Living with a digestive disorder
Everyone gets a stomachache from time to time. It is typically easily treated and goes away quickly. When the pain lasts longer and occurs more frequently, it may be something more serious, such as a digestive disorder. Serious digestive disorders can affect people for a lifetime, and doctors say they are on the rise.

Miguel Regueiro, MD
, a gastroenterologist and clinical director of the UPMC Inflammatory Bowel Disease Center According to Dr. Regueiro, better understanding of the condition, improved diagnosis through targeted testing, and less reluctance to diagnose gastrointestinal (GI) disorders in pediatric and younger patients may also be part of the equation. However, he says, clearly something in the environment has changed. The Division of Gastroenterology at UPMC is consistently recognized by for its efforts to understand the disease and provide the best care to patients.

A European Solution
For Digestive Disorders
The difference in life expectancy between the best and worst European countries is more than 10 years. In the early 1990s, overall Eastern European mortality was 20 to 100% higher compared to the West. The reasons for these differences in mortality are attributed to poor diet, excess alcohol consumption, heavy smoking and other dangerous health behaviors. One dietary explanation for the decreased life span among Eastern Europeans is that their intake of antioxidants from fruits, vegetables and nuts is much lower compared to the West. A severe deficiency of antioxidant vitamins, along with low intake of folic acid and flavonoids, partially accounts for the high level of cardiovascular disease in Eastern Europe.

Scientists can see right inside the intestines using fibre optics. Thousands of flexible optical fibres are bundled together in a tube. They carry an image back from inside the patient to the eyepie. The doctor can see directly into the patients guts. Sigmoidoscopy
To examine the large intestine (the lower part of the colon called the sigmoid), doctors perform a sigmoidoscopy. The sigmoidoscope is similar to an endoscope but it is inserted through the anus and passes up into the large intestine. The doctor can take samples of the lining ( biopsies ) to check for infection or cancerous growths. Picture 19. See question 6.

Facts About Motility Disorders
What are Motility Disorders?
Motility disorders refer to abnormal intestinal contractions, such as spasms and intestinal paralysis, that can result in the inability to eat (which may require patients to receive intravenous nutrition) and severe abdominal pain, nausea, vomiting, maldigestion, weight loss, diarrhea, constipation and incontinence.
Motility Disorders encompass a variety of conditions, including: gastroparesis; achalasia; gastroesophageal reflux disease; chronic intestinal pseudoobstruction; constipation and irritable bowel syndrome. Irritable bowel syndrome, the most common of the disorders, results in pain and diminished quality of life for nearly 15 percent of this country's population. Rarer and more serious disorders produce excruciating pain and make it impossible to eat or digest food.
Below is a listing and short description of some of the most serious and bothersome motility disorders and digestive diseases.
Gastroparesis
This is a disorder of the stomach where it fails to empty food properly. Symptoms include nausea, vomiting, weight loss and loss of appetite. It commonly occurs in patients with diabetes, or in patients with scleroderma or lupus. However, it may also occur after a viral or bacterial illness.
Gastroparesis Diet

Irritable bowel syndrome Inflammatory bowel disease
is a blanket term covering three serious disorders: ulcerative colitis, proctitis, and Crohn's disease. Ulcerative colitis is an inflammation of the colon that produces ulceration of the inside wall. Its primary symptom is bloody, chronic diarrhea, often containing pus and mucus, and associated with abdominal pain and weight and appetite loss. When confined to the rectum, it's referred to as ulcerative proctitis. Though there is no known cause of ulcerative colitis, it does tend to run in families and is more common in Jews and caucasians than other ethnic or racial groups. Sometimes known as ileitis, Crohn's disease is an inflammation of the small and/or large intestine, with accompanying pain, cramping, tenderness, gas, fever, nausea, and diarrhea. Though usually mild, bleeding may occur and may sometimes be massive.

From Our Sponsors Hemorrhoids are dilated veins in the lower rectum and anus. They affect 50 percent of people over age 50. Diets low in fiber are thought to be a major cause. Straining at defecation to evacuate hard stools also seems to promote their development. A high-fiber diet and prompt, strain-free defecation help relieve mildly symptomatic hemorrhoids. Local swelling and pain can be decreased with local anesthetic creams, lotions, or suppositories; astringents; or cold compresses. For those with severe symptoms, rubber band ligation, cryosurgery, injection therapy, or surgical removal may be needed. Constipation is the term given for infrequent or difficult evacuation of small, hard stools, accompanied by mild abdominal discomfort. When the condition is chronic, it may include nausea, stomach rumbling, appetite loss, and malaise. Constipation is a common condition that affects mostly the very young and the aged. While the number of bowel movements is variable in the general population, a normal individual should have at least three per week, or, ideally, one daily.

Crohn's Disease, Irritable Bowel Syndrome, and Coconut Oil for Digestive Health Digestive Health and Weight Gain When we are eating healthy whole foods with the absence of highly refined carbohydrates, weight gain should not be a problem. However, many people who have switched over to healthy diets, and have even limited their carbohydrate intake, still have problems losing weight. One reason is because many Americans suffer from some kind of digestive disorder that prevents them from properly digesting their food. Without a properly working digestive system, essential vitamins and minerals that are necessary to maintain proper weight may not be absorbed adequately from the foods we eat, even if we are eating healthy foods. Candida is one digestive disorder where the digestive tract is overloaded with yeast because of the lack of probiotics in our digestive system. You can read more about Candida and coconut oil here . Other digestive disorders common today would include disorders such as irritable bowel syndrome, ulcerative colitis, gastritis, diverticulosis and constipation. More severe digestive disorders would include Crohn’s Disease, where many people have such a hard time digesting foods properly that they are actually underweight.

