After Surgery
Post-Operative Intensive Care
After surgery you will recover in the Intensive Care Unit. You will be cared for and monitored very closely and your new heart will be supported with intravenous medications for one or two days. Antibiotics are given during and after the transplant to help you ward off infection. You will spend most of your time in bed, but as you recover and equipment is removed, you will be able to sit in a chair and walk around your bedside. Your family will be allowed to visit, but because of your high risk for infection, visits must be limited.
Step-Down Care
From the intensive care unit, you will move to what we call the step-down unit. You will continue to be closely monitored but will increase your activity. Education and physical therapy, which began in the ICU, are intensified during this phase of your recovery. Your doctor will determine how much you can walk, which helps to prevent pneumonia and to increase blood circulation to your new heart.
Pre-Discharge Care
We continue to watch you closely for signs of rejection, infection or other complications. Physical therapy continues with a goal to have you walking and able to climb stairs before you leave the hospital. Adult heart transplant recipients are usually hospitalized for 10 to 15 days after the operation. If you need a longer period before going home, we will arrange for you to stay for a short time at a rehabilitation facility.

The Division of Cardiology is a world-renowned center for every aspect of cardiovascular care and research. The division's more than 40 full-time faculty have enormous expertise in every aspect of cardiac care, ranging from standard procedures such as outpatient consultation, echocardiography, angiography, electrophysiology testing, percutaneous coronary revascularization and heart transplantation, to advanced clinical research into heart failure, cardiac rehabilitation, preventive cardiology, radio-frequency ablation, laser revascularization and cardiac MRI. The division offers outpatient consultation and evaluation for new patients at its downtown Baltimore campus and at several satellite sites. Outpatient services include a general cardiology clinic and specialized clinics devoted to preventive cardiology, heart failure and cardiomyopathy, rhythm problems and electrophysiology and adult congenital cardiac disease. The division provides expedited transfer for inpatients referred for cardiac evaluation and treatment. In addition, for patients who are eligible and interested, the division offers clinical research protocols evaluating new technologies and treatments for cardiac disease.

Procedure Offers Cure for Irregular Heart Beat A revolutionary catheter technique used in the treatment of the most prevalent type of irregular heartbeat is changing the lives of patients who have undergone the procedure at St. Luke’s-Roosevelt Hospital Center in New York City.
Ablation therapy using radiofrequency energy has successfully cured patients with atrial fibrillation, the most common type of heart rhythm abnormality. "It has enabled people who would have previously been placed on heart drugs for life to be completely off medical therapy after the procedure," said Jonathan Steinberg, MD, an electrophysiologist and Chief of Cardiology at St. Luke’s-Roosevelt. "Atrial fibrillation (AFib) is particularly prevalent among the elderly or those with cardiovascular disease. However, it can also be a troubling problem for younger patients without heart disease."
John McFee, a 36-year-old pharmacist and former New York City police lieutenant, had symptoms that felt like squirrels running around his chest, which started as palpitations about three years ago and gradually got worse, giving him a lot of pain and anxiety. "I was told to stay off alcohol and caffeine, tried to reduce stress and was put on a beta-blocker, but nothing helped," he said. He left the NYPD after suffering hip injury and had to have hip replacement. After the ablation procedure by Dr. Steinberg in April he has been totally off his heart medications and is feeling well. "I feel like a new man!" exclaimed Mr. McFee.

Continuum Heart Institute now offers an Adult Congenital Heart Disease (CHD) Clinic staffed by specialists in adult cardiology with adult CHD training. The clinic also is supported by a team of experts in pediatric cardiology, noninvasive cardiac imaging and cardiac electrophysiology. Although the number of infants born with CHD has remained constant at about four to six per 1000 live births, advances in pediatric cardiology and cardiothoracic surgery over the past three decades have dramatically changed the outcomes and prognoses for these patients. Over 85 percent of these children can now be expected to reach adulthood. As a result, the population with adult congenital heart disease is expected to continue to increase. This means that there is an entire population of adults with corrected anomalies who need follow-up and another population (such as immigrant groups) without awareness of their anomalies. Surgical repair is seldom curative, and more often palliative. It leaves residue for manifestation of infection, arrhythmias, congestive heart failure and exercise intolerance, making care from adult CHD-trained cardiologists essential.

The Heart Station
The Heart Station, on the 2nd floor of St.Louis Children's Hospital is responsible for all inpatient and outpatient non-invasive cardiac testing. These tests are used to diagnose and treat children with congenital and acquired heart disease. The heart station also provides staff and equipment for all the cardiology testing required during satellite clinic visits.

Echocardiography
Transthoracic Echocardiography
High frequency sound waves are used to create a 2 dimensional picture of the heart. The test is performed with a device, called a transducer, placed on the chest. The transducer sends out, and collects the high frequency sound waves. The information is stored in a bedside machine that records the images and displays them on a TV monitor.
Transesophageal Echocardiography
This test is the same as the transthoracic echocardiogram, but instead of being placed on the chest, the transducer is placed in the esophagus (food pipe). This provides clear images of the heart when adequate images are unable to be seen from the front of the chest in the usual manner. This test is always performed with the patient sedated.

What is an Echocardiogram (or Echo)?
An Echocardiogram is an ultrasound of the heart. It is safe and non-invasive. High-frequency sound waves are used to visualize all aspects of the heart chambers, valves and blood vessels entering and leaving the heart. It allows your cardiologist to evaluate the structure and function of the heart, and determine whether any congenital heart defects are present.
Why is an echocardiogram performed?
This test is performed if there is any suspicion that your child has a cardiac defect.
