Criteria for Borderline Personality Disorder
A pervasive pattern of instability of interpersonal relationships, self-image, and affects, and
marked impulsivity beginning by early adulthood and present in a variety of contexts, as indicated
by five (or more) of the following:
1. frantic efforts to avoid real or imagined abandonment. Note: Do not include suicidal or
self-mutilating behavior covered in Criterion 5.
2. a pattern of unstable and intense interpersonal relationships characterized by alternating
between extremes of idealization and devaluation
3. identity disturbance: markedly and persistently unstable self-image or sense of self
4. impulsivity in at least two areas that are potentially self-damaging (e.g., spending, sex,
substance abuse, reckless driving, binge eating). Note: Do not include suicidal or self-mutilating behavior covered in Criterion 5. 5. recurrent suicidal behavior, gestures, or threats, or self-mutilating behavior 6. affective instability due to a marked reactivity of mood (e.g., intense episodic dysphoria, irritability, or anxiety usually lasting a few hours and only rarely more than a few days)

Traits involving emotions:
Quite frequently people with BPD have a very hard time controlling their emotions. They may feel ruled by them. One researcher (Marsha Linehan) said, "People with BPD are like people with third degree burns over 90% of their bodies. Lacking emotional skin, they feel agony at the slightest touch or movement." 1. Shifts in mood lasting only a few hours. 2. Anger that is inappropriate, intense or uncontrollable.
Traits involving behavior:
3. Self-destructive acts, such as self-mutilation or suicidal threats and gestures that happen more than once 4. Two potentially self-damaging impulsive behaviors. These could include alcohol and other drug abuse, compulsive spending, gambling, eating disorders, shoplifting, reckless driving, compulsive sexual behavior.

Generic Criteria for Personality Disorders
  • A lasting pattern of behavior and inner experience that markedly deviates from norms of the patient's culture. The pattern is manifested in at least 2 of these areas:
    -Affect (appropriateness, intensity, lability and range of emotions)
    -Cognition (how the patient perceives and interprets self, others and events)
    -Impulse control
    -Interpersonal functioning
  • This pattern is fixed and affects many personal and social situations.
  • These symptoms cause clinically important distress or impair work, social or personal functioning.
  • This stable pattern has lasted a long time, with roots in adolescence or young adulthood.
  • The symptoms aren't better explained by another mental disorder.
  • They aren't directly caused by a general medical condition or substance use, including medications and drugs of abuse.
Coding Notes
If the personality disorder is the only diagnosis or the main reason the patient has come for evaluation, "(Principal Diagnosis)" should be appended to the Axis II diagnosis. For example: Axis I V71.09 No diagnosis

Overview It's one of several types of personality disorders, all of which reflect an inability to accept the demands and the limitations of the outside world. These disorders may regularly interfere with your behavior and your interactions with family, friends or co-workers. Among the other personality disorders are paranoid personality disorder, antisocial personality disorder, histrionic personality disorder and narcissistic personality disorder. People with BPD have an enormous need for love and a terrible fear of closeness. They have disturbed thinking and are constantly in a state of emotional turmoil. They're calm and rational at times, but they may explode into inappropriate anger or rage at some perceived rejection or criticism.
