Since erosions of hyaline cartilage are frequently found in RA patients, especially in the small joints of the hands and feet, this could explain the localization of the pathological lesions in this disease to these sites. These results are consistent with the hypothesis that RA is an autoimmune reactive arthritis following infection of the upper urinary tract by Proteus mirabilis microbes.

What is juvenile ankylosing spondylitis?
Juvenile ankylosing spondylitis (JAS) is a type of arthritis that affects the spine and the sites where the muscles, tendons, and ligaments are attached to bone. "Ankylosing" means stiff or rigid, "spondyl" means spine, and "itis" refers to inflammation. The disease causes inflammation of the spine and large joints, resulting in stiffness and pain. The disease may result in erosion at the joint between the spine and the hip bone (the sacroiliac joint), and the formation of bony bridges between vertebrae in the spine, fusing those bones. In addition, bones in the chest may fuse. JAS is considered to be a multifactorial condition. Multifactorial inheritance means that "many factors" are involved in causing a health problem. The factors are usually both genetic and environmental, where a combination of genes from both parents, in addition to unknown environmental factors, produce the trait or condition. Often one gender (either males or females) is affected more frequently than the other in multifactorial traits. Multifactorial traits do recur in families because they are partly caused by genes. A group of genes on chromosome 6 code for HLA antigens play a major role in susceptibility and resistance to disease. Specific HLA antigens influence the development of many common disorders. Some of these disorders, such as JAS, are autoimmune related and inherited in a multifactorial manner. When a child has a specific HLA antigen type associated with the disease, he/she is thought to have an increased chance to develop the disorder. The HLA antigen associated with JAS is called B27. Children with the B27 HLA antigen are thought to have an increased chance (or "genetic susceptibility") to develop JAS; however, it is important to understand that a child without this antigen may also develop JAS. This means HLA antigen testing is not diagnostic or accurate for prediction of the condition. Males are affected with JAS three times more often than females.

DEFINITION: Ankylosing spondylitis is an autoimmune disease that causes inflamation of the spine and sacroiliac joints and can cause inflammation of the eyes, lungs, and heart valves. It varies from intermittent episodes of back pain that occur throughout life to a severe chronic disease that attacks the spine, peripheral joints and other body organs, resulting in severe joint and back stiffness, loss of motion and deformity as life progresses. A dramatic loss of flexibility in the lumbar spine is an early sign of AS. Although most symptoms begin in the lumbar and sacroiliac areas, they may involve the neck and upper back as well. Arthritis may also occur in the shoulder, hips and feet. As with many of the autoimmune diseases, many body systems can be effected in extreme cases.
AT RISK: Ankylosing spondylitis typically starts as an insidious onset of back pain in the late teens or early twenties, and seems to effect men more than women. It is a genetically determined disease, susceptability to which is related to the presence of the HLA-B27 antigen in the patients' cells.

I was diagnosed with Hep C about 2 years ago, and genotyped 1b. Because I have juvenile rheumatoid arthritis and ankylosing spondylitis my doctor recommended I do not have Interferon treatment. Are there any other options at this point or do I just wait until another treatment becomes available? Are there any other drugs in clinical studies at this time? My ALT and AST's are in the 70's. HFHSM.D.-D.M.

I appreciate your question and I also imagine you are a little frustrated. I understand that you have hepatitis C and you are not an ideal candidate for interferon based therapies because of your juvenile rheumatoid arthritis and ankylosing spondylitis.
The first question, I have for you is: do you know what your liver biopsy shows. If you have a benign liver biopsy with little scarring or fibrosis, you might not want to do anything right now. Hepatitis C typically causes liver disease over decades and doesn’t even cause cirrhosis in half of infected individuals. Typically women who do not drink alcohol are likely to have relatively slow progressing disease.
One drug that is being used in clinical trials is a drug called interleukin-10 (IL-10). This is a drug that does not get rid of the virus but has anti-inflammatory activity that some liver specialist feel can slow the level of fibrosis in individuals with hepatitis C. Because it is anti-inflammatory or even immune-suppressing it might be safe in individuals with juvenile rheumatoid arthritis and ankylosing spondylitis. However, I think most of the studies looking at IL-10 in hepatitis C require a liver biopsy.

