      NUTRITION SUPPORT GUIDELINES FOR THE PICU PATIENT The purpose of this website is the following:
      • To provide guidelines for the initiation of appropriate nutritional support for pediatric patients in the PICU in a timely manner. To provide guidelines for the provision of an appropriate amount of nutrition given the patient's critical illness and underlying nutritional status aimed at minimizing the catabolic response and improving nutritional status, as appropriate to the patient's condition.
      Nutrition support of the critically ill, post-trauma or post-operative patient continues to develop as a clinical science in light of advances in nutrition, pharmacology and technology. Given the wide diversity in pediatric critical care states, we are challenged to accurately assess the nutritional requirements of our patients and the best mode of nutrition support to optimize their recovery and their nutritional and medical outcomes. Given the complexity of nutritional support in the critically ill child, this website has been organized according to the different categories based on the focus of the nutrition intervention.

Pediatric Nutrition Guidelines for Discharge Home
When you return home after your transplant, your immune system is still functioning below normal. You may also have increased nutrient needs due to medications or loss of muscle mass. Calories, protein, fluid, vitamins, and minerals are necessary for you to regain your immunity, maintain or achieve your weight goal, strengthen your muscles, and feel better!
Food Safety
  • Allogeneic patients should continue the "Immunosuppressed Patient Diet" until off all immunosuppressive therapy (such as prednisone, FK506, cyclosporine). Autologous patients should continue the diet during the first three months after transplant. When you discontinue the Immunosuppressed Patient Diet continue to use care when purchasing foods, preparing meals and dining out. Situations where you cannot control the cleanliness of the food put you at greater risk for acquiring a food-borne illness. Examples include salad bars, smorgasbords, delicatessens, bulk food bins, free food samples offered in stores. Review the Food Safety Guidelines in the Patient/Caregiver Manual. Continue to avoid herbal and other preparations, since they may contain harmful ingredients or infectious organisms. Herbs are not regulated for purity or for effect on health.

Director: Program Description: The Pediatric Gastroenterology and Nutrition Program fully evaluates and treats pediatric patients with intestinal, hepatobiliary and nutrition-related disorders. The pediatricians are sensitive to the needs of the infant, child and adolescent with GI disorders. Procedures are performed only on carefully selected patients. Specific attention is paid to nutritional requirements through the full assessment of patients with chronic illnesses, so that normal growth and development may be achieved. A team approach, including physicians and a specialized pediatric nutritionist, is utilized. Diseases, such as malabsorption, chronic diarrhea, failure to thrive, abdominal pain, peptic disease, constipation, encopresis, inflammatory bowel disease, gastrointestinal hemorrhage, hepatitis, portal hypertension, parasitic infestation and hyperlipidemia, are evaluated. Procedures available include: esophagogastrodude-noscopy; colonoscopy; sigmoidoscopy; endoscopic biopsy; removal of foreign body and polypectomy; rectal, enteral and liver biopsy; 24-hour esophageal pH probe; esophageal manometry; hydrogen breath testing; total parenteral nutrition; enteral tube feeding and percutaneous endoscopic gastrostomy (PEG) placement.

This is to provide an abbreviated form and not to replace the actual manual. The hospital and JCAHO mandates that all patients be screened for nutritional status after three days (ICU patients sooner). You will see these screening notes on the chart and the dietitian may speak to you about their concerns. These must be relayed to the fellow or service attending for discussion. Consultations for nutritional support from the GI service are only obtained upon attending request. Caloric requirements: 0-1 year 90-120 kcal/kg/day 1-7 years 75-90 kcal/kg/day 7-12 years 60-75 kcal/kg/day 12-18 years 30-60 kcal/kg/day Of course, physical status (metabolic condition, bedridden, active) will impact on these baseline values. For infants, the basic principle is that sufficient calories need to be supplied to assure continued growth and development. For chronically hospitalized infants it is important to maintain an accurate growth record in the front of the chart and /or at the bedside.

Pediatric Nutrition Rosen von Rosenstein, Nils. The Diseases of Children and Their Remedies. Translated into English by Andrew Sparrman, M.D. London: T. Cadell, 1776. Nils Rosen von Rosenstein (1706-1773) wrote the first modern textbook of pediatrics and included remarkably modern instructions for artificial feeding of infants with diluted cow's milk, advising appropriate hygienic precautions. He laid the foundation of pediatrics as a specialty and gave an impetus to research which influenced the future course of pediatrics. When the last Swedish edition was published in 1851, a total of at least twenty-five editions in eight different languages had passed through the printing presses. Researchers have known for about 25 years that atherosclerosis or hardening of the arteries begins in childhood. But it has taken time to find markers for artery disease and to see if the markers important for problems in adults also are significant in children. Both studies looked at C-reactive protein, a marker of systemic inflammation that is produced in the liver. High levels of C-reactive protein are a known predictor of heart disease, cardiovascular problems, and type 2 diabetes in adults. The first study, published in the January issue of Obesity Research, looked at obesity, levels of fasting insulin and C-reactive protein in the blood. Obesity is associated with higher than normal levels of insulin and C-reactive protein in older children and adults but little is known about these associations in very young children. The researchers found that greater body mass index, even in very young children, is associated with higher fasting insulin levels and that fasting insulin is associated with C-reactive protein level. The researchers saw no observable relation of fasting glucose level with the body fat measurements, indicating that glucose equilibirium was maintained despite higher fasting insulin levels among the children with above average weights. But the findings that the children with higher body mass index had higher fasting insulin levels indicate that insulin resistance, a potential warning sign for type 2 diabetes, begins much earlier in life than previously thought.
