Proper diet and nutrition are key elements in the prevention of all types of disease, including arthritis. An important first step in treating arthritis lies in achieving normal body weight. Excess weight puts increased stress on weight-bearing joints affected with arthritis. Eat a diet that is high in fiber and that is not tainted with chemicals, which stress the body. Maximize your intake of fresh vegetables, fruits, fish, nuts, seeds, and whole grains. Make sure your menu includes cold-water fish such as salmon and halibut. Avoid saturated fats, hydrogenated and partially hydrogenated oils, rich fattv foods, fried foods, and refined sugar. These substances make the internal environment more acidic. Acid in the joints promotes inflammation, which worsens symptoms and increases pain. Fatty meats, eggs, margarine, shortening, caffeine, alcohol, tobacco, sugars and dairy products should be dramatically cut down or eliminated from the diet. About one-third of those who suffer from rheumatoid arthritis are sensitive to solanines, or nightshade plants, which include potatoes, peppers, eggplant, tomatoes, and tobacco. These should be eliminated from the diet.

It may surprise you to know that arthritis is the most common chronic condition and the leading cause of disability. Arthritis affects some 40 million Americans, or 15 percent of the U.S. population. Half of all older than 65 report some type of arthritis. Although there are many types of arthritis, by far the most common, the one that occurs as we age, is osteoarthritis (OA), formerly known as degenerative arthritis or "wear-and-tear" arthritis. OA occurs when the cartilage, the cushion or shock absorber between the bones in a joint, frays and breaks down. As the cartilage is lost, OA patients often experience pain, stiffness and limitation of motion in the joint. Ultimately, OA most commonly occurs in the knees, hands, hips, spine and feet. It frequently impairs the ability to walk or get in and out of bed, chairs, and cars, and limits mobility and usual activities. What causes OA?

The Arthritis Foundation says X-rays can find signs of osteoarthritis in most people over 60. (1) This is believed to be primarily due to abnormal nutrient absorption and metabolism rather than to inadequate intake, as their nutritional status correlates better with disease severity than with dietary intake. (2) However, food habits of patients are often poor, and total energy intake is often too low. (3) Malnutrition is also common among children with juvenile rheumatoid arthritis. Chronic inflammation can cause anorexia, reduced mobility can make eating and preparing foods difficult, drugs to reduce inflammation can cause nausea, and abdominal pain can reduce absorption. (4) Even if their calorie and protein intakes are high, these children may still have a poor nutritional status. (5)

Rheumatoid arthritis (RA) is a disease in which the body's immune system attacks cartilage in the joints. It affects about 2 percent of the population, but more women suffer from it than men. The so-called Mediterranean dietrich in olive oil, fish, poultry, fruits and vegetablescan relieve the symptoms of rheumatoid arthritis. Although this diet is usually recommended to cut the risk of heart disease, a study reported in the February 19, 2003 edition of the Annals of the Rheumatic Diseases found that it also reduced pain and improved physical function in rheumatoid arthritis sufferers. After three months on the diet, arthritis patients lost an average of 6.6 pounds, lowered their cholesterol levels and had significantly less pain in their joints than the control patients who did not change their eating habits.

Dr. David Fisher, MD, an orthopaedic surgeon specializing in hip and knee replacement, says that there is no significant, proven correlation between specific foods and reducing arthritis symptoms except in terms of maintaining ideal bodyweight. Dr. Fisher is a member of Orthopedics Indianapolis, an orthopaedic group practice in Indiana. A healthy diet is important for everyone, of course. Arthritis patients, however, sometimes look beyond this notion of healthy eating and look to nutrition for a cure. The link between nutrition and arthritis is complex. There is limited evidence that diet can influence some forms of arthritis, but to fully understand this, the type of arthritis and the kind of diet must be considered. A Look at Some of the Theories behind Diets for Arthritis According to the American College of Rheumatology (ACR), arthritis patients spend nearly $1 billion annually on unproven remedies. Prominent among these unproven, questionable or controversial remedies are diets for arthritis. It is easy for arthritis patients to believe that there is a link between diet and arthritis. If this were true, changing their diet would enable patients to get better control over their disease. A small Swedish study in the March issue of the Annals of the Rheumatic Diseases has found that people who followed this eating regimen had less inflammation and were more active at the end of three months. Outside experts, however, say this study is far from the final word on the subject. "The design is flawed and the results are, at best, modest," says Dr. Clifton Bingham, director of the Seligman Center for Advanced Therapeutics at the Hospital For Joint Diseases in New York City. "The results they find are statistically significant, but it's not clear that any of the results are clinically significant." Until now what has been lacking is a good randomised trial of the effects of weight reduction on patients with knee arthritis. Now we have one [1], we can be more confident that weight reduction is a good thing for people with painful knees.
    Randomised trial
    The setting was Danish patients who were overweight adults with primary knee osteoarthritis according to standard diagnostic guidelines, with radiological evidence of damage. Overweight was defined as a BMI of over 28 kg/m , and for participation they had to express a clear unequivocal motivation for weight loss and no major health problems.