Living With Lactose Intolerance
What Is Lactose Intolerance? Lactose intolerance is a deficiency of the enzyme lactase, which is responsible for the digestion of lactose, the major sugar in milk. (Milk and all products made from milk contain lactose.) Normally lactase, which is produced in the lining of the small intestine, breaks down lactose into the simpler sugars glucose and galactose. The liver then converts galactose into glucose, which enters the bloodstream and is used by cells as an energy source. Most people with lactose intolerance do not have enough lactase to digest lactose properly. Diagnosing Lactose Intolerance Lactose intolerance usually develops slowly, and the symptoms associated with it can have other causes. If you think you are lactose intolerant, you can first check for it on your own. Try excluding all dairy products from your diet for about three days; if drinking a glass of milk after this period causes symptoms, you may be lactose intolerant. Your doctor can confirm a diagnosis of lactose intolerance by taking a medical history and discussing with you the results of your dietary manipulations. If necessary, the doctor can run a blood or breath test to confirm the diagnosis.

Gallbladder Disorders Symptoms Moderate to severe pain in the upper-right abdomen, often spreading into the chest, right shoulder, and back. Pain due to gallstones may occur in separate episodes lasting from 30 minutes to four hours. Inflammation of the gallbladder causes pain that gradually increases in severity and may worsen upon deep breathing. Cancer of the gallbladder produces steady pain. Possible nausea and vomiting accompanying the attacks of pain. Fever and chills (gallbladder inflammation). Abdominal fullness or bloating. Severe belching, heartburn, and gas. Weight loss (cancer of the gallbladder). Jaundice. When To Call Your Doctor Call a doctor if you develop severe abdominal pain. Digestive Disorders Protocol
It is estimated that some form of digestive disorder affects more than 100 million people in America. That is more than half of the U.S. population. For some people, digestive disorders are a source of irritation and discomfort that may cause them to drastically limit their lifestyles and to frequently miss work. For others, the disorders may be extremely crippling and even fatal.
The Gastrointestinal Tract The gastrointestinal tract (GIT) is a long muscular tube that functions as the food processor for the human body. The digestive system includes the following organs: mouth and salivary glands, stomach, small and large intestines, colon, liver and pancreas, and the gallbladder. Irritations or inflammation of the various sections of the GIT are identified as gastritis (stomach), colitis (colon), ileitis (ileum or small intestines), hepatitis (liver), and cholecystitis (gallbladder). The GIT is not a passive system. Rather, it has the capability to sense and react to the materials that are passed through it. For a healthy digestive system, every person requires different food selections that match their GIT capacity.

GI generally falls into two categories — functional and inflammatory... Functional GI disorders , such as Irritable Bowel Syndrome (IBS), chronic diarrhea, constipation and intestinal pain, are characterized by symptoms and not by a visible sign of disease or injury. Inflammatory GI disorders include Inflammatory Bowel Disease (IBD), Crohn’s Disease and ulcerative colitis. Women are three times more likely than men to develop IBS, and most IBS sufferers experience their first symptoms before their mid-thirties. Irritable bowel syndrome (IBD), a prominent GI Disorder usually begins in early adulthood and results in recurring cramping, pain or discomfort in the lower abdomen; bloating; diarrhea; constipation; or alternating diarrhea and constipation. Symptoms can range from mild to severe, and can occur daily or periodically. The syndrome affects 25 to 45 million Americans, about two-thirds of them women, according to the International Foundation for Functional Gastrointestinal Disorders in Milwaukee. Doctors suggest that hormones seem to be a contributing factor in this disease. Dr. Lawrence J. Brandt, the chief of gastroenterology at the Montefiore Medical Center in the Bronx agrees that during some phases of the menstrual cycle, intestinal activity is not normal as it is slower in some women and faster in others. Dr. Brandt believes that there is a clear relationship between hormones and IBD but it could not be considered a hormonally mediated disease.

Celiac Disease A Gluten-Free Diet Persons on a gluten-free diet must avoid all foods containing wheat, rye, barley and (possibly) oats. This includes foods containing spelt, triticale, and kamut and most grain, pasta, cereal and processed foods. While this excludes many foods, persons with celiac disease can still enjoy a well-balanced diet, substituting foods made with potato, rice, soy, or bean flour. There are also a variety of gluten-free breads and pastas available at health food stores and through special food companies. Scientists are still trying to determine whether persons with celiac disease can eat oats, as some people are able to without having a reaction. Until studies are more definitive, persons with celiac disease should follow their physician's or a dietitian's advice. Plain meat, fish, rice, vegetables, and fruits do not contain gluten, so these can be eaten without worry. It is important for persons with celiac disease to be especially careful when dining out, or eating at school, work, or social functions. A gluten-free diet must be a lifetime commitment to keep celiac disease under control. Consult your physician, dietitian, or other healthcare professional to learn more about a gluten-free diet.

What is irritable bowel syndrome (IBS)?
Irritable bowel syndrome (IBS) is an intestinal disorder that causes the following:
  • crampy pain gassiness bloating changes in bowel habits
IBS has inaccurately been called by many names, including the following:
  • colitis mucous colitis spastic colon spastic bowel functional bowel disease
IBS is called a functional disorder because there is no sign of disease when the colon is examined. Because physicians have been unable to find an organic cause, IBS often has been thought to be caused by emotional conflict or stress. While stress may worsen IBS symptoms, research suggests that other factors also are important. IBS often causes a great deal of discomfort and distress, but it is not believed to:
  • cause permanent harm to the intestines. lead to intestinal bleeding of the bowel. lead to a serious disease such as cancer.
It has not been shown to lead to serious, organic diseases, nor has a link been established between IBS and inflammatory bowel diseases such as Crohn's disease or ulcerative colitis.
