Pediatrics Nutrition Goal: To familiarize students with the differences in nutritional requirements between adults and children with respect to fluids, electrolytes, calories, carbohydrates, proteins, fats and trace elements. Objectives: 1. Given the age, weight and disease state of a child, calculate the daily maintenance fluids and total daily fluid requirement. 2. List the indications and monitoring parameters (with monitoring frequency) for a child receiving TPN. 3. Given the age, weight and disease state of a child, be able to describe how to initiate and advance TPN with respect to fluids, carbohydrates, protein and fat. 4. Given the age, weight, disease states and TPN order for a pediatric patient, determine if the TPN order is adequate in terms of supplying appropriate calories, protein, fats, electrolytes, minerals, vitamins and trace elements. 5. For both central and peripheral TPN, list the maximum concentration of dextrose that can be used. 6. Explain why neonates and young infants need special amino acid formulations.

Overview

In California, participation in PedNSS began in 1988.

The hospital provides patient meals that meet nutritional needs and appeal to the taste of young people. In addition, food and nutrition services gives the patients special service by using bright tray mats, celebrating holidays, and observing "fun days" with tray favors and games. Specialized formulas are prepared each day to supply the unique nutritional requirements of individuals patients. Clinical nutrition services include calorie counts, nutrition counseling, formula instructions, and daily screening to identify patients at nutrition risk. The clinical staff includes registered dietitians and diet technicians who work directly with patients and families to develop nutrition plans that meet the medical and cultural needs of the family. Pediatric Nutrition Emphasis
The Pediatric Nutrition emphasis is a residency-training program outside the Division of Nutrition that is offered to individuals who desire increased knowledge and additional clinical experience in pediatric nutrition. Registered dietitians who are interested in improving their pediatric skills and U of U CMP students who have completed their second year of study may apply to a 6-12 week pediatric practice experience provided at Primary Children's Medical Center. Primary Children's Medical Center is a 240-bed tertiary care pediatric hospital located next to the University of Utah campus and University Hospital. A fee is charged for the residency; however, scholarships are available. Up to 4 students are accepted each year. Leadership Education Excellence in Pediatric Nutrition Program Two types of programs are offered. The first program is a four- to six-month Pediatric Nutrition Fellowship for dietitians/nutritionists. This fellowship program provides didactic and clinical experiences that provide or enhance pediatric nutrition knowledge, clinical expertise and leadership skills of the dietitian/nutritionist. These dietitians/nutritionists will obtain core pediatric nutrition competencies related to infants, children and adolescents with special health needs. During the last two months of the six-month fellowship program, fellows may specialize in one of three areas: 1) care of newborns in intensive care units and following discharge; 2) nutritional care of children with special health needs; and 3) nutritional care of adolescents including those with type 1 diabetes. For next fellowship programs (4 to 6 months each) contact Program Chair or Coordinator. The second type is continuing education workshops. A four-day national continuing education workshop for dietitians/nutritionists, nurses, physicians and other health care providers will be held in September 2006.

Fellowship Internship Overview Program Outline Leadership Education Excellence in Pediatric Nutrition Fellowship Program The fellowships are in coordination with the Department of Pediatrics (Section of Neonatal-Perinatal Medicine) at the Indiana University School of Medicine. The training will occur in the Indiana University Hospitals, including the James Whitcomb Riley Hospital for Children and University Hospital. Objective The objective of the fellowship program is to prepare dietitians/nutritionists to function as specialists in the care of high-risk neonates throughout the continuum from acute hospital care to ambulatory community care. Qualifications The fellow must be a registered dietitian and have a minimum of 12 months of clinical experience. A master's degree in nutrition or enrollment in a master's degree program in nutrition is highly desirable. Content The program content will be individualized to meet the interests, needs and abilities of the leadership education excellence in pediatric nutrition fellow. The fellowship program consists of core content that includes lectures that discuss medical and nutritional principles of the management of normal infants and high-risk infants and children with specific diseases or conditions such as prematurity, bronchopulmonary dysplasia, cyanotic heart disease, myelomeningocele, cystic fibrosis and failure to thrive. The core content also includes guided clinical experiences in managing the nutritional care of high-risk infants with these conditions and others. The core training will occur in the high-risk infant and toddler units and newborn intensive care units at James Whitcomb Riley Hospital for Children and University Hospital, community public health clinics in Fort Wayne and South Bend and the Indiana State Board of Health central office in Indianapolis. An outline of the program follows.

Nutrition support is provided for patients with malignancy undergoing chemotherapy and for patients with AIDS; chronic liver, renal, or pulmonary failure; gastrointestinal failure, including inflammatory bowel disease and intestinal malabsorption syndromes. Perioperative nutrition support is also given to patients undergoing major surgical procedures.
Inpatient nutrition support services are the responsibility of a team of health-care professionals, including a designated hospital dietitian, a designated pharmacist, and the attending physician of each month. Patient follow-up and new clinical nutrition consultations are also provided by outpatient clinics, each attended 2-3 times weekly by Drs. Halsted, Crim, and Wolfe. Consultations focus on evaluation and care of patients with intractable morbid obesity, combined hyperlipidemias, anorexia and bulimia nervosa, and chronic intestinal failure requiring home total parenteral nutrition (TPN).
