Page 2 Page 3 What is the risk of developing esophageal cancer from reflux disease? In the last twenty years the incidence of this cancer has increased faster than any other malignancy. Many experts agree that it is no accident that this has happened since the advent of strong acid suppression drugs. More people are taking more of these drugs and more people are turning up with cancer. Despite this phenomenon the pharmaceutical industry has continued to produce more and stronger acid lowering medications. Scientists in the research of the esophagus have discovered that the longer a patient with GERD has the disease and takes these medications the more likely he is to get cancer. This fact was demonstrated in a study published in the New England Journal of Medicine. We now recognize that acid is not the chemical that causes cancer of the esophagus. This is the reason why acid-lowering medications may make you feel better but may not protect you from the complications of the disease, even cancer.

STOMA ('Parastomal') HERNIAS A Hernia is a weakness or split in the muscle wall of the abdomen which allows the abdominal contents (usually some part of the intestine) to bulge out. The bulge is particularly noticeable upon tensing the abdominal wall muscles - such as occurs when coughing, sneezing, straining or simply standing. Stomas pose an additional problem. When a stoma is brought out to the surface of the abdomen it must pass through the muscles of the abdominal wall, thus a potential site of weakness is immediately created. In the ideal situation the abdominal wall muscles form a snug fit around the stoma opening. However, sometimes the muscles come away from the edges of the stoma thus creating a hernia - in this case, an area of the abdominal wall adjacent to the stoma where there is no muscle. Factors that can contribute to causing a stoma hernia to occur include coughing, being overweight or having developed an infection in the wound at the time the stoma was made. The development of a stoma hernia is often a gradual phenomenon, with the area next to the stoma stretching and becoming weaker with the passage of time. This weakness, or gap, means that every time one strains, coughs, sneezes or stands up, the area of the abdomen next to the stoma bulges, or the whole stoma itself protrudes as it is pushed forwards by the rest of the abdominal contents behind it. As with all hernias the size will increase as time goes by. Stoma hernias are rarely painful, but are usually uncomfortable and can become extremely inconvenient.

An inguinal hernia is when a loop of intestine enters the inguinal canal, a tubular passage through the lower layers of the abdominal wall.

Causes, incidence, and risk factors: Usually, there is no apparent cause of a hernia , although they are sometimes associated with heavy lifting. A hernia occurs when part of an organ (usually the intestine) protrudes through a weak point or tear in the thin muscular wall that holds the abdominal organs in place. In men, a hernia can develop in the groin near the scrotum . A direct inguinal hernia creates a bulge in the groin area, and an indirect hernia descends into the scrotum. Inguinal hernias occur less often in women than men. Inguinal hernias are also seen in infants and children. Hernias can occur when a portion of the peritoneum (the lining around all the organs in the abdomen) does not close properly before birth. This causes a small portion of the intestine to push out into the opening (a bulge might be seen in the groin or scrotum). According to the American Academy of Pediatrics, 5 out of 100 children have inguinal hernias (more boys than girls). Some may not have symptoms until adulthood.

A condition in which a portion of the stomach protrudes upward into the chest through an opening in the diaphragm (the sheet of muscle used in breathing that separates the chest from the abdomen).
Alternative Names: Hernia - hiatal
Causes, incidence, and risk factors: The cause is unknown, but hiatal hernias may be the result of a weakening of the supporting tissue. Increasing age, obesity , and smoking seem to be risk factors in adults. Children with this condition usually have it present at birth (congenital). It is usually associated with gastroesophageal reflux in infants.
Hiatal hernias are very common, especially in people over 50 years old. This condition can cause regurgitation of gastric acid from the stomach into the esophagus.

What is an inguinal hernia?
An inguinal hernia is a bulge of soft tissue through a weak spot in the abdominal wall in the groin area Inguinal hernias can occur in one or both sides of the groin at the same time or at different times. The bulge usually contains tissue lining the inside of the abdomen (peritoneum) and may include fatty tissue from inside the abdomen, a loop of intestine, or fluid (ascites). There are two types of inguinal hernias: direct and indirect.
  • A direct inguinal hernia occurs when a weak spot develops in the lower abdominal muscles from the normal stresses of living and aging. Tissues push through this weak spot as pressure within the abdomen increases. An indirect inguinal hernia is the most common type of inguinal hernia. Normally, an opening in the inguinal canal closes shortly before birth. An indirect hernia develops when this opening does not close, causing abdominal tissues to push through the inguinal canal. Symptoms may be present at birth or may develop later in life. In men, the hernia may push into the

Diagnosis/Definition
  • An abnormal protrusion (swelling) of a body part through a natural or surgically created area of weakness in a muscle or fascial wall. Common subtypes are: INGUINAL (groin): swelling in the upper scrotum or prepubic area. UMBILICAL: hernia through the umbilical ring. INCISIONAL: swelling and weakness in the area of a prior surgical incision. EPIGASTRIC: swelling in the midline between the umbilicus and the xyphoid. VENTRAL: swelling anywhere in the anterior abdominal area (overlaps with umbilical and epigastric hernias, but also includes less common abdominal wall hernias, such as SPIGELIAN).
Initial Diagnosis and Management
  • Diagnosis is made on basis of history of swelling of area in question, usually confirmed by the presence of a physically evident protrusion, which normally increases with straining and becomes less prominent when the patient is recumbent and relaxed.

Questions and Answers about Hernias What is a hernia? A hernia is a weakness or defect in the abdominal wall. It may be present from birth, or develop over a period of time. If the defect is large enough, abdominal contents such as the bowels, may protrude through the defect causing a lump or bulge felt by the patient. Hernias develop at certain sites which have a natural tendency to be weak; the groin, umbilicus (belly button), and previous surgical incisions. Why should hernias be repaired? Once a hernia has developed, it will tend to enlarge and cause discomfort. If a loop of bowel gets caught in the hernia, it may become obstructed or its blood supply may be cut off. This could then become a life-threatening situation. Since hernias can be repaired effectively and with minimal risk, most surgeons therefore recommend that hernias be repaired when diagnosed, unless there is serious medical problem which makes it too risky. HERNIA and HYDROCELE An inguinal hernia is a common condition in males that occurs when a segment of intestine passes through the abdominal wall in the groin on one or both sides. Inguinal hernia is more common in premature infants and appears as swelling in the groin. It may be more evident with crying and straining. It is important to repair this condition to prevent the intestinal segment from becoming strangulated or "fixed" in the groin wall. The procedure is called a herniorrhaphy. Hydrocele is a condition where the scrotum is abnormally large due to excess fluid in the scrotum and poor drainage. The size of the hydrocele may fluctuate and this condition may occur on one or both sides. Many times a hydrocele is associated with inguinal hernia due to a weakening in the groin.
