Pulmonary hypertension is rare in patients with systemic lupus erythematosus (SLE) although pulmonary involvement occurs in about 50%. Pulmonary hypertension without obvious pulmonary infiltration is even rarer, the mechanism of which is unclear. We report here a case of SLE who presented with severe pulmonary hypertension and cor pulmonale without any evidence of parenchymal lung disease.
Case Report
The patient was a 19-year-old Saudi female who was admitted to King Khalid University Hospital, Riyadh with a two-year history of dyspnea, easy fatigability, palpitations, anorexia, loss of weight, dry cough and episodic attacks of fever every three to four months with night sweats for the last two years. Five days prior to admission she developed pain in various large and small joints with swelling in interphalangeal and right wrist joints and described features of Raynaud's phenomenon. Chest roentgenogram showed cardiomegaly with prominent pulmonary conus. No lung infiltrate was noticed. High resolution computed tomography (CT) scan was not done. Ultrasound of the abdomen revealed an enlarged liver with mild ascites. Pulmonary function test revealed a restrictive pattern with decreased diffusion lung capacity. Ventilation/perfusion scan of the lungs did not reveal any evidence of pulmonary embolism. Cross-sectional echocardiography and Doppler studies revealed significant enlargement of the right atrium and right ventricle with dilated pulmonary artery and moderate degree of pericardial effusion. Moderate tricuspid incompetence was also detected.

Twofold Photos, Inc. Arctic wolves are the safest mammals on earth, largely because they live in the most inhospitable regions of the planet. They are concentrated in North America, mainly along its polar edge, and in Greenland, where few humans adventured throughout history. However, some species can be found in Wood Buffalo National Park, Canada. Arctic Wolves (Canis Lupus Arctos) are in many ways similar to their Grey cousins, which can be found almost all the way across the western hemisphere. Because Arctic wolves haven't dealt with humans as much as their gray counterparts have, they react in a specific way, should a human appear within their range. While most grey wolves seek safety either in attack or in flight, an Arctic wolf may simply stand still and stare. This can even contribute to establishing long-term contacts with animals and befriending them. A senior research scientist of the Biological Resources Division, managed to spend several summers in a company of Arctic Wolves. He even had his boots unlaced by one of them. Because of lack of access, there have been few opportunities for a careful study of this subspecies. Therefore, little is known about their history and evolution. But their kinship with gray species is evident, because Arctic Wolves retain great similarity with those.

Your body has its own system for fighting off sickness called the immune system , but sometimes this system doesn't work correctly. When this happens, a person may have an autoimmune (say: aw-toh-im- yoon ) disease. Lupus (say: loo -pus) is one of those diseases. A healthy immune system is like an army that can protect you against germs and illness. It makes antibodies (say: an -tye- bah -deez), which are special chemicals that fight off infections. But with a disease like lupus, the army gets confused and begins attacking the healthy cells in your body. It does this by making autoantibodies (say: aw -toh- an -tye- bah -deez), which are antibodies that attack the body's normal cells. People with lupus may not look sick, but they may feel run down, weak, or nauseated. Their joints may hurt and they may have swollen glands. There are three types of lupus: Systemic lupus erythematosus (say: sis- teh -mik loo -pus er-uh-thee-muh- toh -sus) is also called SLE and is the type that most people mean when they talk about lupus. It's the most serious form of lupus. SLE can affect the skin , joints, tendons, and other body organs like the brain heart lungs , and kidneys Discoid lupus (say: dis -koyd loo -pus) is a skin disease that causes a rash on the face, neck, scalp, and ears. It's a much rarer form of lupus than SLE. It doesn't affect other organs the way SLE can, but the rash can leave scars.

Systemic lupus erythematosus SLE ) is a challenging 'autoimmune' whose wide variety of manifestations makes it a necessary part of the differential diagnosis for children and adolescents with many different presenting complaints. Le syndrome des antiphosphoslipides (SAPL) se caractérise par une thrombose artérielle et/ou veineuse, des fausses couches répétées et la présence d'anticorps antiphospholipides (aPL), i.e. anticorps anti-cardiolipine (aCL), anticorps anti-bêta2GPI, anticoagulants de type lupique (LA). Ces aPL qui interagissent avec différentes protéines de coagulation, ainsi qu'avec des plaquettes ou des cellules endothéliales, pourraient jouer un rôle dans la pathogénie de la maladie. L'incidence de ce syndrome reste inconnue ; cependant, la prévalence serait basse dans la population générale (1-4,5%) et augmente avec l'âge. Les principales manifestations cliniques sont : la survenue d'une thrombose, une morbidité chez la femme enceinte, une thrombocytopénie, la présence de symptômes neurologiques, un livedo reticularis et une anémie hémolytique. Les aPL ont été décelés chez environ 1/3 des patients atteints de lupus systémique érythémateux. Un titre élevé d'aCL, la présence de LA et surtout d'anticorps anti-bêta2GPI sont des indicateurs précoces des manifestations cliniques du SAPL chez ces patients. La thrombose est traitée par la warfarine à haute dose [ International Normalized Ratio (INR) supérieur ou égal à 3]. Il est recommandé aux femmes enceintes atteintes du SAPL de prendre de l'aspirine à faible dose (80 mg/j) ainsi que de l'héparine non fractionnée sous-cutanée ou de l'héparine de faible poids moléculaire. *Auteur : Dr M. Handout on Health: Systemic Lupus Erythematosus Systemic Lupus Erythematosus This booklet is for people who have systemic lupus erythematosus, commonly called SLE or lupus, as well as for their family and friends and others who want to better understand the disease. The booklet describes the disease and its symptoms and contains information about diagnosis and treatment as well as current research efforts supported by the National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS) and other components of the Department of Health and Human Services' National Institutes of Health (NIH). It also discusses issues such as health care, pregnancy, and quality of life for people with lupus. If you have further questions after reading this booklet, you may wish to discuss them with your doctor. Defining Lupus Lupus is one of many disorders of the immune system known as autoimmune diseases. In autoimmune diseases, the immune system turns against parts of the body it is designed to protect. This leads to inflammation and damage to various body tissues. Lupus can affect many parts of the body, including the joints, skin, kidneys, heart, lungs, blood vessels, and brain. Although people with the disease may have many different symptoms, some of the most common ones include extreme fatigue, painful or swollen joints (arthritis), unexplained fever, skin rashes, and kidney problems.
