W ithin each of these specialties there are very different approaches depending on the particular practitioner seen and his or her specific training and experience. This leads a great deal of the time to differing diagnosis and very different opinions about therapy from each professional seen. Patients can become very confused about which opinion or therapy recommendation is best for them.   • Intra-articular disease - this is the damaging changes that occur to the jaw joint, such as degenerative joint disease (DJD), unusual growth and development, and other rarer causes. These are changes to the bony and soft tissue components - changes that for the most part are irreversible. Myofascial pain disease (MPD) - in a very simple explanation - the muscular and ligamental pains that occur due to overuse and tearing. This is quite often reversible.

What kind of doctor handles this problem? Dear Reader, The temporomandibular joint (TMJ) itself is not a problem, it is the joint where the lower jaw, called the mandible, and the temporal bone of the skull meet. We use this joint more frequently than most of the other joints in our bodies. Every time we chew, talk, bite down, or swallow we put the TMJ to work. Injury to the joint, known as TMJ dysfunction, is caused by two main events: overuse of the joint, and favoring one side of the mouth when chewing and biting down. Overuse usually applies to clenchers and grinders - people who tend to clench their jaws tightly together for a variety reasons, primarily stress. Some people clench and grind their teeth while sleeping and are totally unaware of the behavior. Gum chewers, who constantly work the joint, tend to be susceptible to TMJ dysfunction . By chewing on only one side of the mouth, undue stress is put on that side's temporomandibular joint. This does not allow for even distribution of force applied to the joint and pain will eventually develop. The same happens to people who have "improper bites," meaning their teeth do not fit together correctly causing one side of the mouth to close first when biting down and chewing. You can locate the temporomandibular joint by placing your finger on that triangle of skin in the front of your ear and then moving it a teensy bit forward. You should be able to feel a small depression between bones there. Now open and close your mouth a few times to feel the motion of the joint. If this hurts, you most likely have TMJ dysfunction.

    What is the point of this website? During the thirty years I have practiced as a restorative dentist, I have been fascinated by the habit of some individuals to grind and clench their teeth. This phenomenon is peculiar for several reasons:
    • The flattened teeth of our ancestors indicate that compression and grinding of teeth has existed for centuries. Even today, it is of epidemic proportions because in most cases it is not diagnosed, not well understood, and consequently not treated properly.
      Thirty percent of the affected patients are not aware that they are damaging themselves, as this habit tends to be subconscious.
      Compression and grinding of one's teeth simultaneously applies undue pressure to the bone supporting the teeth and to the temporomandibular joint (TMJ), where the mandible (lower jaw) hinges with the skull. In my opinion, this is responsible for the majority of problems related to this joint.
      The physical signs of compression are not recognized by the majority of the dental profession. A patient may be chronically damaging him or herself by compressing and grinding, but may nonetheless be quite comfortable. If this is the case, the signs of damage should be recognized when the patient has his or her teeth cleaned. The problem is that sixty-five percent of the dental profession does not recognize these signs and does not understand the implications.

    I am a Vietnam Era U. S. Air Force Veteran who served from 1962 to 1966 under the maiden name of Larmore. After completing Basic Training at Lackland (January 1963), I was stationed with the 3201st Air Base Wing WAF Squadron at Eglin AFB, Florida. In 1965 I was selected as the Fire Prevention Queen for the base. My last assignment before receiving a medical discharge was in Air Operations at Eglin. Prior to entry into the Air Force, I graduated from Samuel Ready Girls' School, Baltimore, MD. Before that I attended Wicomico County Public Schools including Nanticoke Elementary School from which my mother had graduated. After discharge from the Air Force, I moved to Towson, Maryland and began attending Johns Hopkins University Summer School. There was a slight 9 month interruption along the way.     Temperomandibular Joint (TMJ) Treatment: a Method that Works Temperomandibular Pain Dysfunction Syndrome or TMJ symptoms are quite common. They include local joint pain, ear ache, headache, noises and pain when you chew, and various associated muscle pains. The ability to open the mouth wide may be limited. Ordinary chewing of food may be difficult. In some patients it may be due to the teeth not being perfectly "straight". Treatment is to get a good occlusion or bite. Problem is I see patients with the most perfect bite possible with TMJ, and see patients with terrible occlusion without TMJ symptoms. In other patients I feel that it is the muscle imbalance that is of paramount importance. Correcting the bite may take care of the problem, using mirror biofeedback may also relieve the condition. In TMJ, the person opens his mouth to the side, not in the midline. There may be a noise called crepitus. This is the sound made because the joints are swollen, or the membranes are worn away or displaced. This usually means that the muscles that guide the jaw are not functioning correctly, they aren't balanced. Here correction of the bite is certainly desirable when this is the cause of the muscle imbalance. But there may be another causes. Often patients with TMJ express their stress by biting hard or gnashing their teeth. This may replace clenching the fist or sweating when tense. The more the discomfort the more they bite down or clench. The more they clench, the more the pain. The more the pain, the more the anxiety, the more they bite down, the more nervous and so on. In TMJ the person may grind his teeth in his sleep; this is most upsetting to the partner and to the dentist who has to repair the damage.

    Do you have Temperomandibular (TMJ) pain? If the jaw is painful, take papain/ bromelain proteolytic enzyme tablets , and dissolve them in the mouth, between the cheek and the gums. This reduces joint swelling, hence the pain, and helps the jaw to assume the midline position. Be sure to check the enzyme potency of any enzyme tablets you use - . Don't chew, rest the jaw by supporting it gently with an ace bandage or a bandana - just enough to take the weight off.
