Apnea of prematurity commonly occurs in newborns who are born before 34 weeks of pregnancy, increasing in frequency and severity among the most prematurely born. In these newborns, the part of the brain that controls breathing (respiratory center) has not matured fully. As a result, the newborns may have repeated episodes of normal breathing alternating with brief pauses in breathing. In very tiny premature newborns, apnea can be caused by temporary obstruction of the pharynx due to low muscle tone or a bending forward of the neck; this is called obstructive apnea. Over time, as the respiratory center matures, episodes of apnea become less frequent, and by the time the newborn approaches term, they no longer occur.

    Definition of Apnea
    Apnea is the most common problem of ventilatory control in the premature infant frequently prolonging hospitalization and the need for cardiopulmonary monitoring. The standard definition of apnea is cessation of inspiratory gas flow for 20 seconds, or for a shorter period of time if accompanied by bradycardia (heart rate less than 100 beats per minute), cyanosis, or pallor.     Apnea has been classified into three types depending on whether inspiratory muscle activity is present. If inspiratory muscle activity fails following an exhalation, it is termed Central Apnea .     Chronic Obstructive Airways Disease
    There can be large falls in arterial oxygen saturation during sleep in patients with chronic obstructive pulmonary disease. In general, these falls can be predicted from daytime oxygen levels and are not due to classical OSA syndrome, but to a number of factors which combine to produce worsening hypoxaemia during sleep. These patients are often at the limits of their respiratory capacity, and the loss of the intercostal and accessory muscle contribution to breathing, and the increase in upper airways resistance associated with REM sleep, cause marked falls in ventilation. In addition, patients may be hypoxic while awake and small falls in ventilation and PaO2 lead to large falls in arterial oxygen saturation during sleep. The hypoxic episodes demonstrated by overnight oximetry in a patient with chronic obstructive pulmonary disease differ in character from those seen in patients with OSA syndrome (figure 10). There is some evidence that increases in upper airway narrowing during sleep, short of apnoeas and even snoring, may contribute to the development of CO2 retention in these patients. However classical sleep apnoea is not significantly more common in unselected COPD patients with raised CO2 levels, and excessive nocturnal hypoxaemia is not one of the predictors of long- term survival in patients with COPD.

    WASHINGTON - People who die in their sleep may stop breathing because they have lost too many brain cells, U.S. researchers reported Monday. Sleep apnea - a condition in which people stop breathing for long stretches of time in their sleep - may sometimes be caused by the destruction of cells in the brain stem, where autonomic functions such as breathing are controlled, they said. Tests on rats showed that the loss of key brain stem cells that die off with age caused such disrupted sleep that the animals eventually stopped breathing completely. The same thing may be happening in elderly people, said neurobiologist Jack Feldman of the University of California Los Angeles. "We wanted to reveal the mechanism behind central sleep apnea, which most commonly affects people after age 65," Feldman said in a statement. "Unlike obstructive sleep apnea, in which a person stops breathing when their airway collapses, central sleep apnea is triggered by something going awry in the brain's breathing center."

While the study involved only 55 patients, they showed an 82 percent success rate compared to the historical level of less than 50 percent found in earlier, more invasive surgeries, said the report from Chang Gung Memorial Hospital, Taipei. Obstructive sleep apnea causes snoring and interruptions in breathing during sleep. Surgical correction involves removal of some of the tissue in the throat to widen the airway. The modified technique used in the study in the new issue of the Archives of Otolaryngology is called extended uvulopalatal flap surgery. It removes fatty tissues, soft glands and the tonsils to increase airway space but spares muscle tissue.

    WHAT IS SLEEP APNEA?
    General Description of Sleep Apnea and Snoring
    Sleep apnea is a disorder in which a person stops breathing during the night, perhaps hundreds of times, usually for periods of 10 seconds or longer and sometimes for as long as a minute. In most cases the person is unaware of it, although sometimes they awaken and gasp for breath. It is usually accompanied by snoring. People who have sleep apnea may not even be aware of the condition, but it inevitably causes daytime sleepiness.
    Sleep apnea is generally categorized as obstructive, central, or mixed. A less severe form of obstructed breathing called upper airway resistance syndrome (UARS) is also worth mentioning.
    Obstructive Sleep Apnea
    Obstructive sleep apnea (OSA), the most common form of apnea, occurs when tissues in the upper throat (or airway) collapse at intervals during sleep, thereby blocking the passage of air. In general, OSA occurs as follows:
    • On its way to the lungs, air passes through the nose, mouth, and throat (known as the upper airway).

    What is obstructive sleep apnea?
    Obstructive sleep apnea syndrome (OSAS) is a debilitating sleep and breathing disorder defined as the cessation of breathing for 10 seconds or more (an apnea) at least five times per hour of sleep. During sleep, the body's muscles relax, which can cause excess tissue to collapse into the upper airway (back of the mouth, nose and throat) and block breathing. When breathing is interrupted by an obstruction in the airway, the body reacts by waking enough to start breathing again. These arousals may occur hundreds of times each night but do not fully awaken the patient, who remains unaware of the loud snoring, choking and gasping for air that are typically associated with obstructive sleep apnea.
