Pediatric nutrition support
M Marian Pediatric patients now survive and thrive because of advancements made in nutrition support. Growth and development is of paramount importance in this population and adequate nutrition is necessary. Growth of premature infants receiving nutrition support mirrors intrauterine rates. Survival without nutrition support is not always assured because of inadequate nutrient stores. Institution of nutrition support in children with chronic illnesses also promotes growth and development. Enteral nutrition is preferred because of immunologic benefits, lower cost, and less frequent infectious complications. Parenteral nutrition should be considered when enteral nutrition fails or is contraindicated. Successful nutrition rehabilitation can result if both enteral and parenteral nutrition are initiated slowly and advanced as tolerated. Ongoing monitoring is essential while promoting "normal" growth and development.

The human liver is on of the most important organs involved in the metabolism and storage of nutrients. Carbohydrate metabolism
The liver plays a major role in carbohydrate metabolism. Galactose and fructose - products of carbohydrate digestion, are converted into glucose in the hepatocyte. Liver stores glucose as glycogen and then returns it to the blood when glucose levels become low. Protein metabolism
Proteins are converted to substrates that are utilized in energy and glucose production. Liver detoxifies ammonia by converting it to urea, 75% of which is excreted by the kidneys. Synthesis of vital protiens of the blood takes place in the liver. Fat metabolism
Fatty acids from the diet are converted in the liver to produce energy.

Introduction
On June 26, 1998 Janssen Pharmaceutica issued an announcement to physicians and patients that the labeling for cisapride was being changed to reflect increased warnings regarding the association between use of the drug and adverse cardiac events. Additionally, the list of drugs that potentially could increase the risk of a cisapride-related adverse effect was expanded. The announcement had a major impact on practitioners because of the frequency with which cisapride is used throughout the world. Although well intended, the letter caused confusion as to whether cisapride should be used and if so, what precautions should be taken prior to or after administering the medication. In response to the situation, the North American Society for Pediatric Gastroenterology and Nutrition (NASPGHAN) convened a Committee in August, 1998 to address these and other issues engendered by the new warnings regarding cisapride use (Dr. Allen Mitchell joined the Committee in November, 1998). The purview of the Committee was to develop a medical position statement on the appropriate use of cisapride in infants and children. Members of the Committee included six pediatric gastroenterologists, two pediatric cardiologists, a pediatric pharmacologist, and a pediatric pharmacoepidemiologist. The Committee worked independently but received cooperation from the Janssen USA staff, including Mr. Joseph Defilippis, Drs. Mark Klausner and Alan Joslyn, as well as Dr. Stephen Spielberg (Global Head, Pediatric Drug Development, Johnson and Johnson, Inc).

Research focuses on gastrointestinal and nutritional conditions associated with chronic diseases of childhood, total parenteral nutritional-induced cholestatsis, irritable bowel syndrome and chronic abdominal pain of childhood and digestive and pancreative enzyme analysis.
