Although this is, strictly speaking, not a sexual dysfunction, it is a disorder that can severely disrupt the sexual relationship of a couple. ISD is the persistent and pervasive inhibition of sexual desire. ISD is present if there is both a low rate of sexual activity and a subjective lack of desire for sexual activity; desire here includes sexual dreams and fantasies, attention to erotic material, awareness of wishes for sexual activity, noticing attractive potential partners, and feelings of frustration if deprived of sex. The DSM IV describes Hypoactive Sexual Desire Disorder as:
  • Persistent or recurrent deficient (or absent) sexual fantasies and desire for sexual activity. The judgment of deficiency or absence is made by the clinician, taking into account factors that affect sexual functioning, such as age, sex, and the context of the person's life."

Disorders
Sexual disorders include problems of sexual identity, sexual performance, and sexual aim. There are three major catergories of sexual disorders: sexual dysfunctions paraphilia , and gender identity disorders
Sexual Dysfunctions Sexual dysfunctions prevent or reduce an individual's enjoyment of normal sex and prevent or reduce the normal physiological changes brought on normally by sexual arousal. These dysfunctions can be classified by the phase of the sexual cycle in which they occur. It is important to keep in mind that the diagnosis of sexual dysfunction is made only when the disability persists. Any of them could occur occasionally or be caused by a temporary factor such as fatigue, sickness, alcohol, or drugs.
The Desire Phase There are two types of dysfunctions that can occur during the desire phase. One is hypoactive desire , which is basically a disinterest in sexual activity. It results in a complete or almost complete lack of desire to have any type of sexual relation. This can often result in the participation in intercourse as a simple marital duty. The second type is an aversion to sex This is different from simple hypoactive sexual desire in that sexual activity actually repulses the person or makes them unusually apprehensive. This is most often the result of a traumatic sexual experience, such as molestation as a child or rape.

Mental Health Disorders Sexual Disorders (excluding paraphilias) In this section we consider the kinds of problems that can arise in what most societies and most individuals consider as "normal" heterosexual relationships. In the section on paraphilias (literally, abnormal attractions) we consider diagnoses of sexual problems that involve socially disapproved forms of sexual behavior, and in the section on gender identity disorders we cover problems that arise when people are not satisfied with their biologically determined sexual identity. The importance of sex is so great that it is hardly necessary to say, in the case of each possible diagnosis, that the symptoms must "cause clinically significant distress or impairment in social, occupational, or other important areas of functioning." This DSM-IV requirement is almost guaranteed to apply to any substantial and persistent difficulty in an area so critical to human self-images and relationships.

It's been said that adolescents are "hormones with feet." And, according to the most comprehensive US sex survey since the 1948 Kinsey Report*, more than 40% of women and 30% of men aged 18 to 59 regularly have no interest in sex, or have difficulty getting aroused, or can't have an orgasm or suffer from pain during intercourse, Comfort with sex and easy, successful sexual performance between partners are fairly rare. GB Communications Psychological definition of Philias
Taken from DIAGNOSTIC AND STATISTICAL MANUAL OF MENTAL DISORDERS (Third Edition) published by the American Psychiatric Association, 1987. Pgs 279-281 Sexual Disorders. SEXUAL DISORDERS
The Sexual Disorders are divided into two groups. The Paraphilias are characterized by arousal in response to sexual objects or situations that are not part of normative arousalactivity patterns and that in varying degrees may interfere with the capacity for reciprocal, affectionate sexual activity. The Sexual Dysfunctions are characterized by inhibitions in sexual desire or the psychophysiologic changes that characterize the sexual response cycle. Finally, there is a residual class, Other Sexual Disorders, for disorders in sexual functioning that are not classifiable in any of the specific categories.
PARAPHILIAS
The essential feature of disorders in this subclass is recurrent intense sexual urges and sexually arousing fantasies generally involving either (1) nonhuman objects, (2) the suffering or humiliation of oneself or one's partner (not merely simulated), or (3) children or other nonconsenting persons. The diagnosis is made only if the person has acted on these urges, or is markedly distressed by them. In other classifications these disorders are referred to as Sexual Deviations. The term Paraphilia is preferable because it correctly emphasizes that the deviation (para) lies in that to which the person is attracted (philia).

The clinical practice of Dr. David B. Adams is located in The Medical Quarters in the northside of Atlanta at the junction of Scottish Rite, Northside and Saint Joseph's Hospitals. Dr. Adams consults to occupational medicine, surgeons, nurse case managers, insurers and employers regarding the psychological impact of work-related injury and the role of psychological factors in short- and long-term disability. SEXUAL DISORDERS A variety of organic (physical) and functional (psychological) sexual disorders may arise following injury. The boundary between what is physical and what is psychological becomes blurred. Sexual Dysfunctions are characterized by disturbance in sexual desire and the physical changes that characterize the response cycle of sexual excitation and sexual activity. They are dysfunctions when they case personal distress and/or interpersonal (social) difficulties. They can arise in the area of desire, excitement, orgasm, and/or resolution. They can be of lifelong or acquired duration and can be generalized to many situations or situational and occur only in specific situations. They can be due to psychological factors or due to a combined physical and psychological problem.

Classification of Female Sexual Disorders
The classification of female sexual disorders has undergone several revisions and continues to evolve as knowledge expands. Several useful classification systems have been created, but no one system stands as the hard-and-fast rule or gold standard. The following section discusses two of the most widely known and used classifications.
DSM-IV Classification
DSM-IV: Diagnostic and Statistical Manual International Statistical Classification of Diseases and Related Health Problems-10 ICD-10 ), published in 1992, contains a classification system for female sexual disorders that is based on the Masters and Johnson and Kaplan linear model of the female sexual response. The DSM-IV The DSM-IV categorizes female sexual disorders as follows:
  • Sexual desire disorders

The nature of any sexual disorder is usually complex and may change during different life stages. A number of psychological and sociological variables may affect female sexual function, including emotional and psychological well-being, the aging process, menopause, the presence of diseases, and the use of certain medications (see box). Prevalence of HSDD increases with age, particularly after age 60, but is linked more closely to age than to menopause status.
