A prestigious program sends doctoral students across the U.S. to counsel community health organizations—and gain real world experience for themselves. By Brian W. Simpson Photos by Howard Korn and Doug Barber Perched at the upper end of Penobscot Bay, the town of Bucksport, Maine, markets itself as the "Jewel at the End of the Bay." The town of 5,000 boasts a small town ambience, a rich history dating back to 1764, employment from a large paper mill in town and major port facilities, and one of the state's biggest tourist attractions: the nearby, historic Fort Knox. But Bucksport, located in Maine's Down East region, also has its share of public health problems: alcoholism, high rates of chronic disease like diabetes, an elderly population in need of long-term care, and health care access issues (the nearest hospital is 18 miles away). What would happen when the seed money ran out? How could the group from Bucksport, Maine, ensure its public health programs would be sustained? Since Maine lacks the traditional public health infrastructure of county health departments, town residents can't rely on government-driven solutions. Instead, concerned citizens organized themselves into the Bucksport Community Health Advisory Committee to find the answers themselves. Their efforts got a big boost in 2000, when the group won a two-year $100,000 grant from the

Health Care, Nutrition, and Goal One. ERIC Digest.
In April, 1993, President Clinton sent to congress the Goals 2000: Educate America Act, which embraces the objectives of Goal One of the six national education goals. Goal One states that "By the year 2000, all children in America will start school ready to learn." One of the objectives formulated to meet this goal asserts that "Children will receive the nutrition and health care needed to arrive at school with healthy minds and bodies." Problems which can affect children's later development and school readiness include lack of prenatal care, low birth weight, maternal substance abuse, malnutrition, vaccine-preventable diseases, sensory impairments, high blood lead levels, and anemia. Efforts to promote children's health and nutrition, and to prevent children's illness, include federal and state programs, and initiatives to serve preschool children in health care settings, in preschool programs, and in community settings. These efforts address prenatal care, health and nutrition of the developing child, parent education, preschool teacher education, and health and nutrition instruction for preschool children.
FEDERAL AND STATE PROGRAMS
Many of the federal programs profiled above are administered by states, or are jointly funded with federal and state monies. Several states have launched efforts of their own to serve preschoolers' health and nutrition needs. The Kentucky Education Reform Act of 1990 authorized the establishment of family service centers in many of the state's school districts. Project Healthy Start in Hawaii consists of one-stop service centers located throughout the state to serve children and families at risk (Boyer, 1991). As part of Project Network of the Mississippi State Department of Health, nutritionists provide support and technical assistance to caregivers; develop education materials; and develop and implement community-based educational projects for nutrition students and health professionals (Ford, 1991).

The New Jersey Urban Ecology Program (NJUEP) is a state-based organization that fosters the health of people, landscapes, and the diverse economies that nurture them using food and agriculture-based community development strategies. The New Jersey Urban Ecology Program (NJUEP) is housed within the Department of Nutritional Sciences at Rutgers University. The primary goal of the New Jersey Urban Ecology Program (NJUEP) is to facilitate the growth of sustainable food systems and healthy communities by focusing on the issues of food, nutrition, health and the environment. Introduction
By the year 2030, there will be 70 million Americans over the age of 65, and people 85 years and older are the fastest-growing segment of the population, according to estimates from the American College of Sports Medicine (Goodwin, 2000). Osteoporosis is a serious condition in which bones become thinned, brittle and easily broken. Because osteoporosis develops gradually and progresses slowly, it is called the "silent disease," and people often don't know they have it until after their 60's. American women over age 65 have a 50% chance of suffering from osteoporosis; men have a 20% chance (Looker et al., 1995; Melton, 1995). The lifetime risk of fracture in women older than 50 years is nearly 40%. For men older than 60 years, lifetime risk is as high as 25% (Lie, 2000). Hip fracture is predictive of another patient-oriented outcome: mortality. Within 1 year of hip fracture, there is 20% excess mortality in women and 30% excess mortality in men (Cooper, 1997). Nearly $14 billion is spent as a result of the 1.5 million fractures that occur annually in this country (Seeman, 2000).

BACKGROUND This program evolved as a result of comprehensive research studies which showed a lack of understanding of the benefits of calcium consumption as well as a lack of knowledge on calcium-rich food sources. Medical studies have also shown that these target audience groups are at higher risk for nutrition related diseases such as childhood lead poisoning, osteoporosis, and high blood pressure. TARGET AUDIENCES The target audiences for the program are African American and Hispanic parents/caregivers of children 1-8 years of age and adolescents. EDUCATIONAL OBJECTIVES The educational objectives for this project are:
  • Direct education to limited resource parents/caregivers of children 1-8 and adolescents: to promote the importance of calcium consumption for calcium-rich foods, through direct agency dissemination of information and educational materials that are science-based, culturally-sensitive and suitable for low-literacy audiences
  • Information disseminated to medical care providers and community agencies regarding this issue illustrating the use and implementation of the materials created; and
  • Retail promotion of calcium-rich foods

Dr. Caracta is the director of the Chest Clinic at Mount Sinai Hospital and a member of the Faculty Practice Associates in Pulmonary Medicine. Her research interests include gender differences in pulmonary disease, particularly lung cancer, and address the recent epidemiologic trends and studies that suggest that gender and hormonal factors may be important in the pathogenesis of lung cancer. Dr. Caracta also serves as consultant and co-investigator in clinical trials conducted at Mount Sinai directed at novel chemotherapies and the use of complementary medicine in lung cancer.
