June 18, 1999 Web posted at: 9:19 AM EDT (1319 GMT) By Marvin Ross (WebMD) It is a condition normally associated with postmenopausal women but osteoporosis, or brittle bones, is also seen in men. According to Dr. P. Peris of the University of Barcelona, "Osteoporosis in men has received much less attention; however, it is increasingly recognized as a problem in clinical medicine." In a 1995 study published in the British Journal of Rheumatology, Peris pointed out that 30 percent of all hip fractures occur in men and that vertebral fractures are much more common in men than previously thought. The female-to-male ratio is only 2-to-1. According to Dr. Allan Gold, an endocrinologist and senior physician at the Montreal General Hospital, a recent Canadian survey found that 20 percent of men have serious bone loss in their vertebrae, and by age 70 the figure is as high as 30 percent. Gold said that "men in their 80s have a fracture rate that is equal to women's."

O steoporosis and low bone density are significant risk factors for morbidity and mortality in older adults. These conditions are characterized by poor bone strength and are associated with an increased risk of fractures from even slightly traumatic events, such as falls from standing height or lower. Vertebral fractures are the hallmark of osteoporosis, but hip, Colles' and other fractures are also common. Low bone density may be a silent condition until a complication occurs. Increased attention is being given to the early diagnosis and treatment of low bone density as well as the prevention of fractures. Several medications have recently been labeled for the treatment of osteoporosis, but their marginal benefits require careful consideration of their cost. Osteoporosis and Osteopenia As commonly defined, osteoporosis is a condition in which bone mineral density is 2.5 standard deviations below the average bone density in gender-matched young adults. Osteopenia is a less advanced state of low bone mineral density. The risk of fracture increases two to three times for every 10 percent drop in bone density.

O steoporosis is a public health problem affecting 75 million persons in the United States, Europe and Japan, including one third of postmenopausal women and most of the elderly in the United States, Europe and Japan. Osteoporosis results in more than 1.3 million fractures annually in the United States. Family physicians and other primary care professionals should recognize the implications involved when patients present with fractures, identify patients with subclinical osteoporosis and know when to implement preventive strategies in younger patients. An algorithm (Figure 1) is presented to assist in the evaluation of osteoporosis.

This booklet has been produced for anyone interested in finding out more about osteoporosis. You may have the condition yourself, or you may be a friend or relative of someone with osteoporosis. Whatever reason you have for reading this, we hope you will find it useful. Unfortunately we cannot hope to answer all your questions, because everyone is different and this booklet is no substitute for individual consultation with a doctor. What is osteoporosis?
The word 'osteoporosis' means, literally, 'porous bone'. It is a condition where you gradually lose bone material so that your bones become more fragile. As a result, they are more likely to break even after a simple fall. Osteoporosis is quite common in Britain. Each year there are around 70,000 hip, 120,000 spine and 50,000 wrist fractures due to osteoporosis.

The Department of Rheumatology at the Hospital for Joint Diseases is dedicated to the diagnosis and treatment of all patients with diseases of the joints such as Arthritis ; bone diseases such as Osteoporosis ; soft tissue diseases such as Tendonitis ; and autoimmune diseases such Lupus and Scleroderma This is accomplished through a balance of education, research, and clinical care of patients with various rheumatic disorders.
