jaw joint. The TMJ connects the lower jaw to the skull (temporal bone) under your ear. Certain facial muscles control chewing. Problems in this area can cause head and neck pain, a jaw that is locked in position or difficult to open, problems biting, and popping sounds when you bite. The TMJ is comprised of muscles, blood supplies, nerves, and bones. You have 2 TMJs, one on each side of your jaw. Muscles involved in chewing (mastication) also open and close the mouth. The jawbone itself, controlled by the TMJ, has 2 movements: rotation or hinge action, which is opening and closing of the mouth, and gliding action, a movement that allows the mouth to open wider. The coordination of this action also allows you to talk, chew, and yawn.
  • If you place your fingers just in front of your ears and open your mouth, you can feel the joint and its movement. When you open your mouth, the rounded ends of the lower jaw (condyles) glide along the joint socket of the temporal bone. The condyles slide back to their original position when you close your mouth. To keep this motion smooth, a soft disc lies between the condyle and the temporal bone. This disc absorbs shock to the temporomandibular joint from chewing and other movements. Chewing creates a strong force. This disc distributes the forces of chewing throughout the joint space.

What is the Temporomandibular Joint?
The temporomandibular joint (TMJ) is a joint that slides and rotates just in front of your ear, consisting of the temporal bone (side and base of the skull) and the mandible (lower jaw). Mastication (chewing) muscles connect the lower jaw to the skull, allowing you to move your jaw forward, sideways, and open and close. The joint works properly when the lower jaw (mandible) and its joint (both the right and left) are synchronized during movement. Temporomandibular Disorder (TMD) may occur when the jaw twists during opening, closing or side-motion movements. These movements affect the jaw joint and the muscles that control chewing. What is Temporomandibular Disorder?
TMD describes a variety of conditions that affect jaw muscles, temporomandibular joints, and nerves associated with chronic facial pain. Symptoms may occur on one or both sides of the face, head or jaw, or develop after an injury. TMD affects more than twice as many women than men and is the most common non-dental related chronic orofacial pain. What causes TMD?

They all suffer from what has come to be known as temporomandibular joint (TMJ) disorders. These disorders can have a variety of causes and are believed to result when the chewing muscles and jaw joints do not work together correctly. In many cases, TMJ disorders can be successfully treated. HOW THE CHEWING MUSCLES AND JAW JOINTS WORK
The structures that make it possible to open and close the mouth are very specialized and work together when you chew, speak and swallow. These structures include the muscles, ligaments, bones and joints of the jaw. Five pairs of muscles allow you to open and close your mouth. They also control forward, backward, and side-to-side movements of the lower jaw. Also involved in these movements are the temporomandibular joints. Each of these important joints has two sections, connected by a disk, that make possible the hinge and gliding actions needed to open the mouth widely (Fig. 1).
