The Lucile Packard Children's Hospital's Teen Health Van is a mobile clinic that provides free, comprehensive primary health care services to homeless and uninsured adolescents from San Francisco to San Jose. The van, which is in its ninth year of operation, is supported through the Packard Foundation for Children's Health by grants and gifts from foundations, corporations, and individuals. Medical Center Report writer Krista Conger talked with Seth Ammerman, MD, associate clinical professor in the Department of Pediatrics in the Division of Adolescent Medicine and a member of the LPCH staff, about his work on the van. 1. Who visits the Teen Health Van? Ammerman: We provide a medical home for adolescents who often have no other access to care. We perform more than 900 examinations each year, and our return visit rate is more than 60 percent, which is remarkably high for these youth. We see twice as many girls as boys. Many of our patients with serious illnesses such as diabetes or seizure disorders are not getting appropriate treatment or are not taking their medications. In some cases we see kids who've never had any immunizations past childhood. They also often have behavioral and mental health problems such as anxiety, depression and even sometimes psychosis. We frequently see teens with substance abuse problems or who are engaging in risky sexual behaviors. Often one patient has multiple problems, which makes treatment more complicated.

Explore your own attitudes
Studies show that kids who feel they can talk with their parents about sex because their moms and dads speak openly and listen carefully to them are less likely to engage in high-risk behavior as teens than kids who do not feel they can talk with their parents about the subject. So explore your feelings about sex. If you are very uncomfortable with the subject, read some books (see Readings for Parents ) and discuss your feelings with a trusted friend, relative, physician, or clergy member. The more you examine the subject, the more confident you'll feel discussing it.

What is sexual orientation?
Sexual orientation is determined by the sex or sexes you are romantically, physically, emotionally, and sexually attracted to. Heterosexuals are individuals attracted to the opposite sex, homosexuals are individuals who are attracted to the same sex, while bisexuals are people who are attracted to both sexes. Homosexual men are usually referred to as gay while homosexual women are referred to as lesbians.
What if I'm not sure what my sexual orientation is?
Discovering your sexual orientation can sometimes be confusing. Most people don't just wake up one day and decide their sexual orientation. It takes time and its normal not to be sure. Experimentation is natural - as long as you look out for the safety of yourself and others. You may want to date the individuals of the opposite sex or you may decide to date those of the same sex. Exploration doesn't determine your sexual orientation, it just helps to discover your feelings.

The health issues facing teens are unique — and so are many of the web sites geared toward teens and health. If your teen has questions about nutrition, sexuality, fitness, hormones or emotions, steer him or her to one of these teen-oriented sites: www.teengrowth.com and www.kidshealth.com (click on teens). Kids may have questions about health or sexuality that they don’t want to ask their parents. Teengrowth.com can help children understand their anatomy (and that of the opposite sex) without the embarrassment of asking. There is also a comprehensive question-and-answer section with topics ranging from bad breath to body piercing. The "emotions" section educates teens on self-esteem, emotional abuse, stress and suicide. Kidshealth.com has a separate section for parents, kids and teens. Teens will find articles on the basics of health and an entire section on sexual health. This site also has information on safety and a section about health problems often faced by youths.

Teens in Crisis Want Answers By Rabbi Yehudah Fine When I surfaced from the dark world of Times Square and the night world of the five boroughs of New York City several years ago, I set out to talk to America's teens and their parents about how they truly could once again reclaim the dignity and intimacy in their lives. The street was a world where bodies washed up on the shores of a broken American dream. I made a promise to all those kids I lost that I would move back upstream and engage with meaning as many kids as possible before they too dropped into the endless river of night. The hardcore truth is that we ignore teens in our society today. Ignorance is not bliss. Ignorance of their struggles leaves them at risk and leaves them to fend for themselves in their most desperate hours. There is no comfort in knowing that when kids are raped, abused, contemplate suicide or are self medicating themselves that they hold fast to their wounds and keep them as secrets. The secret keepers learned long ago that they could never turn to the adult world for help. And for those who do not have hidden trauma in the walls of their heart, they too are certain about one thing in their lives. When it comes to dealing with important issues, their life experience has taught them that adults simply are not available to talk with. There is nothing more tragic than knowing that a child in desperate trouble truly believes there is nowhere to turn to for help. Over the past two years, I have been out on the road and informally surveyed nearly 8,000 high school students across the country. My interest was to get a sense of the backdrop of their lives. What were the issues they thought about? What were their concerns? How aware were they of their peers' struggles? And who did they feel they could talk to about these issues? In other words, the quest was to uncover not only the major issues in their daily lives but also to discover who was talking to them.
