SUSCEPTIBILITY AND RESISTANCE
Susceptibility is universal, but children generally have milder illness than adults. All four dengue serotypes produce clinically identical disease, and all can produce DHF and dengue shock syndrome (DSS) in decreasing order of frequency: serotypes 2, 3, 4, and 1. Individuals infected with one strain maintain lifelong homotypic immunity while remaining susceptible to infections with other heterotypic strains. Interestingly, DHF/DSS is more likely to develop if an individual previously infected with one serotype is later inoculated with a different viral strain. DHF and DSS usually occur as a second dengue infection in children and in infants born to dengue-immune mothers. Repeated episodes of DHS/DSS have not been described in the same individual.
CLINICAL FEATURES
DENGUE HEMORRHAGIC FEVER AND DENGUE SHOCK SYNDROME
Clinical criteria for DHF and DSS are shown on the Table on the previous page and include hemoconcentration (Hct > 20% of baseline), thrombocytopenia, and circulatory collapse, often associated with severe end-organ dysfunction. DHF and DSS are recognized primarily in children; in tropical Asia, DSS is observed almost exclusively among indigenous children 15 years of age and younger. Ilness is often biphasic, beginning abruptly with fever, malaise, headache, anorexia, nausea and vomiting, cough, and facial flushing. Severe bone and limb pain are often absent. Coincident with defervescence, the patient’s condition worsens with profound weakness and prostration, diaphoresis, restlessness, facial pallor and circumoral cyanosis, cool and clammy extremities, rapid but thready pulse, and a narrow pulse pressure (

October 6, 1999 Web posted at: 2:58 p.m. EST (1858 GMT) From Correspondent Charles Zewe NUEVO LAREDO, Mexico (CNN) The biggest outbreak of "dengue fever" to hit south Texas in almost 20 years has sickened more than a dozen people in the border town of Laredo. Epidemiologists there have confirmed 14 cases of the viral illness, which is spread by mosquitoes and nicknamed "breakbone fever." Over 100 cases have been identified just across the Mexican border and many more are suspected. Dengue is very painful, but rarely fatal.

Dengue Fever
(breakbone fever, dengue hemorrhagic fever) What is dengue fever? Dengue fever is a mosquito-borne disease caused by a virus. The disease is mainly tropical in origin but occasionally residents or visitors from other countries may arrive in this country with dengue fever. Although cases originating in the United States are virtually unknown, epidemic levels have recently been reported in parts of the Caribbean and Central America. Who gets dengue fever? Dengue fever may occur in people of all ages who are exposed to infected mosquitoes. The disease occurs mainly in tropical Asia and the Caribbean, usually during the rainy seasons in areas with high numbers of infected mosquitoes. How is dengue fever spread?

SAN SALVADOR, El Salvador (AP) President Francisco Flores declared a national emergency this week after an epidemic of dengue fever claimed a 17th victim. Dengue fever is transmitted by the bite of infected mosquitoes and is characterized by the sudden onset of high fever, severe headaches, severe joint and muscle pain, nausea, vomiting and a rash.

Indonesia battles dengue fever

Story Tools JAKARTA, Indonesia (CNN) Indonesia is battling an early outbreak of dengue fever with health officials reporting 189 deaths so far this year. The country has about 9,500 cases of the disease, which does not usually spread until later in the year. Fumigation has begun around the country to kill the mosquitoes that carry the disease, Dr. Umar Fahmi Achmadi, Director General for Contagious Disease Control and Environmental Sanitation, said. Achmadi has requested additional government funding to pay for the effort. Because of the early outbreak of the disease, Achmadi said tests are being run to determine if this is a new subtype. In 2003, the country had about 50,000 cases of the illness.

Hong Kong reports first-ever dengue fever case

HONG KONG, China Hong Kong has reported its first locally-contracted case of dengue fever, prompting authorities to step up efforts to control the mosquito-borne disease. A 21-year-old construction worker was stricken with dengue fever and hospitalized last week, and is currently in a stable condition, the territory's department of health said over the weekend. It did not give his name. The man was working on a construction site on Ma Wan island, off Hong Kong's New Territories and near Lantau Island, and had not traveled abroad during the disease's incubation period. Preliminary blood tests showed that two of his colleagues had the disease early in the month but had recovered. Other workers have been put under medical surveillance. The government has fumigated Ma Wan and urged any people working at or living near the building site and suffering from symptoms such as high fever, headaches, rashes, cramps and severe back and muscle pains to call hotlines they have set up.

RESUMEN ABSTRACT Dengue is a viral disease with clinical features that vary in intensity according to certain host and viral strain characteristics. Dengue hemorrhagic fever is practically a new clinical entity for the majority of physicians in the Americas and therefore the need to disseminate the most important clinical and epidemiological features that allow for the opportune diagnosis and therapy. The clinical spectrum of the disease includes asymptomatic to severe hemorrhagic forms and each stage has its own signs and symptoms. This clinical description explores the physiological mechanisms involved in the pathogenesis of dengue hemorrhagic fever. This paper gives special emphasis to the alarm signs that foretell the presentation of the shock syndrome requiring emergency treatment and advances a classification scheme for epidemics. This scheme identifies the type of treatment and which patients need observation, laboratory tests and hospital care.
