Reactive Attachment Disorder ... I had heard of the terms before, but until I agreed to take my 12-year-old nephew in, “reactive attachment disorder”, sometimes known as “detachment disorder”, or simply “attachment disorder”, held no personal significance to me. My wife and I took our nephew in to live with us in January of 2002. While we knew, and anticipated that there would be significant problems, we had no idea as to the extent of his disability. Our nephew was diagnosed with Reactive Attachment Disorder, or RAD, shortly after coming to live with us and, despite the fact that this is a disorder that was barely on my screen of consciousness prior to that time, my wife and I have both come to understand it quite well. Making it go away was another matter entirely. As a paramedic for more than twenty years, I had come to appreciate signs and symptoms pointing to specific diagnoses, yet I was astounded to see how closely and how clearly our nephew fit the pattern for reactive attachment disorder. I only wish that treating the problem were as easy as diagnosing it appeared to be Through books, tapes, classes, and conferences, my wife and I have devoured all of the useful information we can find on attachment disorders and, as we have learned more, we’ve added to this site. If you’ve just begun your own search for ways in which you can help your child, we understand what you are looking for, and believe that this will be a good place to start. We’ve also created a support board on Delphi Forums, and you can find a link to it in our Books and Resources section, along with other support forums on the subject.

Greetings Welcome to the Attachment Disorder Site, home of the Attachment Disorder Email List. Here you will find a wide variety of information to help you understand attachment issues. Attachment Disorders range in severity. In fact, the attachment continuum runs from securely attached through degrees of attachment issues all the way to those who suffer from severe attachment disorder as in my son's case. With the addition of hundreds of children adopted from institutional settings and from the foster care system, it is important for all of us to become educated regarding the potential and unique problems these children and their families face. It is by this understanding that we can forge ahead and learn to help our families heal. We must also remember that all adopted and foster care kids do not suffer from attachment disorder. A lot has to do with the individuals themselves - their physiological and psychological make-up, the type of environment they were in and the duration they were in that environment. With this in mind, we do not want people to become wary of adoption because of attachment disorder. We want people to become educated about it. I often wish I had known what was wrong sooner as I look back on years that were spent utilizing ineffective therapies and parenting strategies. We would have started on the road to healing sooner.

In their book, Love and Attachment: Contemporary Issues and Treatment. Considerations, Tosone and Aiello have compiled an excellent array of articles touching on the complicated subjects of love and attachment. Each article highlights different challenges and issues faced by clinicians in everyday practice. Tosone's Illusion, Disillusion, and Reality in Romantic Love examines the nature of romantic love, offers an excellent literature review, and moves to developmental and diagnostic concerns dealing with such considerations as transitory love, addictive love, and the pathways leading to mature object love. Tosone's well-written piece comprises a fascinating coalition of psychoanalytic opinions on the subject of love, stressing the transformative nature of love relationships in mastering past traumas, and by her sensitive case examples, establishes the therapeutic or analytic situation as the center from which past traumas can be reworked and repaired. Clinical Practice with Intercultural Couples by Soncini reminds therapists to be alert not only to their countertransference but to be sensitive to the history and cultural traditions of their patients. Case examples illustrate the extreme conflict caused when more constrictive foreign customs confront the focus on individuality and libera1ity in the United States. Soncini examines the special issues of intercultural marriage, i.e., gender issues, raising children, and other typical areas of conflict, enhancing ones clinical knowledge of the subject.

Introduction
Respite, temporary relief for caregivers and families, is an important support service to parents who have adopted a child. It is particularly important for families who adopt a child with medical, developmental, or psychological disabilities, or one who has experienced abuse or neglect. This factsheet provides strategies for addressing the needs of adoptive families using respite services, and their children, as well as the providers serving those families. People who choose to adopt a child with special needs and make that child a permanent part of their family should be offered a range of support services to help them care for their child. The availability of these services should be known to the family at the start. An adoptive family that may have used neighbors or relatives for child care in the past may now discover that their new family member requires a higher level of care than a extended family member or neighborhood babysitter feels comfortable giving. In other cases, extended family members may not accept the adoption or may not live close enough to offer assistance. Respite can provide regular caregiving breaks for these families.
Important Issues in Adoption
It is critical that individuals working with children who have been adopted and their families be aware of the unique issues relating to adoption. The National Resource Center on Special Needs Adoption has identified eight core issues that affect all adoptive families to some degree.

The face of adoption has changed dramatically since the 1950s. Long gone is the time when there were more healthy infants than homes available. Nowadays, adopting children with special needs once considered "hard to place" or "unadoptable" has become the norm rather than the exception. Many of our adopted children come to us with some kind of special need. In fact, whether you've adopted domestically or internationally, your family stands a greater chance of needing outside help than families whose kids are home-grown. In domestic adoption, if you adopt a child from a public agency you also get that child's history ... neglect, abuse, shunting from one foster home to another.     What are the subtle signs? 1. Sensitivity to rejection and to disruptions in the normally attuned connection between mother and child. 3. Difficulty discussing angry feelings or hurt feelings. 4. Over valuing looks, appearances, and clothes. 5. Sleep disturbances. Not wanting to sleep alone. 6. Precocious independence. A level of independence that is more frequently seen in slightly older children. 7. Reticence and anxiety about changes. 8. Picking a scabs and sores. There are effective ways for a parent to help his or her child. Parents and the right parenting are vital to preventing subtle signs from becoming anything more than sensitivities. Parenting consistently with clear and firm limits is essential. Discipline should be enforced with an attitude of sensitive and responsive empathy, acceptance, curiosity, love, and playfulness. This provides the most healing and protective way to correct a child. These sensitivities are not a mental illness or Reactive Attachment Disorder. They are subtle signs of attachment sensitivities. So, what can you do?

But when it comes to the health and medical care of an adopted child, there are some unique issues and concerns that you may want to keep in mind before, during, and after the adoption is complete. Gathering Information Before You Adopt
If you have an open or semi-open adoption - one in which you meet the mother and sometimes the father - you should be able to get substantial health information. In an open adoption, you may help arrange the birth mother's prenatal care , go with her to doctor visits, and be present for the birth. You can also request health records through the agency or attorney who is arranging the adoption. With an older child who is already living in the United States, you may be able to get a sense of the child's general health by spending time with him or her before the adoption takes place or by serving as a foster parent beforehand. Before you adopt, it's a good idea to have as much medical information as possible, including:

It involves the assessment of a variety of factors to inform agency planning and the construction of an effective recruitment strategy. Preliminary planning of external and internal factors must be addressed in order to reach interested families and actually place children with foster and adoptive parents. External Factors The agency must be aware of its reputation in the communities that it serves and from which parents will be recruited. If the perception of the agency is a negative one, people in the community will not respond to recruitment efforts. Regardless of agency auspice, the agency is a part of the child welfare system, and might not have a good reputation in many communities. The television and press have often presented a one-sided view of foster care and child welfare only reporting failures and not successes. Media coverage about a child who was abused because the system did not respond as expected, or presenting child welfare workers as only removing children from their families does not lead to positive perceptions. Consequently, perceptions will have to be addressed in designing recruitment programs. Strategies involving public relations and building community relationships will be described later in this paper.
