Oppositional Defiant Disorder
A pattern of negativistic, hostile, and defiant behavior lasting at least 6 months, during which four (or more) of the following are present:
  • often loses temper often argues with adults often actively defies or refuses to comply with adults' requests or rules often deliberately annoys people often blames others for his or her mistakes or misbehavior is often touchy or easily annoyed by others is often angry and resentful is often spiteful or vindictive
Note: Consider a criterion met only if the behavior occurs more frequently than is typically observed in individuals of comparable age and developmental level. The disturbance in behavior causes clinically significant impairment in social, academic, or occupational functioning.

Oppositional Defiant Disorder (ODD)

Oppositional Defiant Disorder (ODD) is a diagnosis given to children who display a pattern of negative and defiant behavior to parents, teachers and others who have authority over them. Many children disobey their parents or teachers from time to time. In fact, oppositional behavior is very common in pre-school children and teenagers. It's important that we don't mislabel these normal "phases" of childhood as signs of a behavioral disorder. Children with ODD have frequent run-ins with authority figures and are oppositional far more often than other children their age. Boys are more likely to be diagnosed with ODD than girls, especially before puberty. Children typically begin to show signs of ODD before age 8 and no later than 13-15. While mental health professionals are very cautious with diagnosing ODD before the school-aged years, many children with ODD had "difficult" temperaments as toddlers. They were often fussy, argumentative, and likely to throw temper tantrums even as very young children. The following is a list of signs that may suggest that a child has ODD (APA, 1994). Be careful, as mentioned earlier, many children are oppositional from time to time. Children with ODD really stand out from other kids due to their poor behavior. Also, this pattern of behavior must have been going on for at least 6 months before the diagnosis of ODD can be considered.

Oppositional defiant disorder
My son's had behavioural problems since he could breathe. I've been to various doctors, psychiatrists and psychologists without any luck. But I haven't given up. I've searched the web and think my son suffers with the conduct disorder of oppositional defiant disorder (ODD), but how do I go about getting him the right treatment? I've heard Ritalin could help him but I don't know where to start. Alyson

Your priority must be to get an accurate diagnosis made by an expert in child psychiatry. Until you have this there's no point thinking too much about treatment because you need to know what's appropriate to your son's condition. If your son does have ODD he'll need specialist psychiatric help and advice with regard to schooling. Treatment may include stimulant medication such as Ritalin, but this will depend on several factors such as the specific nature of his problem, the treatment approach taken by his specialists, and whether there's an element of attention deficit disorder (ADD and ODD often co-exist and it's usually the ADD element of the problem that responds to stimulants). Treatment also includes behavioural and other psychological therapies, non-stimulant medications and special diets.

A child who routinely and persistently argues and refuses to follow rules can be said to have "oppositional defiant disorder." Because such children anger so easily, they have trouble getting along with family, friends, schoolmates, and teachers. They may also have learning difficulties and may have trouble expressing themselves. The disorder is found in children ages 3 to 18 years. Before the teenage years, it affects more boys than girls.
Causes
Doctors do not know the cause of oppositional defiant disorder. The problem seems to be more common among children who have other emotional problems and in those whose parents have serious marital problems.
Signs/Symptoms
Symptoms may first show up in the home. Later, children exhibit defiant behavior in other places. A child who displays the following symptoms for more than 6 months is considered a victim of the disorder:

by: Anthony Kane, MD Introduction Oppositional defiant disorder (ODD) is a psychiatric behavior disorder that is characterized by aggressiveness and a tendency to purposefully bother and irritate others. These behaviors cause significant difficulties with family and friends and at school or work. Oppositional defiant disorder is sometimes a precursor of conduct disorder. Much of the literature tends to lump these two conditions together. However, they seem to be distinct entities and, although conduct disorder does have a genetic component, ODD does not. Description Oppositional defiant children show a consistent pattern of refusing to follow commands or requests by adults. These children repeatedly lose their temper, argue with adults, and refuse to comply with rules and directions. They are easily annoyed and blame others for their mistakes. Children with ODD show a pattern of stubbornness and frequently test limits, even in early childhood. These children can be manipulative and often induce discord in those around them. Commonly they can incite parents and other family members to fight with one and other rather than focus on the child, who is the source of the problem.

OPPOSITIONAL DEFIANT DISORDER
DEFINITION Oppositional defiant disorder
is defined as an enduring pattern of uncooperative, defiant, and hostile behavior toward authority figures that does not involve major antisocial violations, is not accounted for by the child's developmental stage, and results in significant functional impairment. A certain level of oppositional behavior is common in children and adolescents, and it is almost expected when the process of individuation and separation. It should be considered a disorder only when the behaviors are more frequent and intense than in unaffected peers and when they cause dysfunction in social, academic, or work-related oppositional defiant disorder, oppositional disorder. DSM-IV classification requires the presence of at least four criteria out of a list of eight . No age limits are set, but the disorder typically emerges in childhood.
HISTORY
T he signs of oppositional defiant disorder tend to cluster with mild physical aggression.
EPIDEMIOLOGY
In cross-sectional studies conducted in the general population, the prevalence of oppositional defiant disorder varies from 1.7 to 9.9, with a weighted average of 5.7 percent. The rate is higher in boys.

Symptoms are less severe than those associated with Conduct Disorder but sometimes indicate the early stages of Conduct Disorder (CD) and may sometimes lead to the development of Anti-social Personality Disorder during adulthood. Oppositional defiant disorder (ODD) is a behavior disorder, usually diagnosed in childhood, that is characterized by uncooperative, defiant, negativistic, irritable, and annoying behaviors toward parents, peers, teachers, and other authority figures. Children and adolescents with ODD are more distressing or troubling to others than they are distressed or troubled themselves. What Causes Oppositional Defiant Disorder?
While the cause of ODD is not known, there are two primary theories offered to explain the development of ODD. A developmental theory suggests that the problems begin when children are toddlers. Children and adolescents who develop ODD may have had a difficult time learning to separate from their primary attachment figure and developing autonomous skills. The bad attitudes characteristic of ODD are viewed as a continuation of the normal developmental issues that were not adequately resolved during the toddler years.
