History of the Clinic and Hernia Repair
Until the advent of laparoscopic surgery and the interest in laparoscopic repair of groin hernia, there were only two well known clinics in the world specialising in the treatment of hernia. These clinics were the Shouldice Centre in Canada and the Lichtenstein Centre in the United States. Prior to the advent of the key hole approach which led to a renewed world wide interest in hernia repair, most surgeons were performing a standard Bassini hernia repair. This involved stitching of tissues under tension. When the results of this procedure were critically examined it was found that there was an unacceptably high recurrence rate. The Bassini hernia repair was effective and efficient in 80% of patients but there were many patients troubled by recurrence of hernia and pain after surgery. There was a tendency to treat hernia repair as a relatively minor operation. Because the procedure was so well established in the minds of surgeons and because of the absence of controversy about the established method of repair, there was little pressure for change. read more...

Professor Michael Bailey
As Professor of Surgery and Consultant Surgeon at the Royal Surrey County Hospital and the University of Surrey in Guildford, Michael Bailey is at the forefront of modern abdominal surgery. A pioneer of Laparoscopic surgery and its safe training, Professor Bailey performed one of the first laparoscopic gallbladder removal operations in the UK in 1990 and has since become internationally recognised for his technical and teaching expertise. Most commonly, a hernia is a swelling that you or your doctor discover in your child's inguinal ("groin") area. They are much more common in boys than in girls, although inguinal hernias do occur in girls. A hernia usually pops up without warning and is noticed at bath or diaper change time, often when the child is crying. The area they first appear in lies just above the scrotum, on a line drawn from the scrotum up to the nipple on that side. What has happened is that there was a weakness in the floor of the abdomen where the blood vessels, nerve and spermatic cord leading down to the testicle penetrate the wall of the abdominal cavity. Over time, as the baby cries and normally strains to push out his bowel movements, the pressure gradually works this weakness into an opening through the abdominal wall (first smaller and then larger) and ultimately through a sort of tunnel and down into the testicular sac (scrotum). When the hole is large enough, a short section of intestine can slide down into the tunnel and the scrotum.

Introduction Local anaesthesia may be employed in hernia operations, either on its own or combined with general anaesthesia. The choice of technique will be influenced not only by local resources and skills, but also by patient preference. Advantages of Local Anaesthesia for Hernia Repair With a careful technique, local anaesthesia causes minimal physiological disturbance. This may be particularly useful for patients with cardiovascular or respiratory disease for whom there may be advantages in avoiding a general anaesthetic. The absence of postoperative sedation or drowsiness allows early ambulation and diminishes the requirement for recovery facilities. Local anaesthesia provides postoperative analgesia for up to four hours and may be administered by the surgeon. When adrenaline is mixed with the local anaesthetic (normally in a dilution of 1:200,000) useful vasoconstriction is produced resulting in a relatively bloodless field.
Disadvantages of Local Anaesthesia for Hernia Repair Surgery on the awake patient under local anaesthesia must be carried out gently. Although pain sensation is usually blocked by the anaesthetic, traction on certain tissues, particularly the peritoneum, is uncomfortable. The patient should be warned that some sensation may be experienced during the operation but that it will not be painful. Larger hernias, particularly those with incarcerated bowel may prove unsuitable for local anaesthesia.

A hernia is a protrusion or bulge of a part of the body tissue, fat, or an organ through an abnormal opening of the surrounding tissues. There are numerous types of hernias, each type named after its affected area.
A hernia which can be pushed back into the abdomen is called reducible . Hernias which are not reducible are called incarcerated . If the blood supply to an incarcerated hernia is pinched off the hernia becomes strangulated . A strangulated hernia is an emergency situation and must be brought to the immediate attention of your veterinarian.
Some of the most common types of hernias are: Umbilical hernia
An umbilical hernia is the most common type of hernia found in puppies. In the case of umbilical hernias, a portion of fat or internal organs protrudes through an incompletely closed umbilical ring. Umbilical hernias may be present at birth, or may be acquired. The most common means of acquiring an umbilical hernia is as a result of the umbilical cord being severed too close to the abdominal wall.
In most cases umbilical hernias are small and reduce as the puppy grows. Generally, by the time the pup is six months old the umbilical hernia will shrink and disappear on its own. If the pup has a large hernia, or one that can be pushed into the abdomen with a finger, consult your veterinarian regarding possible surgical repair.

In fact, hernia repair is the one of the most common surgeries performed on children. Hernias can be treated. But it's important to recognize the symptoms of the different types of hernias so that you can get your child the appropriate medical care and prevent the hernia from leading to other medical problems. What Causes Hernias?
When part of an organ or tissue in the body (such as a loop of intestine) pushes through an opening or weak spot in a muscle wall, it can protrude into a space where it does not belong. This protrusion is called a hernia. A hernia may look like a bulge or lump.
Some babies are born with various small openings inside the body that will close at some point, but have not closed yet. Nearby tissues can squeeze into such openings and become hernias. Sometimes tissues can squeeze through muscle wall openings that are only meant for arteries or other tissues. In other cases, strains or injuries create a weak spot in the muscle wall, and part of a nearby organ can be pushed into the weak spot so that it bulges and becomes a hernia.
