Hernia
Definition
Hernia is a general term used to describe a bulge or protrusion of an organ through the structure or muscle that usually contains it. Description
There are many different types of hernias. The most familiar type are those that occur in the abdomen, in which part of the intestines protrude through the abdominal wall. This may occur in different areas and, depending on the location, the hernia is given a different name. An inguinal hernia appears as a bulge in the groin and may come and go depending on the position of the person or their level of physical activity. It can occur with or without pain . In men, the protrusion may descend into the scrotum. Inguinal hernias account for 80% of all hernias and are more common in men. Femoral hernias are similar to inguinal hernias but appear as a bulge slightly lower. They are more common in women due to the strain of pregnancy A ventral hernia is also called an incisional hernia because it generally occurs as a bulge in the abdomen at the site of an old surgical scar. It is caused by thinning or stretching of the scar tissue, and occurs more frequently in people who are obese or pregnant. An umbilical hernia appears as a soft bulge at the navel (umbilicus). It is caused by a weakening of the area or an imperfect closure of the area in infants. This type of hernia is more common in women due to pregnancy, and in Chinese and black infants. Some umbilical hernias in infants disappear without treatment within the first year.

Inguinal hernia Prince Charles is far from alone in needing an operation to repair a hernia. It is the commonest male operation performed in the UK, with the NHS performing over 100,000 hernia repairs every year. What is a hernia? A hernia is caused by a tear to the stomach muscles. In some cases part of the intestine or stomach will bulge through the hole and form a painful lump under the skin. The commonest type of hernia is found in the groin. It is known as an inguinal hernia because the weak spot usually occurs along the inguinal canal, an area where the tube that carries sperm enters the scrotum. Men are more likely to have an inherent weakness along the inguinal canal than women because of the way they develop in the womb. Are there other types of hernia? Yes. Protrusions elsewhere on the abdominal wall are called femoral and umbilical hernias. Highest up of all is the hiatal hernia, which occurs when part of the stomach pushes through the diaphragm separating the chest and the abdomen.

Saddam Hussein: Last seen in public in July Ex-Iraqi leader Saddam Hussein has had an operation to treat a hernia at a Baghdad hospital, officials say.
The deposed president had the procedure under tight security two weeks ago, an Iraqi minister and US officials said. The operation was carried out at Ibn Sina hospital, inside Baghdad's heavily fortified Green Zone. The minister said the operation was a success and that Saddam Hussein - who was captured in December 2003 - was in "good health". Iraq's interim Human Rights Minister, Bakhtiar Amin, confirmed that Saddam Hussein underwent the operation. "The former president is in good health. He underwent a hernia operation at Ibn Sina hospital and it was successful," he told AFP news agency. Heavily guarded Under the previous Iraqi regime Ibn Sina hospital often treated elite members of the Baathist regime and the Iraqi elite.

Heartburn, Hiatal Hernia, and Gastroesophageal Reflux Disease (GERD)
On this page: Gastroesophageal reflux disease, or GERD, occurs when the lower esophageal sphincter (LES) does not close properly and stomach contents leak back, or reflux, into the esophagus. The LES is a ring of muscle at the bottom of the esophagus that acts like a valve between the esophagus and stomach. The esophagus carries food from the mouth to the stomach. When refluxed stomach acid touches the lining of the esophagus, it causes a burning sensation in the chest or throat called heartburn. The fluid may even be tasted in the back of the mouth, and this is called acid indigestion. Occasional heartburn is common but does not necessarily mean one has GERD. Heartburn that occurs more than twice a week may be considered GERD, and it can eventually lead to more serious health problems. Anyone, including infants, children, and pregnant women, can have GERD.

Inguinal Hernia
A hernia is a condition in which part of the intestine bulges through a weak area in muscles in the abdomen. An inguinal hernia occurs in the groin (the area between the abdomen and thigh). It is called "inguinal" because the intestines push through a weak spot in the inguinal canal, which is a triangle-shaped opening between layers of abdominal muscle near the groin. Obesity, pregnancy, heavy lifting, and straining to pass stool can cause the intestine to push against the inguinal canal. Symptoms of inguinal hernia may include a lump in the groin near the thigh; pain in the groin; and, in severe cases, partial or complete blockage of the intestine. The doctor diagnoses hernia by doing a physical exam and by taking x rays and blood tests to check for blockage in the intestine. The main treatment for inguinal hernia is surgery to repair the opening in the muscle wall. This surgery is called herniorrhaphy. Sometimes the weak area is reinforced with steel mesh or wire. This operation is called hernioplasty. If the protruding intestine becomes twisted or traps stool, part of the intestine might need to be removed. This surgery is called bowel resection. (Bowel is another word for intestine.) CHID is a database produced by health-related agencies of the Federal Government. This database provides titles, abstracts, and availability information for health information and health education resources.

