Chronic Fatigue Syndrome Overview
Chronic fatigue syndrome (also called CFS) is a disorder without a known cause, although CFS may be related to a previous infection. CFS is a state of chronic fatigue that exists without other explanation for 6 months or more and is accompanied by cognitive difficulties (problems with short-term memory or concentration). You may have CFS if you meet the following criteria:
  • If you have severe chronic fatigue for 6 months or longer and all other known conditions that could cause fatigue have been excluded by your health care provider
    If you simultaneously have 4 or more of the following symptoms: significant problems with short-term memory or concentration, sore throat, tender lymph nodes, muscle pain, pain in several joints without swelling or redness, headaches that are different in pattern or severity from previous headaches, feeling tired and unrefreshed even after sleeping, and extreme tiredness lasting more than 24 hours after you exercise or exert yourself
Chronic fatigue syndrome affects tens of thousands of people. It occurs more commonly in females than in males. This condition occurs most commonly in young to middle-aged adults. People with CFS are often unable to perform normally at work and home because of their long-term fatigue and problems with short-term memory. This can lead to depression, but depression is not a cause of CFS.

Incoming Presidents Message I am proud to assume the role of president of the AACFS, an organization with a pressing and compelling mission. The AACFS exists promote research, education, and advocacy to further our understanding and eventually develop effective treatments for this disabling illness and related conditions, such as fibromyalgia. There has never been a more important time to assume our role as clinician and scientist advocates in this important mission. Our patients are terribly ill, misunderstood, and suffer at the hands of a poorly informed medical establishment and society. Data presented by Bill Reeves at the AACFS’s conference in Madison in October 2004 underscored the need and just how poorly we have informed our medical community: 85% of the estimated 500,000 – 800,000 CFS patients in the United States remain undiagnosed. The loss of productivity is estimated at 9 billion dollars/annum, representing roughly half of the household income/impacted family ($20,000/year/household). This does not include the cost of health care, loss of tax revenue or other societal costs. Dr Reeves noted that the severity of illness is often as severe as patients with congestive heart failure or chronic obstructive pulmonary disease (COPD). It is in this setting that we find funding for basic and clinical research dropping, with less than half the number of protocols funded in the past four years, from a starting point we all found wholly inadequate. Yet, the key research questions are basic to our understanding of human health, and should easily drive the much needed critical mass of investigators into the field. A Medline search reveals a sum total of nearly 3,000 articles pertaining to CFS in 2004, 1000 more than 1999. That is 1000 more than 1994, and 800 more than 1990, every 5 years we produce roughly 1000 new articles, reflecting a flat line of investigator growth in our field. Clearly, as an organization, we must prioritize working with governmental agencies and foundations to develop the research funding base necessary to speed the pace of new knowledge, and find effective strategies to bring that knowledge to the practicing physician.

I am a Vietnam Era U. S. Air Force Veteran who served from 1962 to 1966 under the maiden name of Larmore. After completing Basic Training at Lackland (January 1963), I was stationed with the 3201st Air Base Wing WAF Squadron at Eglin AFB, Florida. In 1965 I was selected as the Fire Prevention Queen for the base. My last assignment before receiving a medical discharge was in Air Operations at Eglin. Prior to entry into the Air Force, I graduated from Samuel Ready Girls' School, Baltimore, MD. Before that I attended Wicomico County Public Schools including Nanticoke Elementary School from which my mother had graduated. After discharge from the Air Force, I moved to Towson, Maryland and began attending Johns Hopkins University Summer School. There was a slight 9 month interruption along the way. Chronic Fatigue Syndrome (CFS, also known as M.E. or Myalgic Encephalomyelitis) is a serious and debilitating illness that can last for years, and sometimes for life. The latest research suggests that the symptoms of CFS may be caused - at least in part - by a continuing immune response to a real or perceived challenge. Symptoms of CFS include overwhelming exhaustion, both physical and cognitive, memory and concentration impairment, an intense 'flu like feeling, muscle pain, sleep disturbance, headaches, disturbance of balance and other symptoms. A new theory by Prof Colchester published in the Lancet this week has surprisingly been given a major outing by the media.
Although laudably admitting there are obvious flaws in the 'other' theory Prof Colchester argues that the cause of BSE/vCJD lies in the human bones floated down rivers in India as part of Hindu funeral rites. These bones are salvaged from the rivers by poor Indian people who then may sell them on to animal feed companies.
Consider the following : One might expect there to be about 200 victims of sporadic cjd throughout all India in a year, a tiny fraction of the 100s of thousands who would die of all other causes .

The complexities of the Chronic Fatigue Syndrome and the methodologic problems associated with its study indicate the need for a comprehensive, systematic, and integrated approach to the evaluation, classification, and study of persons with this condition and other fatiguing illnesses. We propose a conceptual framework and a set of guidelines that provide such an approach. Our guidelines include recommendations for the clinical evaluation of fatigued persons, a revised case definition of the chronic fatigue syndrome, and a strategy for sub grouping fatigued persons in formal investigations.
