PHLEBITIS
Dawn at Altered Perceptions mentioned flea bites in this blog on search phrases. It reminded me of something. Between my steamship and railroad careers I did a two year stint in trucking. Signed on as a salesman, because I wanted to be able to drink on a company dime, and immediately got transferred to Charleston to run the terminal there. Shit happens like that in transportation. You never know when you're the little Dutch Boy with a big thumb and a dike needing fingering.
Anyway, I had a husband-wife team, about fifty-five years old, the two, and the husband got sick. When I asked the wife where her husband was, she said "He's in the hospital. They say he has "flea-bite-us", Kim, and I can't understand it! We don't even own a dog!"
I have many more trucking stories, because I ran trucking operations in Savannah, Charleston, Memphis, and Nashville as a railroad guy (don't ask, we own a trucking company, and you do double-duty), but you're nice people, and I don't want to offend you needlessly. In other words, give me a reason. Any reason will suffice.

The Teflon intravenous catheter: incidence of phlebitis and duration of catheter life in the neonatal patient

Children's Hospital, Oakland, California. The risks and benefits of intravenous therapy for neonatal patients have not been adequately evaluated. A study was conducted to assess the duration of life of the Teflon catheter used for intravenous therapy in neonates, the incidence of phlebitis associated with this catheter, and the factors which contribute to catheter life and phlebitis. Seventy-two neonates undergoing intravenous therapy with the Teflon catheter were studied. Study results indicated a mean catheter life of 30.1 hours, with 33 of the 72 catheters lasting less than 24 hours. Erythema occurred in 32 cases and phlebitis occurred in 5 cases. In an article on phlebitis in the June number I am reminded to say that for forty or more years I have found belladonna to be an excellent and, so far as I know, the best remedy for external application. In puerperal phlebitis I always, after elevating the leg, apply a plaster of the solid extract of belladonna. The specific effects, flushing of the face, dilation of the pupils, dryness of the mouth, etc., will quite often be a signal to withdraw the application for a few hours to again apply it when these symptoms subside. After 24 to 48 hours remove the belladonna and apply a flannel bandage as snugly as can be well borne and upon this pour hot alcohol every hour. In a recent hospital experience my patient had a crural phlebitis following an ovariotomy, and was treated in this manner while a patient in an adjoining room was treated in the ordinary way. My patient was sitting up on the fourth day comfortable while the other one was still no better. A. L. C

D., CHICAGO An inflammation of any portion of the structure of a vein which results in changes in the coats of the vessel, is denominated phlebitis. The disease occurs as a complication of other conditions, and is not always diagnosed as an independent condition. In fact in many chronic cases it is not recognized at all, and while it contributes very materially to the prolongation of the disease, as a separate and distinct condition it receives no treatment. This at times becomes a serious error. The disease may be either acute or subacute. A chronic form is not always recognized, as in this form structural changes are apt to produce obliteration of the lumen of the vessel, and result in conditions, that are otherwise recognized. The location of phlebitis, or the conditions attending it, supply a variety of names for this disease. There is plastic , or hyperplastic phlebitis, the condition above described;

Background: The approach to septic phlebitis depends on the location and type of the involved veins, the underlying cause of the septic phlebitis, and the organisms involved. Septic phlebitis of the deep veins is a life-threatening emergency that may fail to respond to even the most aggressive therapy. Peripheral septic thrombophlebitis is a common problem that can develop spontaneously but most often is associated with breaks in the skin. Peripheral septic phlebitis commonly is caused by intravenous (IV) catheters, venipuncture for phlebotomy, or licit or illicit IV injections. Peripheral septic phlebitis often produces a septicemia that can seed secondary sites of infection. Catheter-related septic phlebitis is one of the most common causes of fever after the third postoperative day. Catheter-associated phlebitis may develop at any site but is most frequent after cannulation of lower limb veins and veins at the groin. Septic phlebitis of a superficial vein without frank purulence is referred to as "simple phlebitis." Simple phlebitis can be benign, but it also can progress to cause serious complications and even death.

Vinorelbin is semisynthetic catharanthus alkaloid displaying activity on an extremely wide variety of tumors, such as multiple myeloma, bronchial carcinoma and breast cancer. In metastasizing breast cancer it has even been used as frontline therapy. As a very troublesome side effect, phlebitis with formation of small vesicles proximal and distal to the puncture site (with or without extravasation) may occur. The illustrations show ths complication 7 days after infusion of 50 mg vinorelbin (infused iv in 250 ml 0,9% NaCl). These incidents can be avoided by following the suggestions of e.g. Anita Margulies et al. (Onkologische Krankenpflege, 2nd ed. Berlin: Springer 1997; p. 898).
