    The Borderline Personality Disorder (BPD) Essential Feature The essential feature of the Borderline Personality Disorder is a pervasive pattern of unstable interpersonal relationships, self-image, and affect. There is marked impulsivity that begins by early adulthood (DSM-IV™, 1994, p. 650). The ICD-10 refers to BPD as the Emotionally Unstable Personality Disorder which is characterized by impulsivity, unpredictable moods, outbursts of emotion, behavioral explosions, quarrelsome behavior, and conflicts with others. The ICD-10 divides this category into two types: the impulsive type (characterized by emotional instability and lack of impulse control) and the borderline type (characterized by disturbances of self-image, aims, and internal preferences; chronic feelings of emptiness; intense and unstable interpersonal relationships; and self-destructive behavior) (ICD-10, 1994, p. 228). Benjamin notes that individuals with BPD often suffer from thought disorders, affective disorders, dissociative disorders, alcohol and substance abuse, eating disorders, and anxiety disorders. She suggests that with BPD, everything that can go wrong has gone wrong. BPD involves disorder in every domain of function: cognition, mood, and behavior (Benjamin, 1993, p. 113).

    The term "borderline" goes back a long way. For centuries, European society excluded people regarded as "insane" from normal life, confining them to asylums or driving them from one town to another. By the 18th century, a few doctors were beginning to study the people in asylums, and discovered that some of these patients had, by no means, lost the powers of reason: they had a normal grasp of what was real and what wasn't, but they suffered terribly from emotional anguish through their impulsiveness, ragefulness, and a general difficulty in self-government caused others to suffer. They seemed to live in a borderland between outright insanity and normal behavior and feeling.

    What is borderline personality disorder?
    BPD is one of many personality disorders listed in the manuals used by clinicians when they are giving someone a psychiatric diagnosis. The word 'personality' refers to the ongoing pattern of thoughts, feelings and behaviour that makes us who we are. A personality disorder may be diagnosed when it's felt that several areas of someone's personality are causing them or others problems in everyday life. This diagnosis is very controversial, because it implies that someone's whole personality is flawed - rather than just one aspect of them. Some psychiatrists argue that it's impossible to treat someone's personality and that it's wrong to apply medical terms and treatments to a personality.     The core disorder in BPD is emotional dysregulation. Emotional dysregulation is viewed as a joint outcome of biological disposition, environmental context, and the transaction between the two during development. The theory asserts that borderline individuals have difficulties in regulating several, if not all, emotions. This systemic dysregulation is produced by emotional vulnerability and by maladaptive and inadequate emotion modulation strategies.     The symptoms of borderline patients are similar to those for which most people seek psychiatric help: depression, mood swings, the use and abuse of drugs and alcohol as a means of trying to feel better; obsessions, phobias, feelings of emptiness and loneliness, inability to tolerate being alone, problems about eating.
    The one word that best characterizes borderline personality is "instability." Their emotions are unstable, fluctuating wildly for no discernible reason. Their thinking is unstablerational and clear at times, quite psychotic at other times. Their behavior is unstable, often with periods of excellent conduct, high efficiency and trustworthiness alternating with outbreaks of babyishness, suddenly quitting a job, withdrawing into isolation, failing.
