I. General Considerations
Are health-care providers required to honor a DNR order completed on the IDPH Uniform DNR Order Form?
Licensed hospitals, certain licensed long-term care facilities such as nursing homes, and licensed emergency medical services personnel must honor a DNR order, or a copy of an order, completed on the IDPH Uniform DNR Order Form. How may the new IDPH Uniform DNR Order Forms be obtained? Are photocopies of the form valid? Generally, yes. Photocopies of a completed IDPH Uniform DNR Order Form are valid. Each health-care facility, however, may have different policies on whether copies of DNR orders completed on a form other than an IDPH Uniform DNR Order Form are accepted as valid. It is advisable to check with a health-care facility regarding its DNR policy. Why does the new IDPH Uniform DNR Order Form make a distinction between full cardiopulmonary arrest and pre-arrest emergency?

Introduction
When a statute is vaguely worded or key statutory terms are undefined, attorneys and others try to determine exactly what the General Assembly intended the law to mean by investigating the legislative history of the law. Legislative history refers both to the chronology of events that take place as a bill goes through the legislative process and the statements and documents generated during that process. These records of the process may contain remarks by the sponsor(s) of the legislation as to his/her/their intent in introducing the bill and the comments of other legislators regarding their understanding of the bill's purpose.
Researchers of Illinois legislative intent are cautioned that it may not be possible to locate clear statements in the materials described in this guide because legislators frequently do not explain their intent or understanding in published sources.
This guide identifies sources and describes techniques used in researching Illinois legislative history: The last page is a checklist that can be used as a reminder of the elements and sources consulted in compiling an Illinois legislative history.

The function of the Office of Executive Inspector General is to investigate allegations of wrongful acts or omissions committed within the agencies, boards, commissions or the public university system or the vendors doing business with those agencies, boards commissions or the public university system. When should I contact the Office of Executive Inspector General? You should contact the Office of Executive Inspector General whenever you suspect or witness a wrongful act or omission by a state employee or vendor of the state. What types of complaints does the Office of Executive Inspector General investigate? The Office of Executive Inspector General investigates alleged violations of any law, rule or regulation committed by any state employee responsible to the Governor or vendor doing business with an agency responsible to the Governor. In other words, the office investigates allegations of waste, fraud, abuse, or corruption including, for example, ghost payrolling, bid rigging, theft, abuse of state property, procurement fraud, and abuse of time, among others. Procedures for Investigating and Reporting Misconduct
and Incidents at State Facilities State Official Employees Ethics Act established among other things, the Office of Executive Inspector General (“OIG”) to investigate misconduct in agencies, boards and commissions directly responsible to the Governor. Title 20, ILCS Section 2605/2605-50 established the Illinois State Police, Division of Internal investigation (“ISP/DII”) to (i) initiate internal State Police investigations and, (ii) investigate allegations of official misconduct by State office holders and employees directly responsible to the Governor, as directed by the Governor. To clarify the duties and responsibilities of the OIG and the ISP/DII, I hereby establish the following procedures for investigating and reporting allegations of misconduct by state office holders and employees, vendors of the State, and incidents at State facilities. Except as outlined in Section III of this Order, the OIG must be notified immediately of all alleged acts of misconduct by any employee in, or any entity that has a contract with, an agency, board or commission directly responsible to the Governor. The OIG will determine whether to investigate the allegations, refer the allegations to the ISP/DII for investigation, or request that the reporting agency conduct an internal investigation. The ISP/DII shall be the primary agency responsible for the investigation of criminal conduct by state employees or criminal incidents at State facilities, as further described in Section III.

Student Conduct Any conduct, on or off campus, of students as individuals and as members of campus groups, that threatens the security of the University community, the rights of its individual members, or its basic norms of academic integrity will be subject to disciplinary action and possibly dismissal from the program. The ideas of different members of the University community will frequently conflict, and we do not attempt to shield people from ideas that they might find unwelcome, disagreeable, or even offensive. There are, however, some circumstances in which behavior so violates our community’s standards that formal University intervention may be appropriate.
