Cuttack Though occasional night emission is a natural physiological process for men who abstain from coitus, due to overvalued ideas about loss of semen and its ill consequences many young men get worried in which case scientific information and counselling is sufficient to allay their anxiety. But there are some men who suffer from Dhat syndrome due to `excess night emissions everyday or every other day, have many vague somatic symptoms such as fatigue, weakness, loss of appetite, guilt, anxiety and fear. Some of them have impotence and premature ejaculation and dread marriage. This condition is observed in the Indian sub-continent; it is a clinical reality which requires treatment. Sex education, psychotherapeutic counselling along with anxiolytics, antidepressants or antipsychotics are not enough to help them. Experience from the studies on the efficacy of clomipramine on treatment of premature ejaculation encouraged the present study. Fifty patients who attended a sex clinic with the problem of night emission (frequency more than 10 times/month) and/or associated with Dhat syndrome and sexual dysfunction were taken up. They were treated with conventional counselling and 10-25 mg of clomipramine in a single evening dose for a period of 3 months.

Since child birth my wife has the desire to make love but can not reach orgasam. She claims to feel herself getting close but then then it stops. Oral and manual stimulation which worked before does not work now. Other symptoms are pain during initial penial entry and pain the next day. We have sinced stopped making love and she services me by masturbation only, about once a week. This has been going on for about 3 years now and I think we need help. Please review and offer any help. Thanks.
There are couple possibilities messing her up:
1. Her vagina was artificially narrowed by sawing episiotomy or birth tears right after she completed her vaginal delivery. Some crazy doctors intentionally do it in an attempt to increase the patient's sexual pleasure!

Abstract
A hypothesis is presented for eating disorders that contends that these syndromes together with the phenomenon of the pursuit of thinness are manifestations of female intra-sexual competition. It is argued that the present-day environment of Western coutries presnts a range of conditions that have led to the overactivation or disruption of the archaic female sexual strategy of maximising 'mate value'. The hypothesis deals with the ultimate level of causation and is therefore compatible with a range of theories dealing with the proximate level of causation. Department of Pediatric Urology, "Aghia Sophia" Children's Hospital, Athens, Greece Key words: Ambivalent genitalia, Hermaphroditism, Pseudohermaphroditism, Sexual differentiation * The views expressed in this article have been proposed by the author to the Health Care Office of the European Association of Urology as guidelines for the management of intersex patients. o ne of the most challenging problems a urologist has to face is the diagnosis and management of patients with sexual differentiation disorders. Such disorders most often manifest themselves by the abnormal appearance of the external genitalia when the baby is born, casting doubt on the baby’s gender and causing a lot of anxiety on the part of the family. Problems of ambiguous genitalia should, in principle, be managed by a team of specialists in order to reach a prompt and correct diagnosis. Urologists have an important role is this group and should work in close cooperation together with neonatologists, pediatric endocrinologists and geneticists, in order to achieve the best possible management of these patients. Abstract In 1973, the Board of Trustees of the American Psychiatric Association voted to delete homosexuality as a mental disorder from the seventh printing of the second edition of the Diagnostic and Statistical Manual of Mental Disorders, or DSM-II. Twenty-three years later, the inclusion of diagnostic categories for Gender Identity Disorder and Transvestic Fetishism in the fourth edition of the DSM continues to raise questions of consistency. In this paper, issues of gender identity and sexual orientation are compared in light of current definitions of mental disorder. Introduction Although human sexual orientation and gender identification represent distinct phenomena and are considered largely orthogonal, they are intricately intertwined in medical theory and social consequence. Since Magnus Hirschfeld (1910) coined the term "transvestite," studies have revealed that most gays and lesbians do not crossdress and most crossdressers are heterosexual. Nonetheless, sexual orientation and gender identity share many common elements pursuant to the definition of mental illness, including theories of etiology, social stigma, cross-cultural occurrence, and perceptions of distress, disability, and disadvantage. These parallel elements raise questions of consistency in light of the deletion of homosexuality as a mental disorder from the seventh printing of the second edition of the Diagnostic and Statistical Manual of Mental Disorders, or DSM-II, in 1973. If homosexuality and transgender identity or expression were classified as mental disorders for essentially the same reasons, then it remains unclear how those reasons uphold the continued inclusion of diagnostic categories for Gender Identity Disorder and Transvestic Fetishism in the DSM-IV today.
