President's Initiative on Race
On February 21, 1998, President Clinton announced a new initiative that sets a national goal of eliminating by the year 2010, longstanding disparities in health status that affect racial and ethnic minority groups. The president announced that the federal government will, for the first time, set high national health goals for all Americans, ending a practice of separate, lower goals for racial and ethnic minorities. To help reach these ambitious targets, the president also announced a plan to mobilize the resources and expertise of the federal government, the private sector, and local communities to eliminate disparities that for too long have been treated as intractable. The plan includes the following:

Health Care, Nutrition, and Goal One. ERIC Digest.
In April, 1993, President Clinton sent to congress the Goals 2000: Educate America Act, which embraces the objectives of Goal One of the six national education goals. Goal One states that "By the year 2000, all children in America will start school ready to learn." One of the objectives formulated to meet this goal asserts that "Children will receive the nutrition and health care needed to arrive at school with healthy minds and bodies." Problems which can affect children's later development and school readiness include lack of prenatal care, low birth weight, maternal substance abuse, malnutrition, vaccine-preventable diseases, sensory impairments, high blood lead levels, and anemia. Efforts to promote children's health and nutrition, and to prevent children's illness, include federal and state programs, and initiatives to serve preschool children in health care settings, in preschool programs, and in community settings. These efforts address prenatal care, health and nutrition of the developing child, parent education, preschool teacher education, and health and nutrition instruction for preschool children.
FEDERAL AND STATE PROGRAMS
Many of the federal programs profiled above are administered by states, or are jointly funded with federal and state monies. Several states have launched efforts of their own to serve preschoolers' health and nutrition needs. The Kentucky Education Reform Act of 1990 authorized the establishment of family service centers in many of the state's school districts. Project Healthy Start in Hawaii consists of one-stop service centers located throughout the state to serve children and families at risk (Boyer, 1991). As part of Project Network of the Mississippi State Department of Health, nutritionists provide support and technical assistance to caregivers; develop education materials; and develop and implement community-based educational projects for nutrition students and health professionals (Ford, 1991).

  • Economic Impact Studies
    According to a study published in the January 21 issue of Obesity Research, providing medical treatment to obese Americans cost $75 billion in 2003. The study, State-Level Estimates of Annual Medical Expenditures Attributable to Obesity , was sponsored by the Centers for Disease Control to estimate the state-by-state costs of obesity, separate from costs incurred from those related to overweight, which cumatively amounts to $93 billion. Several state legislators have been confronted with concrete numbers of how much obesity has affected their states in terms of health care costs, from $87 million in Wyoming to $7.7 billion in California. These numbers may be used by lawmakers to justify obesity related legislation touching on snack taxes, physical education standards and vending machine restrictions.

    Target Population
    The estimated 43,500 Native Hawaiians residing on the island of Hawaii, mostly in coastal towns and villages.
    Services Provided
    Hui Malama focuses on providing preventive health services. Staff provides health education and outreach and transportation to and from treatment. Care managers provide assistance in getting health insurance, medications, scheduling doctor appointments and routine health maintenance. Hui Malama does screenings for prostate, breast, and cervical cancer; cholesterol; high blood pressure; and diabetes; and promotes colo-rectal education and testing. Prenatal classes and referrals are also available. A nutritionist provides diet and nutrition counseling.
    Major Health Issues
    One of the greatest challenges faced by people residing on the island of Hawaii is the great distance they have to travel to receive health care. Hui Malama has worked to alleviate that problem by setting up outreach offices in remote areas, and by providing transportation to and from the doctor in four of its own vans. Hui Malama has also worked hard to make Native Hawaiians more aware of their health needs through screening and education programs.
    Staffing
    The Hui Malama program has a professional staff that includes an executive director, three care managers, seven outreach workers, and a nutritionist.

    Hoola Lahui Hawaii's (HLH) mission is to preserve and perpetuate the Hawaiian race by achieving community wellness through innovative and culturally appropriate health services and education programs. The organization serves the island of Kauai and individuals from Niihau. Established as one of five federally funded Native Hawaiian Health Care Systems, Hoola Lahui Hawaii provides care to Hawaiians and their families with a primary focus on health promotion and prevention. As of September 2001, HLH is also a section 330 community health center.
    Special Populations/Health Issues
    Native Hawaiians; immigrants. As Kauai has a small island population, the number of services available is limited for obvious economic reasons. Additionally, many Hawaiians do not feel comfortable receiving care from western medical practitioners.
    Services
    HLH provides health services including comprehensive dental services and limited primary care services on its mobile health outreach van. The agency also provides programs in health promotion, disease prevention, health education, nutrition, exercise, and chronic disease management.
    Patient Profile
    Since 1992, Hoola Lahui Hawaii has served more than 8,000 individuals and continues to serve some 1,200 annually. As funding is primarily geared towards Hawaiians, HLH targets this population and their families. However, services are open to anyone based on a sliding fee scale. HLH continues to serve the uninsured with over 65% of their clients having no insurance.

    Colorado: Launching Colorado on the Move TM : A Comprehensive Effort to Increase Physical Activity
    Public Health Problem
    Between 1987 and 2000, obesity reached epidemic proportions in the United States, with more than 45 million adults classified as obese. In Colorado, adult obesity (body mass index [BMI] 30) rates increased from 6.9% in 1990 to 14.9% in 2001; the prevalence of overweight (BMI 25) among adults also rose from 36.7% to 51.6% during those years. Evidence That Prevention Works
    Research has demonstrated that preventive measures such as improved nutrition, tobacco cessation, increased physical activity, and early detection and intervention may prevent heart disease, stroke, and other chronic diseases. Physical activity helps to control weight; contributes to healthy bones, muscles, and joints; reduces falls among the elderly; helps to relieve the pain of arthritis; reduces symptoms of anxiety and depression; and is associated with fewer hospitalizations, physician visits, and medications. Program Example
    Through a partnership with the University of Colorado Health Sciences, the Colorado Physical Activity and Nutrition Program is implementing two community interventions in Peetz, Colorado, which has a large rural population, and in the Denver Metro Black Churches, which allows program coordinators to work in an already established urban and African American setting to reach high-risk populations. These interventions focus on a physical activity component to encourage participants to walk 2,000 steps a day more than they walked before the program. Pedometers are offered to assist participants in their efforts. Additional worksites and community sites throughout the state are also participating. The intervention will introduce a nutrition component as well, most likely promoting a 5 A Day campaign. The program has been named

    Information will be presented on building and working with community coalitions using the “Children and Weight: What Schools and Communities Can Do” Kit developed by the University of California. The Spectrum of Prevention model will be presented, as will examples from three different county-based coalitions who have successfully garnered resources of all kinds through their collaborative efforts. These model coalitions will demonstrate their efforts in promoting health for all children using a multi-level coalition approach.
