Osteopathy gains acceptance, but may be losing identity
February 4, 2000 Web posted at: 12:03 PM EST (1703 GMT) By Elaine Herscher (Healtheon/WebMD) When Dr. Nancy Nichols began her osteopathic practice 15 years ago, no hospital in her town of Mesa, Arizona, would grant her privileges. Today, she's welcome to practice at all of them. To Nichols, that represents real progress for her profession. Long considered pseudo-doctors by the medical establishment, osteopathic doctors are in fact licensed physicians who can do surgery and prescribe drugs but have added training in manipulative therapy. It's the manipulation part of the practice that has earned them a reputation as "alternative" practitioners. But only 6.2 percent of osteopathic physicians now practice manipulation on the majority of their patients, leading many to worry that their profession will soon have nothing that distinguishes D.O.s from M.D.s. "It's sort of like being the victim of our own success," says Dr. Eugene Oliveri, D.O., president of the American Osteopathic Association. The November 4, 1999 issue of the New England Journal of Medicine warned osteopaths that they are losing something valuable.

Osteopathy is a medical practice based on the theory that diseases are due chiefly to loss of structural integrity which can be restored to harmony or equilibrium by manipulation. The manipulation allegedly allows the body to heal itself. Osteopaths use manipulation for diagnosis, treatment, and prevention of disease. Andrew Taylor Still (1828-1917), a Civil War surgeon in the Union army, is credited with discovering os teopathy as an alternative to the medical practices common in his day, practices which failed to save his three children from spinal meningitis. Still became convinced that he could cure diseases by shaking the body or manipulating the spine. In his autobiography, he says he could "shake a child and stop scarlet fever, croup, diphtheria, and cure whooping cough in three days by a wring of its neck" (Barrett) . He also advocated clean living, including abstinence from alcohol and medically prescribed drugs. Surgery was to be avoided, if possible. Today, D.O.s (doctors of osteopathy) complement manipulation with standard medical methods of diagnosis and treatment, including recommending drug therapy and surgery if appropriate. D.O.s have four years of medical training at a college of osteopathic medicine and do a one-year internship in primary care. Some continue their education in an area of osteopathic specialization. Nevertheless, there has not been scientific validation of Still's theory of shaking and manipulating to remove obstructions.

What is it?
The term 'manipulation' comes from the Latin manipulare , meaning 'to handle'. It covers a range of techniques that use the hands to realign the structural system of the body, relax muscles and improve circulation. The most common forms of manipulation practised in complementary medicine are osteopathy and chiropractic. Physiotherapists also use manipulation techniques.
History and theory
Therapeutic touch has been used for healing since ancient times. Manipulation was practised in ancient Greece and 'bonesetters' were part of all early Eastern and European medical traditions. What is it?
Cranial osteopathy is a specialist form of osteopathy , favoured by osteopaths who feel most comfortable working with gentle, subtle techniques. An American osteopath, William Garner Sutherland, developed the technique in the 1930's. Sutherland found that the cranial bones (the skull bones encasing the brain) weren't fused in adulthood, as was widely believed, but actually had a cycle of slight involuntary movement. This movement was influenced by the rhythmic flow of cerebrospinal fluid (the nourishing and protective fluid that circulates through the spinal canal and brain) and could become blocked. He developed refined and subtle techniques using very gentle manipulative pressure to encourage the release of stresses and strains in the cranium and throughout the body.

In 1975, David and Kerry Ruddick opened the Summertown Clinic making it the longest established Clinic of Complementary Medicine in Oxford.
After graduating from the British College of Naturopathy and Osteopathy in London in 1968 both David and Kerry worked at the Maidstone Osteopathic Clinic in Kent and also began their own clinic in the City of London. It was very apparent that different disciplines each have a place in health care, so they invited other professionally qualified practitioners to join them at the Clinic and so the Summertown Clinic came into being.
The Summertown Clinic has grown over the years becoming a centre of many therapies practiced by caring professional practitioners.

Near the turn of the 19th century, the son of a pioneer physician took another look at the way things were being done in medicine and saw a better way. Always fascinated by the human anatomy and the science of healing, Andrew Taylor Still pursued a life of study and practice to eventually establish the healing art of osteopathy. Still first articulated the idea of improving medical practice while living in Kansas in 1874. It was at that time, he had a seminal thought: The human body has much in common with a machine, one which ought to function well if it is mechanically sound. Still was a typical frontier physician, having been trained through apprenticeship, with some medical lectures attended later. Like nearly all frontier physicians, he did many things besides practice medicine: farming, mechanical work, and fighting in the Civil War. His medical practice included caring for both settlers and American Indians. He faced epidemics such as cholera, malaria, pneumonia, smallpox, diphtheria, and tuberculosis. After the War, spinal meningitis claimed three of his children and he began searching for a better system of medicine. This new system promised simply to support health, which on the surface would not seem controversial. But the end of the 19th century was a time of multiple schools of healing, and on the frontier there was medical competition and a mistrust of new ideas.

He now regularly attends to the podiatric needs of the Royal Ballet Company at the Royal Opera House and has done so since 1992. In 1991 Peter was appointed to the Royal Masonic Hospital until its closure a few years ago. Here he worked with leading physiotherapist Vivien Grisogono and contributed to one of her many books on running and running injuries. Peter has an active interest in running and general fitness both personally and through his two children, Ben and Sophie. He treats athletes from many different disciplines. The recent acquisition of the latest Ultrasound Imaging System from America allows many injuries to be safely examined. This helps in choosing the correct treatment and permits the progress of the condition to be checked upon.

What Is It? Osteopathy is a holistic approach to the practice of medicine. It emphasizes that all systems of the body are interrelated and that any adverse changes in one part of the body may impair the functioning of other systems or organs. For this reason, doctors of osteopathy (D.O.s) treat the body as a whole, often focusing special attention on the musculoskeletal system (bones and muscles) in an effort to find the underlying cause of an ailment. Osteopathy also stresses preventive medicine, encouraging patients to maintain a healthy lifestyle and eat a healthy diet. Osteopathy was first developed in 1874 by Missouri physician and surgeon Andrew Taylor Still, who became disenchanted with conventional medicine after losing three of his children to spinal meningitis. Still became convinced that health and illness were largely dependent upon the soundness and mechanical functioning of the body's bones, muscles, and joints. Chiropractic and osteopathy
Chiropractic and osteopathy are both manipulation-based therapies used for treating problems associated with bones, joints and the back. The two therapies have much in common, but chiropractors tend to focus on the joints of the spine, and the nervous system, while osteopaths put equal emphasis on the joints and surrounding muscles, tendons and ligaments.
How they work
Chiropractors and osteopaths share conventional medicine’s view that the human body is like a machine and that any disease is due to a breakdown of part of the machine. However, they also believe that many health problems can be caused by poor posture and misalignment of muscles and joints (and, with chiropractic, particularly the spine). They suggest that if the structure of the body is improved and the spine put back into alignment, the function of the body improves, problems will be alleviated and good health will be restored. Misalignment is usually thought to be due to an external cause, such as a fall or other accident (even one that happened years before), or to long-term poor posture.
