Graves' disease, a condition in which the thyroid gland is hyperactive, is rarely a life-threatening condition. Although Graves' disease may develop at any age and in either sex, it most often affects women 20 to 60 years old. The disease can affect the thyroid gland, eyes, and skin. Unfortunately, the cause of Graves' disease is unknown. Current research, however, suggests that a defect in the immune system may be the underlying cause of the disorder. Normally, the immune system uses antibodies (protein substances that the body makes) to help protect itself against viruses, bacteria, and foreign substances (antigens) that invade the body. In Graves' disease the immune system mistakenly attacks the thyroid gland, eyes, and skin of the lower legs causing the disease's symptoms. The Thyroid Gland The thyroid gland is a butterfly-shaped organ located just in front and on each side of the windpipe just above the breast bone. The gland releases chemical messengers called hormones into the blood. These hormones influence body processes such as:

Your doctor thinks you have thyroid orbitopathy. This is an autoimmune condition where your body's immune system is producing factors that stimulate enlargement of the muscles that move the eye. This can result in bulging of the eyes, retraction of the lids, double vision, decreased vision, and ocular irritation. This is often associated with abnormalities in thyroid gland function (either too much thyroid (Graves' disease) or too little (Hashimoto's thyroiditis)). The eye findings of thyroid orbitopathy may be independent of treatment of your thyroid abnormalities and may not resolve in spite of the fact that the thyroid is now "controlled." These symptoms may be present even if your thyroid has no apparent problems.

We're sick of hearing that there are no harmful effects of phytoestrogens. Phytoestrogens disrupt thyroid function. (Read the British Food Standards Agencies Committee on Toxicity Report Here) Theodore Kay of the Kyoto University Faculty of Medicine noted in 1988 that 'thyroid enlargement in rats and humans, especially children and women, fed with soybeans has been known for half a century'. In fact, thyroid problems associated with soy were also well known to bird-breeders , which is how Soy Online Service first became aware of the goitrogenicity of soy. Well known, but that fact seemed to escape manufacturers of the first commercially available soy formulas. Those formulas were known to cause in goitre in infants and one can only wonder how many other infants were left hypothyroid or suffering from permanent thyroid damage by soy formulas. THYROID EYE DISEASE What is thyroid eye disease? What causes thyroid eye disease? Does everyone with an overactive thyroid develop thyroid eye disease? Are there any other symptoms? ... Will my eyes return to normal? What is thyroid eye disease? Thyroid eye disease often occurs in people who develop an overactive thyroid gland . Swelling of the muscles and other tissues in the orbits causes the eyes to become pushed forward and more prominent. The eyes often take on a more staring appearance. In more severe cases the swelling may cause stiffness of the muscles which move the eyes. This can cause a "squint" to develop and may result in double vision. Occasionally the swelling behind the eyeball may press on the nerve from the eye to the brain and distrupt vision. Thyroid eye disease is also called thyroid opthalmopathy, Graves’ eye disease or dysthyroid eye disease. What causes thyroid eye disease? Most people with thyroid eye disease have or had or will subsequently develop an overactive thyroid gland. In 20% the thyroid eye disease develops in people who do not have an overactive thyroid at the time (may subsequently develop this years later). In 40% thyroid eye disease occurs whilst the thyroid is overactive and in 40% can occur years after the overactive thyroid has been treated successfully. Overactivity of the thyroid gland is usually caused by an "autoimmune condition" This means that cells which normally protect the body from infection develop a "fault" and begin to recognise the thyroid gland as foreign material and attack it. This stimulates the thyroid gland to produce extra thyroid hormones. The attacking process may spill over to the cells behind the eye causing them to swell. It is not yet known why cells develop the fault that causes them to attack the thyroid gland or why only some patients with overactivity of the thyroid develop thyroid eye disease. Thyroid eye disease does appear to be more common in smokers.

Pretibial myxoedema is a form of diffuse mucinosis . The thickened and inelastic skin is filled with excessive hyaluronic acid and chondroitin sulphate. These are acid mucopolysacharides (glycosaminoglycans). The same material is deposited behind the eyes resulting in protrusion and lid lag on movement of the eyeballs.

The patient has a history of hyperthyroidism which was diagnosed 2 years ago and had been managed medically until he developed agranulocytosis on tapazole and had other complications while on methimazole and propylthyiouracil . On presentation to the eye clinic, he was complaining of horizontal binocular diplopia on extreme up-gaze. He was not bothered by the diplopia. He had some complaints of blurry vision in the morning.
PMH/FH/POH: His ocular history was not significant as he had no prior history of ocular surgery, trauma, or ambylopia.
EXAM
  • Best corrected visual acuities: 20/30 OD and 20/20 OS. Pupils: 0.9 LU RAPD OD. Confrontational VF: full OU. See HVF below. EOM: -2 underaction to elevation OD and -3 underaction to elevation OS (see photos). IOP: normal OU Anterior segment: lid retraction OU and lagophthalmos OU. There were corneal punctate epithelial erosions (PEE) inferiorly OU. There was injection over the medial rectus muscles OU.

My name is Glenn Kardel. I have polyglandular autoimmune syndrome type II, frequently referred to as Schmidt's syndrome. The classic manifestations of this syndrome are adrenal insufficiency plus autoimmune thyroid disease and/or insulin dependent diabetes mellitus. I have all three of the above conditions: Addison's disease , hypothyroidism, and type I diabetes.
Addisons and hypothyroidism are treated with oral hormone replacement medications. Type I diabetes is the most serious and difficult to control of the three conditions. Treatment usually involves balancing insulin therapy administered through injections or a pump, diet, and exercise with the goal of maintaining normal blood sugar levels.
After I was diagnosed with Addisons and started taking hydrocortisone, I started to have highly variable blood sugars. I began to have sugars in the 300's and also frequent hypoglycemic episodes. I started searching for information about the interaction between Addisons and diabetes or any special treatment strategies related to the two conditions and was surprised to find almost nothing.
