Signs and symptoms
Emphysema is characterised by loss of elasticity of the lung tissue; destruction of structures supporting the alveoli; and destruction of capillaries feeding the alveoli. The result is that the small airways collapse during expiration, leading to an obstructive form of lung disease (air is trapped in the lungs in obstructive lung diseases). Features are: shortness of breath on exertionparticularly when climbing stairs or inclines (and later at rest), hyperventilation and an expanded chest. As emphysema progresses, clubbing of the fingers may be observed, a feature of longstanding hypoxia Emphysema patients are sometimes referred to as "pink puffers". This is because emphysema sufferers may hyperventilate to maintain adequate blood oxygen levels. Hyperventilation explains why emphysema patients do not appear cyanotic as chronic bronchitis (another COPD disorder) sufferers often do; hence they are "pink" puffers (adequate oxygen levels in the blood) and not "blue" bloaters (cyanosis; inadequate oxygen in the blood). edit

http://oac.med.jhmi.edu/res_phys/Encyclopedia/Emphysema/Emphysema.HTML
Emphysema
Emphysema is a disease characterized by dilation of the alveolar spaces and destruction of the alveolar walls. With their loss, much of the elastic recoil of the lung is also lost. Compliance of the lung in emphysema is significantly above normal; the lung becomes easy to distend but empties slowly. This results in a chronically overinflated lung (high total lung capacity, functional residual capacity, and residual volume), which lessens the curvature of the diaphragm, making it less efficient in generating even the small swings in pleural pressure necessary for breathing. Pulmonary function tests on a patient with emphysema will reveal a compromised expiratory flow (due to their low lung recoil), including a low FEV1, FVC, and FEV1/FVC ratio. Chronic bronchitis and emphysema
Chronic bronchitis and emphysema stop the lungs from working properly, typically causing breathlessness and wheezing. The two conditions lead to similar symptoms and are often referred to as "chronic obstructive pulmonary disease", or COPD for short.
What causes COPD?
The biggest single cause of COPD is smoking, and the risk increases with the number of cigarettes smoked. Due to industrial smoke and pollutants, it is also more common in people living in cities than in the country, and in people who work in dusty environments. Generally, it is more likely to affect older people and the vast majority of sufferers are aged 70 to 85. Each year, around 28,000 people die from COPD.
Chronic bronchitis
Bronchitis means inflammation of the bronchi – the main airways that lead from the trachea (windpipe) into the lungs. This inflammation may be caused by irritants, such as tobacco smoke or by infection.

Emphysema Bullous emphysema Emphysema is an enlargement of air spaces caused by destruction of alveolar walls. Air spaces greater than one cm are bullae. This photo shows apical bullous disease with relatively little involvement of the rest of the lung. Centriacinar emphysema/distal acinar emphysema This lung shows centriacinar emphysema with holes located around the terminal bronchioles and preservation of more distal air spaces. The parenchyma along the major fissure shows some distal acinar emphysema. Panacinar emphysema involves the entire acinus. Centriacinar emphysema This section of centriacinar emphysema shows the enlarged air spaces around a small airway. Respiratory epithelium remains at the arrow. The more peripheral alveoli are normal. Pathogenesis of emphysemaproposed This photo is taken from a patient with alpha-1-antitrypsin deficiency-related emphysema. It shows an increased number of PMNs in capillaries (arrows and many more) and some alveolar and interstitial blood. These PMNs, if activated, can potentially digest the collagen and elastin of the alveolar wall. Note the megakaryocyte in a capillary (arrowhead at top). Its function is not known, but any contribution of platelets would help to preserve the integrity of the capillary bed. Contents

Types of Emphysema This lung was dried and sliced. It shows the two major types of emphysema. What are they? Answer In a closer view, another type of emphysema can be seen along the major fissure. What type is this? Answer An infrequent type of emphysema is shown here at the lung apex. What is it? Answer This section illustrates which type of emphysema? Answer This photo is taken from a patient with alpha-1-antitrypsin deficiency-related emphysema. It shows an increased number of PMNs in capillaries (arrows and many more) and some alveolar and interstitial blood not thought to be related to the pneumonectomy. These PMNs can potentially digest the collagen and elastin of the alveolar wall (see discussion). Note the megakaryocyte in a capillary (arrowhead at top). Its function is not known, but any contribution of platelets would help to preserve the integrity of the capillary bed. Clinical summary Discussion Table of Contents Answer: The lung shows panacinar (synonym: panlobular) emphysema in the apical segment. It shows centriacinar (synonym: centrilobular) emphysema in the remainder of the lung, but note that the upper lobe is involved more than the middle and lower lobes.

BEST/TIP Statement of Principles concerning CHRONIC BRONCHITIS AND EMPHYSEMA for the purposes of the Veterans’ Entitlements Act 1986 and Military Rehabilitation and Compensation Act 2004 and of the Veterans’ Entitlements Act 1986 (the VEA): Kind of injury, disease or death chronic bronchitis and emphysema and death from chronic bronchitis and/or emphysema , either alone or in combination. "chronic bronchitis" means a respiratory tract disorder characterised by excessive mucus production sufficient to cause cough and sputum for at least three months of each year for at least two consecutive years, where such mucus production is not attributable to another respiratory disease. "emphysema" means a bilateral and diffuse respiratory tract disorder which is characterised by distension of airspaces distal to the terminal bronchiole with destruction of alveolar septa, and without obvious fibrosis. This definition excludes isolated emphysematous bleb and surgical, traumatic, unilateral, focal or localised emphysema, and Swyer-James syndrome (also known as MacLeod's syndrome or hyperlucent lung syndrome).
