Phlebitis is an inflammation of a vein. Thrombosis is the formation of a thrombus or blood clot that may partially or completely block a blood vessel. A clot located in an inflamed, superficial blood vessel is called a thrombophlebitis. A clot formation in a deeper vein is called a deep vein thrombosis. Blood clots may follow injury, surgery, childbirth, or prolonged bed rest. They may also be associated with infection. Symptoms of phlebitis may include:
* a hard and cord-like vein that is very sensitive to pressure,
* a surrounding area that is red and warm to the touch,
* a limb that is cold or swollen, or
* a slight fever. The primary danger in thrombophlebitis, is that the clot may move and travel to a vital organ, causing serious damage. For example, a pulmonary embolism occurs if the clot travels to the lung. If you have symptoms of thrombophlebitis or deep vein thrombosis your doctor may order a Doppler ultrasound examination, a test that uses sound waves to locate clots. A venogram is a test in which dye is injected into a vein, usually in the foot. X-rays are taken to visualize the circulation and any blockages that may be present. Lung scans may be ordered if there is concern that blood clots have traveled to the lungs. Phlebitis is treated with anticoagulant drugs, such as heparin or coumadin. These drugs inhibit the formation of blood clots. They reduce the risk of the clots traveling and obstructing blood flow to vital organs.

Do you know how the solution's pH (4.5) and tonicity (642 mOsm/liter) will affect his risk of chemical phlebitis? In this article, I'll examine the role of pH and tonicity (also called osmolarity) in vascular complications, such as phlebitis and thrombosis. I'll explain steps you can take to reduce your patient's risk of complications and increase the dwell time of intravenous (I.V.) devices. I'll also outline how to make sure your patient has the right vascular access device based on the drug he's receiving and his circulatory condition. Let's start with some basics about pH.

PHLEBITIS, A COMMON BUT USUALLY avoidable complication of I.V. therapy, is an inflammation of one or more layers of the vein wall. Irritation of the innermost layer, the tunica intima, causes tenderness-the first symptom of phlebitis. By the time you see skin color changes (redness), the irritation has progressed to include the tunica media, tunica adventitia, dermis, and epidermis. (See illustration.) On palpation, the vein may be hard and cordlike (a late sign). The best way to prevent phlebitis is to change the I.V. site before inflammation occurs. The Intravenous Nurses Society standard of practice is to change peripheral catheters and tubing every 48 hours and intermittent I.V. tubing every 24 hours.

What is deep vein thrombosis?
Deep vein thrombosis is the development of a blood clot (thrombus) in the deep veins of the legs, pelvis, or arms. Clots can form in superficial veins (called superficial thrombophlebitis or simply "phlebitis") and in deep veins. While blood clots in superficial veins rarely causes serious problems, clots in deep veins require immediate medical evaluation. See an illustration of the leg veins Blood clots in deep veins can grow in size, break loose, and then travel through the bloodstream to the lungs, resulting in life-threatening pulmonary embolism . Deep vein thrombosis can cause other long-term complications. In about 25% of cases, deep vein thrombosis damages the affected vein and leads to long-lasting post-thrombotic syndrome This condition can cause pain, swelling, discoloration, and leg sores.

Prevention of lidocaine-infusion phlebitis by heparin and hydrocortisone

Previous reports have suggested that infusions of lidocaine (lignocaine) cause a high incidence of phlebitis. We investigated the possibility of reducing this high incidence by the addition of small amounts of heparin or hydrocortisone (or both) to the infusate of lidocaine.
