Sleep apnea is a disorder that commonly affects more than 12 million people in the United States. It takes its name from the Greek word apnea, which means "without breath." People with sleep apnea literally stop breathing repeatedly during their sleep, often for a minute or longer and as many as hundreds of times during a single night. Sleep apnea can be caused by either complete obstruction of the airway (obstructive apnea) or partial obstruction (obstructive hypopnea hypopnea is slow, shallow breathing), both of which can wake one up. There are three types of sleep apnea — obstructive, central, and mixed. Of these, obstructive sleep apnea (OSA) is the most common. OSA occurs in approximately 2 percent of women and 4 percent of men over the age of 35. Causes The exact cause of OSA remains unclear. The site of obstruction in most patients is the soft palate, extending to the region at the base of the tongue. There are no rigid structures, such as cartilage or bone, in this area to hold the airway open. During the day, muscles in the region keep the passage wide open. But as a person with OSA falls asleep, these muscles relax to a point where the airway collapses and becomes obstructed. When the airway closes, breathing stops, and the sleeper awakens to open the airway. The arousal from sleep usually lasts only a few seconds, but brief arousals disrupt continuous sleep and prevent the person from reaching the deep stages of slumber, such as rapid eye movement (REM) sleep, which the body needs in order to rest and replenish its strength. Once normal breathing is restored, the person falls asleep only to repeat the cycle throughout the night.

    O bstructive sleep apnea is a disorder in which complete or partial obstruction of the airway during sleep causes loud snoring, oxyhemoglobin desaturations and frequent arousals. As a result, affected persons have unrestful sleep and excessive daytime sleepiness. The disorder is associated with hypertension, impotence and emotional problems. Because obstructive sleep apnea often occurs in obese persons with comorbid conditions, its individual contribution to health problems is difficult to discern. The disorder has, however, been linked to angina, nocturnal cardiac arrhythmias, myocardial infarction, stroke and motor vehicle crashes. Even though obstructive sleep apnea is fairly common, it often remains undiagnosed in primary care practice. Because the disorder is associated with significant morbidity and even some mortality, family physicians need to be familiar with its clinical presentation and treatment.

    S leep apnea is a cessation of breathing during sleep that is caused by repetitive partial or complete obstruction of the airway by pharyngeal structures. Patients with obstructive sleep apnea often are overweight, snore loudly, and complain of daytime fatigue and sleepiness. A sleep laboratory may be used to confirm the diagnosis. Family physicians should be able to diagnose and manage this relatively common disorder. Deciding When to Treat The risk of long-term mortality increases when patients have 20 or more respiratory events per hour during sleep testing.

    Sleep apnea occurs when breathing stops during sleep for at least 10 seconds at least five times an hour. Mild sleep apnea causes few symptoms, but the condition may lead to low oxygen levels, which can be life threatening. Sleep apnea is more common in people who smoke, drink alcohol or are overweight. It may also occur in people at high altitudes. There are two types of sleep apnea: "obstructive sleep apnea" and "central sleep apnea." Obstructive sleep apnea is due to a blockage in the airways. It occurs when air passage in the upper respiratory tract becomes obstructed during sleep (obstruction is caused by soft tissue of the pharynx relaxing and blocking the flow of air). It prevents breathing until low levels of oxygen in the blood cause a person to respond by waking up and taking a deep, snorting breath. Being overweight or having a small tongue or mouth can contribute to the obstruction. In children enlarged tonsils or adenoids are the most common cause of obstruction
