But when some patients came forward with serious medical problems, ABC Action News began investigating. What senior investigator Mike Mason found will open your eyes like no other investigation he's done. Judy Sanders had Lasik surgery last year at the Lasik Vision Institute on Fowler Avenue in Tampa. "I said, 'What's that smell?' Burning, it smelled like hair burning; it was my eyelashes," she recalled. "It was very painful, extremely painful." She went back to LVI for a second surgery; it didn't help. Judy is a nursing supervisor at a hospital emergency room. She has to keep a close eye on her patients, but now she has a hard time staying focused as her vision has gotten worse. Judy is not alone. Six patients all had their surgeries done on the same day at LVI, and every one of them got a terrible eye infection.

FORMATION OF INITIAL DETECTIVE SQUAD
During the re-organisation of the Police Force in 1862 the activities of the detective section came under review and it was finally approved by the Inspector-General, and his advisory planners, that a Detective Force be officially established. The section became effective from the 1st March, 1862, and comprised one Sub-Inspector, six first class Detectives and six second class Detectives. Personal officially appointed to the branch were notified in the March issue of the Police Gazette of that year. EARLY ACTIVITIES
Prior to 1862 a small number of detectives were engaged in police activities within the Colony and some mention of a detective can be found in historical records as early as 1828. However their early activities are veiled in obscurity, possibly due to the fact that the detectives of that era were mostly 'undercover men' and answerable only to the Governor of the day.

Criminal Investigations Division Contact: Lt. Robert Waggoner 325-676-6611 The Criminal Investigation Division (CID) includes several functions in addition to the traditional detective squads. The Crime Stoppers program for Abilene and Taylor County is attached to CID. The Abilene Taylor County Child Advocacy Center is also attached to CID. Crime Analysis and the Identification Unit operate under the CID umbrella. The more traditional investigation roles are divided into three units: Crime Against Persons, Crime Against Property, and Fraud. The thirty-three employees of CID include one Lieutenant, four Sergeants, nineteen Detectives, one Crime Stoppers Coordinator, two Crime Annalist, two Forensic Specialist, one Child Advocacy Center Director and three Administrative Clerks.
The Persons Unit investigates offenses such as homicide, sexual assault, robbery, assault, harassment, etc. in which a person is injured, endangered, or threatened. Homicides and robberies are the more publicized of these crimes. Fortunately, they do not comprise the majority of the work load. Many of these crimes are fights and family violence that require a great deal of arbitration and interviewing to resolve. They also include physical and sexual abuse of children, and two of the unit's eight detectives are assigned to the Child Advocacy Center. They work exclusively on child abuse cases.

The Office of Criminal Investigation's primary goal is to enforce the laws of the State of Rhode Island relating to hazardous waste, solid waste violations, and water pollution. In addition, the Office investigates cases involving fraudulent boat registrations. The Office has achieved regional and national recognition for the numerous investigations that have been successfully prosecuted in state and federal court.
As part of the investigations undertaken by the Office, investigators conduct interviews, surveillance, and other activities for the purpose of charging alleged violators/polluters. Investigators prepare cases for grand jury and court prosecution. The Office conducts joint criminal investigations in conjunction with the Environmental Protection Agency's Office of Criminal Investigation, RI State Police, State Attorney General's Office, and the U.S. Attorney's Office in Providence. The Office is also responsible for conducting background investigations of individuals and entities applying for permits to operate solid and hazardous waste facilities and of applicants applying for permits to transport hazardous waste in Rhode Island.

CID conducts extensive follow up criminal investigations, and maintains liaison with local, state, and federal authorities. General Investigations
The General Investigations Section has four detectives that are assigned to specific geographic areas and investigate a wide variety of cases. Property crimes account for 80% of the total cases assigned and are typically assigned to these four detectives. Burglary, theft and damage to property are examples of property crimes. Two detectives are assigned to persons crimes, with one specifically monitoring juvenile/gang crimes. Assaults, family violence and terrorist threats are examples of person crimes. One detective is assigned to investigate forgery/fraud cases. This investigator handles bad checks, forged checks, forged prescriptions and any other cases of fraud.

INTRODUCTION (Purpose and Intent)
The fundamental character of the academic enterprise should set the context of administrative decision making. This, the values of the academic environment values that encourage free expression, self direction, independence of thought and action, individual responsibilities, collegiality and the highest standards of ethical behavior should guide the determination of methods of investigation and documentation that will be utilized in the resolution of situation involving non-criminal malfeasance, misfeasance or other misconduct of University employees in the performance of duties.

STATEMENT OF POLICY
As noted in Policy No. 0-006 , the University's Office of Inspector General is responsible for conducting a University wide audit program of independent examinations and making reports and recommendations. The Inspector General may conduct special reviews of situations involving possible non-criminal malfeasance, misfeasance and other misconduct of employees. In situations involving non-criminal malfeasance, misfeasance or other misconduct of University employees, the staff of the Office of Inspector General utilizes review and investigative procedures which may include reviewing University records and interviewing employees and others. Such reviews are conducted in a reasonable, straightforward, collegial and ethical manner consonant with the values of the academic community. Reviews of this nature, however, frequently require that a prudent level of confidentiality be maintained for both the protection of the information under review and the parties involved. Information gained and work performed may be considered confidential, as defined in Section 240.253(1)(b), Florida Statutes.

Bureau staff assist in all aspects of the investigation including preliminary enquiries; case management; Incident Room management; general investigation; file preparation and other ancillary aspects of a criminal investigation. Specialist investigation teams within N.B.C.I. carry out these tasks when requested by local Garda officers or on direction of senior Garda management. The Bureau also proactively engages in intelligence gathering on known criminal suspects.

Our interest is in whether any state or federal crimes were committed." Texas Attorney General Abbott immediately forwarded the congressional request to the investigative division of the Texas Rangers, Garrett said.