Each year about 600,000 hernia-repair operations are performed in the United States. Until recently, however, all were performed as traditional, "open" procedures requiring a large incision in the lower abdomen. The result was significant pain for patients.
Today, the minimally invasive technique of laparoscopic surgery can be used to repair the most common types of hernias. Although both traditional and laparoscopic hernia surgery are performed on an outpatient basis, patients treated laparoscopically seem to experience more rapid healing and far less pain during recovery. They can return to normal activity, including work, after only a few days, while recovery from traditional hernia repair can be a three- to five week process.
The laparoscopic approach can be used to repair direct and indirect inguinal (groin) hernias and femoral (below the groin) hernias. Some hernias located in other parts of the abdominal wall can be repaired laparoscopically as well.

Hernia
Hernia Overview
A hernia occurs when the contents of a body cavity bulge out of the area where they are normally contained. These contents, usually portions of intestine or abdominal fatty tissue, are often enclosed in the thin membrane that naturally lines the inside of the cavity. Although the term hernia can be used for bulges in other areas, it most often is used to describe hernias of the lower torso (abdominal wall hernias). Hernias by themselves usually are harmless, but nearly all have a potential risk of having their blood supply cut off (becoming strangulated). If the hernia sac contents have their blood supply cut off at the hernia opening in the abdominal wall, it becomes a medical and surgical emergency. Different types of abdominal wall hernias include the following:
  • Inguinal hernia: Making up 75% of all abdominal wall hernias and occurring up to 25 times more often in men than women, these hernias are divided into 2 different types, direct and indirect. Both occur in the area of where the skin crease at the top of the thigh joins the torso (the inguinal crease), but they have slightly different origins. Both of these types of hernias can similarly appear as a bulge in the inguinal crease. Distinguishing between the direct and indirect hernia, however, is not that important because both are treated the same.
Indirect inguinal hernia: An indirect hernia follows the pathway that the testicles made during prebirth development. It descends from the abdomen into the scrotum. This pathway normally closes before birth but remains a possible place for a hernia. Sometimes the hernial sac may protrude into the scrotum. An indirect inguinal hernia may occur at any age but becomes more common as people age.

120. The Physician And Sportsmedicine: The 'Sports Hernia'
The predominant complaint of athletes with a sports hernia (table 1) is unilateral groin pain, Clinical Findings in Patients Who Have a Sports hernia
http://www.physsportsmed.com/issues/1998/01jan/batt.htm
The 'Sports Hernia': A Common Cause of Groin Pain
Simon Kemp, MB, BS; Mark E. Batt, MB, BChir
THE PHYSICIAN AND SPORTSMEDICINE - VOL 26 - NO. 1 - JANUARY 98 In Brief: Athletes who participate in sports that require repetitive twisting and turning at speed, such as soccer or ice hockey, may be at risk of developing a 'sports hernia'disruption of the inguinal canal without a clinically detectable hernia. Insidious onset of unilateral groin pain is the most common symptom. Concurrent pathologies, such as osteitis pubis and adductor tenoperiostitis, may complicate diagnosis. Plain radiographs and a bone scan can aid differential diagnosis, but herniography is not recommended. Surgery is the preferred treatment. Structured rehabilitation should enable athletes to return to sports activity 6 to 8 weeks after surgery. I njuries to the groin may constitute 5% of all sports injuries (1) in male and female athletes and are particularly common among male and female soccer players, 28% of whom have a history of groin pain in some series (2). Particularly in male athletes, causes of groin pain without a clinically detectable hernia are increasingly ascribed to disruptions to the inguinal canal, especially in cases resistant to conservative treatment. These disruptions are often termed "sports hernia," "sportsman's hernia," or "athletic pubalgia." Little has been published on the prevalence of this condition among athletes, but Lovell (3) found sports hernias during surgery in 50% of 186 male athletesthe study included 186 males and 3 femaleswho had had groin pain lasting longer than 8 weeks. The condition has occurred in athletes in a wide range of sports but appears to be particularly common in activities requiring repetitive twisting and turning movements at speed. These include soccer, ice hockey, field hockey, tennis, and Australian rules football. It has also been infrequently reported in women (4,5), who describe tenderness at the site of the superficial inguinal ring. This infrequency may be a result of either the limited female participation in the sports previously studied, such as soccer, ice hockey, and Australian rules football, or the gender differences in the anatomy of the inguinal canal. This article largely focuses on male athletes.
