This site is for anyone concerned about the genetic health and well-being of individuals and families in the Jewish community.
It is estimated that everybody carries 6 - 8 disease-producing genes, which would be harmful if passed on to our children by both mother and father. Many racial and ethnic groups have "their own" genetic disorder disorders which are not unique to the group, but which occur more frequently in the group. The "Jewish" genetic disorders are a group of conditions that are more common among individuals of Jewish descent. Although these disorders can occur in the general population, they do so at a significantly lower frequency than in the Jewish population.

The body's genetic material is contained within the nucleus of each of its cells, which in an adult person number over 5 trillion. The genetic material consists of coils of DNA (deoxyribonucleic acid) arranged in a complex way to form chromosomes. Human cells each contain 22 pairs of nonsex (autosomal) chromosomes, and one pair of sex chromosomes, for a total of 46 chromosomes. Each DNA molecule is a long double helix that resembles a spiral staircase. The steps of the staircase, which determine a person's genetic code, consist of pairs of four types of molecules called bases (nucleotides). In each step, adenine (A) is paired with thymine (T), or guanine (G) is paired with cytosine (C). The genetic code is written in triplets, so each group of three bases codes the production of one of 20 possible amino acids, which are the building blocks of proteins. For example, "GCT" codes for the amino acid alanine, while "AAA" codes for the amino acid lysine. A gene consists of the code required to construct one protein. Thus, a gene is a collection of DNA in sequence. Genes vary in size, depending on the size of the protein they code for. All inherited characteristics (traits) are encoded by genes (although many traits are governed by more than one gene). Some genetically determined characteristics, such as hair color, simply distinguish people from one another; variations in hair color are not considered abnormal. However, other genetically determined characteristics are important for the body's normal structure or function; variations in the genes controlling such characteristics may result in a hereditary disease.

Comment:
In the not too distant past, any infant or child who had an unexplained developmental disorder that included muscle spasticity and/or unusual body movements (athetosis; dystonia) was often referred to as having cerebral palsy (CP). The children were included in the cerebral palsy category even though they had other major problems which were only sometimes associated with cerebral palsy (e.g. blindness; profound behavioral disturbance, frequent convulsions; muscle twitching; cardiac failure; etc); however, for lack of a better diagnosis, the term cerebral palsy was used.
With the development of genetic testing, we now know that a large number of these unusual presentations which were called C.P. are in fact genetic disorders which affect the child's total body metabolism, only one symptom of which is muscle spasticity and/or poor muscle coordination. There are over 100 of these genetically based metabolic disorders that generally affect many parts of the body, only one of which is the brain. They are usually relatively rare; often have occurred previously in other family members; the symptoms often increase in severity over time and can eventuate in death in childhood or adolescence. Treatments have been developed for some that serve to arrest the metabolic problem; however, many have no useful treatment available at this time.

Gene Therapy
Gene therapy represents a new field of medicine, one which can potentially cure many genetic diseases such as bleeding disorders. Patients with inherited diseases such as hemophilia usually lack a functional protein, which can be replaced with periodic protein replacement therapy. An alternative is to add a normal gene to substitute for the defective one. The normal gene, if delivered to the right cells and in sufficient quantity, can continuously make the normal version of the defective protein, restoring normal clotting mechanisms.
However, this is a new and highly experimental field. Many questions remain unanswered and will only be answered through a series of clinical studies, some of which are ongoing using the genes for factors VIII and IX. There have been many failures and disapointments when investigators have found that the technology for delivering the genes was inadequate when used in humans. Sometimes investigators have underestimated the complexity of the diseases they were attempting to cure. Because of some recent successes, hemophilia has emerged as the top candidate for a genetic disease most likely to benefit from gene therapy.