The safety and efficacy of a Boswellia extract was compared against mesalazine for the treatment of 102 patients with active Crohn's disease in an eight-week randomized, double-blind study. (2) The primary clinical outcome measured was the change in Crohn's Disease Activity Index (CDAI). After therapy with Boswellia extract (3.6 g per day) the average CDAI was reduced by 90, compared to a reduction of 53 for the mesalazine group (4.5 g per day). The authors concluded that the Boswellia extract was as effective as mesalazine, which is the state-of-the-art treatment for this disorder. Considering the observed fewer side effects and better safety profile of Boswellia, they suggested that this novel herbal treatment appears to be superior to mesalazine in terms of a risk-benefit evaluation.

(Ankle-oo-sing Spondie-lie-tiss) When most people hear the word Arthritis they think of elderly folks - Grandma and Grandpa. Although many adults and older people will develop arthritis, it is not very common in children. Juvenile Arthritis (JA) is a term used to categorize the different types of arthritis affecting children. The most common type of JA is Juvenile Rheumatoid Arthritis (JRA). The type of JRA that affects the spine is called Ankylosing Spondylitis (AS). JRA is diagnosed in children less than 16 years of age whose arthritis has persisted for longer than six weeks. What causes JRA or AS?
The exact cause or causes are not understood. Sometimes it is triggered by a bacterial infection or an autoimmune problem. An autoimmune disorder can cause the body to rebel against itself. How does the disease start?

Ankylosing spondylitis (AS) is an inflammatory arthritis that affects the spine. Usually the sacroiliac joints (joints joining the pelvis to the sacrum of the spine) are involved first with gradual involvement of the rest of the spine following. AS falls into the category of diseases designated as the “spondyloarthropathies.” Other members of this group include psoriatic arthritis and reactive arthritis. The term spondyloarthropathy refers to the propensity for spinal involvement. There is a strong genetic predilection with 90 per cent of Caucasian patients showing the presence of the HLA Br27 genetic marker. Many AS patients will have a positive family history for the disease. Spondyloarthropathies, in general, cause early erosion and damage. As the disease progresses, there is gradual destruction of joints with bony bridging leading to marked deformity and loss of function. Like the other spondyloarthropathies, AS is an enthesopathy causing inflammation at the site where the tendon inserts into bone. AS occurs in about 0.5 per cent of the population. It is more common in men than women in a ratio of about 5:1. It should be mentioned that AS is probably under diagnosed in women. When AS does affect women, it tends to happen a bit later, have less hip involvement, have more symphysis pubis involvement, and definitely more cervical spine (neck) involvement.

Travelling with ankylosing spondylitis
I've always enjoyed travelling, but since I came down with ankylosing spondylitis it's gotten a lot harder for me. A recent trip to Winnipeg brought this home for me: I now expect elevated pain every time I travel, and this trip was, as I had predicted, no exception. But at least I'm at the point where I can analyse it a little bit.
From what I've observed, pain and inflammation due to AS are higher when I travel for three reasons:
1. Stress. Stress exacerbates autoimmune disease (about which see Gabor Mat©'s When the Body Says No ). Travel is stressful. And travel is especially stressful for me: I plan out itineraries to each pee break, and rehearse every possible (and several impossible) worst-case scenarios in my head before I get under way. It's so bad that even on weekend excursions I have to leave the packing to my significant other I'm just too wound up to be useful. Once on the road (or at the airport or the train station), I'm fine in fact I'm more than fine: I'm an excellent long-distance driver, and my only problem with air turbulence is nausea, not terror. But getting to that point is sometimes only just short of traumatic. The end result is usually that my joints are screaming by the following morning.
