By TINA ALTAMIRANO Waging battle in the weight-loss war, the recent low carbohydrate diet craze exhibits promise as a possible treatment for diabetes. As the seventh leading cause of death for Hispanics, diabetes is a perpetual concern in the Latino community.
Often linked to weight control and food selection, diabetes ailments include foot maladies, blindness, and kidney and heart diseases that often result from the excessive and erratic blood sugar, or glucose levels that characterize the disease. Prescribed by an increasing number of doctors in conjunction with medication to control and stabilize diabetics' dangerous glucose levels, low carbohydrate diets eliminate or severely restrict many foods that directly affect these levels. Steve Hopkins, coordinator for Walk the Cure, a research fundraiser at the Juvenile Diabetes Foundation in Austin, Texas, says that his low carbohydrate diet plan "allows a lot more bang for your food buck.

The foods convertible into sugar in the digestive canal are those containing starch (not gums), such as arrowroot, tapioca, sago, flours of all the different kinds of cereals (wheat, barley, oats, peas, beans, etc.), potatoes, carrots, beetroot. parsnips, turnips, and other edible roots. Green vegetables, on the other hand, such as spinach, cabbage, turnip tops, Brussels sprouts, and lettuce need not be forbidden, as they contain too small an amount of starch to do much injury. After a time patients get very tired of these substitutes, so it is as well to know that we may occasionally indulge them with well done toast, or very crisp pulled bread, the extra heat having destroyed a considerable portion of the starch normally contained in the article. Even in the most favourable cases for restricted diet, we must never allow ourselves to be deluded into the idea that, because we are mitigating the symptoms, and reducing the amount of sugar in the urine, we are necessarily curing the disease, or we shall frequently be be doomed to sad disappointment. In keeping the patient on restricted diet, we are merely witholding from him the straw and mortar out of which the bricks are made - not removing the makers - so that, as soon as the straw and mortar is refurnished to them, they will again be found at work as actively as ever.

Last Updated: 2005-04-19 15:37:51 -0400 (Reuters Health) NEW YORK (Reuters Health) - Lifestyle changes are more effective than drug treatment in preventing metabolic syndrome, the cluster of disorders that can lead to diabetes and heart disease, according to a new study. Dr. Trevor J. Orchard, at the University of Pittsburgh in Pennsylvania, and colleagues followed 3234 people who had not yet developed outright diabetes but who had high blood glucose levels. The subjects were enrolled in the Diabetes Prevention Program, and were randomly assigned to take the anti-diabetes drug metformin, or an inactive placebo pill, or to undertake an intensive lifestyle intervention designed to achieve and maintain a 7 percent weight loss and 150 minutes of exercise per week. As reported in the Annals of Internal Medicine, metabolic syndrome which is a combination of obesity, high cholesterol, high blood pressure and high blood sugar levels was diagnosed in 53 percent of the participants overall when they were enrolled in the study. After an average 3-year follow-up, the rate of metabolic syndrome decreased from 51 percent to 43 percent in the lifestyle group. However, it increased from 55 percent to 61 percent in the placebo group, and from 54 percent to 55 percent in the metformin group.

Last Updated: 2005-04-19 16:48:39 -0400 (Reuters Health) NEW YORK (Reuters Health) - Lifestyle changes can be more effective than metformin in preventing metabolic syndrome and reducing its prevalence, according to a study of subjects with impaired glucose tolerance. Dr. Trevor J. Orchard, at the University of Pittsburgh in Pennsylvania, and colleagues followed 3234 nondiabetic subjects with fasting plasma glucose level of 5.3 mmol/L or higher enrolled in the Diabetes Prevention Program. As reported in the April 19th issue of the Annals of Internal Medicine, the subjects were randomized to metformin 850 mg b.i.d. (n = 1073), placebo (n = 1082) or intensive lifestyle intervention designed to achieve and maintain a 7% weight loss and 150 minutes of exercise per week (n = 1079). Metabolic syndrome was diagnosed in 53% at baseline between 1996 and 1999. After follow-up averaging 3.2 years, metabolic syndrome had resolved in 18% in the placebo group, 23% of the metformin group and 38% of the lifestyle group. Metformin was associated with reductions in waist circumference and fasting glucose levels, while lifestyle reduced the incidence of all components of metabolic syndrome except HDL cholesterol level.

Diabetes Diabetes mellitus is a condition in which sugar builds up in the bloodstream. Normally, insulin escorts it into the cells. In diabetes, however, insulin works poorly or not at all, allowing sugar to build up and, eventually, to filter through the kidneys into the urine. People with diabetes are at greatly increased risk for circulatory problems in the heart, eyes, kidneys, and extremities. Both insulin-dependent and non-insulin dependent forms of diabetes have strong links to diet for both their causes and treatment. Insulin-Dependent Diabetes While this hypothesis remains controversial, the evidence supporting it is strong. In 1992, the New England Journal of Medicine A 1996 research study published in The Lancet , again suggested that milk may contribute to diabetes in children. Non-Insulin Dependent Diabetes In adult-onset (or non-insulin-dependent) diabetes, insulin is present in the blood, but is not working adequately. This form of the disease can improve or even disappear with the right kind of diet and exercise. Diabetes often accompanies obesity and remits with weight loss. It is also associated with high-fat diets. Although there is a strong genetic component to diabetes, diet and exercise often determine whether the genetic endowment will be expressed or not.

WHAT IS DIABETES?
The two major forms of diabetes are type 1 (previously called insulin-dependent diabetes mellitus (IDDM) or juvenile-onset diabetes) and type 2 (previously called noninsulin-dependent diabetes mellitus (NIDDM) or maturity-onset diabetes). Insulin
Both diabetes type 1 and type 2 share one central feature: elevated blood sugar ( glucose ) levels due to absolute or relative insufficiencies of insulin , a hormone produced by the pancreas. Insulin is a key regulator of the body's metabolism. It normally works in the following way:
  • During and immediately after a meal the process of digestion breaks carbohydrates down into sugar molecules (of which glucose is one) and proteins into amino acids.
  • Right after the meal, glucose and amino acids are absorbed directly into the bloodstream, and blood glucose levels rise sharply. (Glucose levels after a meal are called postprandial levels.)

If your blood sugar is too low or too high, you may need a change in your diabetes medication, diet, or exercise plan. If a change is needed, your doctor or nurse will give you instruction on what you should do. A blood test also can help you decide how to take care of your diabetes. For example, if you feel that your blood sugar is low, a test can tell you whether or not it is too low. Your blood sugar level is found by testing a small drop of blood obtained from one of your fingers. This drop of blood is obtained by sticking your finger with a lancet. The drop of blood is then put on a strip that is placed in a meter to be read. There are many meters on the market today that will read a strip. You need to be sure that you can do the test the right way. Pick a meter that will be easy for you to use. Ask your nurse to help you pick a meter and then teach you how to use it. Blood sugar is usually checked one to four times each day. Ask your doctor or nurse how often you should check your blood sugar. Testing is usually done before meal time.
