KNIGHT RIDDER NEWSPAPERS When parents talk to their children about the birds and the bees, there's one thing that's rarely mentioned: the human papilloma virus. ABOUT HPV The human papilloma virus is the most common sexually transmitted disease, with about 5.5 million new cases occurring in the United States every year. Here are some other facts about the disease:
  • Nearly three out of four Americans from the ages of 15 to 49 have been infected with HPV. Most have just never known it. The body's immune system often can sufficiently suppress or perhaps even kill the virus on its own.
  • Vaginal, oral and anal sex can all spread the disease, as can skin-to-skin contact in the genital region. Abstinence is the best defense. Condoms offer some protection but, because they do not cover the entire genital area, it is possible to acquire the virus while using them.
  • Most HPV cases are symptomless and often harmless. Of the 70 to 100 strains of HPV thought to be in existence today, just two are responsible for causing genital warts. About a dozen are high-risk strains associated with causing cervical cancer. It is possible to acquire more than one virus strain.

        To Learn More About This Topic: Chat with Us Email Us Scientists believe that some abnormal changes in cells on the cervix are the first step in a series of slow changes that can lead to cancer years later. Pre-cancerous changes of the cervix usually do not cause pain. In fact, they generally do not cause any symptoms and are not detected unless a woman has a pelvic exam and a Pap test. About 15,000 women each year learn they have cervical cancer, or cancer of the cervix (the lower, narrower portion of the uterus.) It is important to know that cancer of the cervix is different from cancer that begins in other parts of the uterus, and requires different treatment. Socioeconomic status is a significant risk factor for cervical cancer, since a lack of access to medical care often eliminates the opportunity for early diagnosis by Pap smear screening. Smoking, a high number of sexual partners, and early age of first intercourse are other risk factors. HIV (Human Immunodeficiency Virus) and HPV (Human Papillomavirus) infections are also associated risks for cervical cancer. Fortunately, pre-cancerous conditions of the cervix can be detected and treated through regular pelvic exams and Pap tests (also called a Pap smear, this simple procedure enables doctors to look at cells from the cervix and identify cancerous or pre-cancerous cells). Regular, professional gynecological examinations remain the most effective way to prevent invasive cervical cancer.

    In the early stages of the disease, there may be no obvious signs or symptoms that will tell a woman she is at risk. The only way to detect cancerous cells is by having a regular smear test every 2-3 years.
    RISK FACTORS
    • Genital infection with the human papilloma virus (HPV), which in some cases causes genital warts, plays a role in the development of cervical cancer.
      HPV is passed on during sexual intercourse with an infected partner.

    Smear tests can spot cancer A vaccine that prevents infections known to cause cervical cancer could be available to women within three years, UK experts believe.
    By guarding against human papilloma virus, it could save thousands of lives and may end the need for smears tests. The vaccine would be given to girls before they are sexually active. Trials have shown a jab can offer 100% protection against strains of HPV linked to about 70% of cervical cancers. Protection Some forms of the HPV virus only cause genital warts, but others cause cervical cancer. It is estimated up to half of the young women in Britain have been infected with a high-risk strain of HPV by the time they are 30. The vaccine may even eventually mean the end of smear testing.
    Dr Anne Szarewski from Cancer Research UK 'High risk' HPV types, strains 16, 18 and 31 and 33, have been found to be present in close to 100% of all cervical cancers, according to the NHS Cancer Screening Programmes. Since cervical cancer is the second most common cause of cancer death in women worldwide, preventing HPV would save many lives.

    Cancer is a group of more than 100 different diseases. They all affect the body's basic unit, the cell. Cancer occurs when cells become abnormal and divide without control. All of the organs of the body are made up of many types of cells. Normally, cells divide to produce more cells only when the body needs them. This orderly process helps keep us healthy. If cells keep dividing when new cells are not needed, a mass of tissue forms. This mass of extra tissue, called a growth or tumor, can be benign or malignant. Benign tumors are not cancerous. They can usually be removed and, in most cases, they do not come back. The most important thing is that cells from benign tumors do not spread to other parts of the body. Benign tumors are not a threat to life. Polyps, cysts, and genital warts are types of benign growths of the cervix.

    Table Of Contents: While a handful of sexually transmitted HPVs can cause a variety of conditions that can lead to dangerous cancers if they remain untreated, it is a gross and dangerous exaggeration to typify HPV as a "dreaded virus" and safer-sex, public health messages advocating condom use as a "conspiracy" ("House Approves...", 2000). This fact sheet will give sexually active women and men the facts they need to understand the real nature of HPVs, what conditions HPVs cause, how those conditions can be effectively managed, and how one may reduce one's own risk of becoming infected.

    HPV infections that cannot be seen with the naked eye may be seen with a variety of clinical tools:

    Background and History of Cervical Screening in Canada
    In 1989, a National Workshop on Screening for Cancer of the Cervix reviewed the 1982 recommendations on screening for cancer of the cervix and recognized that programs in Canada were still not fully effective: not all women at risk were being screened; some physicians had not acquired the necessary skill to take satisfactory smears; some laboratories were too small to provide adequate experience for staff and adequate quality control; and some women with detected cytologic abnormalities were receiving inadequate follow-up and management (Miller, 1991). Conversely, some women were being screened too frequently, resulting in inappropriate use of resources. Recommendations reiterated the need for an organized approach and addressed the following issues: the frequency of screening; the management of abnormalities; information systems; training and quality control requirements for laboratories and programs. The recommendations were accepted by the Conference of Deputy Ministers of Health in November, 1990, who requested that a report of a regular review of developments be made to them on a periodic basis. The Society of Obstetricians and Gynecologists of Canada, the Gynecologic Oncologists of Canada and the Society of Canadian Colposcopists supported the development of formal screening programs. The recommendation with respect to a screening interval of 3 years concerned these groups in the absence of adequate information systems and high-quality laboratory services; also of concern was the recommendation for repeat smears without colposcopy for low-grade squamous intraepithelial neoplasia. These groups suggested that until patient information systems and high-quality laboratory services are in place, annual screening of sexually active women should continue to be the standard of practice
