Pediatric Nutrition Data
The following information was obtained from the Pediatric Nutrition Surveillance System (PedNSS), 1997, DHHS, CDC.
In the 1997 PedNSS study, approximately 8 million records were collected from 44 states, the District of Columbia, and 5 tribal governments to monitor the nutritional status of low-income children in federally funded maternal and child health and nutrition programs. The following information presents 1997 data and highlight trends from 1989 to 1997. Most of the total PedNSS records (92%) are from children younger than 5 years, and 32% are from infants aged less than 1 year old. Approximately 1% of the records is from American Indian or Alaska Native children who will be highlighted in this review.
Indicators of Nutritional Status
Underweight
The low prevalence of underweight among children in the PedNSS indicates that acute malnutrition is not a public health problem in the American Indian and Alaska Native children.
Anemia
Iron deficiency is the most common nutritional deficiency in the world. Anemia is an indicator of iron deficiency, which is associated with developmental delays and behavioral disturbances. In the 1997 PEDNSS, 18.4% of children under 2 years and 16.9% of children aged 2 to 5 years had anemia. American Indian or Alaska Native children have a rate of anemia of 18.6%. Anemia was highest (25% or higher) in Alabama, Arizona, the Chickasaw Nation in Oklahoma, DC, Maryland, and Mississippi.
Overweight
The increasing trends of overweight children in the general US population is consistent with the findings from the PedNss. Overweight American Indian/Alaska Native children aged 2 to 5 years has risen from 10 - 12% from 1989 to 1997. This is second only to Hispanic children who prevalence has been the highest for 8 out of the 9 years studied.

Senior Extension Nutrition Program, Aging and Adult Services
Coeur d'Alene, Idaho Aging and Adult Services (AAS) of North Idaho works with University of Idaho Extension in Kootenai County to provide nutrition, money management, and food safety education tailored to suit the unique needs of seniors living in their homes. This program is called the Senior Extension Nutrition Program (SENP). Nutrition Advisors, trained by University of Idaho Extension, travel to AAS clients' homes to provide educational lessons in the importance of a healthy diet for long-term health; planning meals using the Food Guide Pyramid; how to prepare quick, low-cost and nutritious meals; the health benefits of eating fruits, vegetables, and whole-grain foods; medications and health conditions that affect appetite; implications of sudden weight gain or loss; how to make eating alone a pleasant experience; the importance of washing hands, kitchen surfaces, and kitchen utensils; and managing money to make it last throughout the month.

(Selected Programs) Purchaser: Federal Government Program: Health Resources and Services Administration, Centers for Disease Control and Indian Health Services grants Description: The Public Health Service division of the U.S. Department of Health and Human Services administers health care spending programs through the Health Resources and Services Administration (HRSA), Centers for Disease Control (CDC) and Indian Health Services (IHS) grants. HRSA programs provide health services to groups with special needs, such as mothers and infants, disadvantaged minorities, HIV-infected individuals, vulnerable and special need populations. CDC programs are designed for disease and injury prevention; the IHS programs provide federal health services to American Indians and Alaska Natives. The federal government spent an estimated $313 million in Texas for the HRSA, CDS and IHS programs in fiscal 1998. About $170 million of this total went through state agencies. Building Healthy Relationships at Alaska Native Medical Center
After helping Southcentral Foundation perfect the art and science of advanced access scheduling in the 45,000-patient clinic where he works, Douglas Eby, MD, MPH, somewhat unexpectedly, says this: "We want to tell the world: It’s not about access . Access is only a tool that helps create relationships because it breaks down barriers. Relationships are really what it’s all about." It is only through solid relationships, says Eby, that you can begin to get at insidious underlying health issues such as depression, domestic violence, and obesity. Eby is vice president of medical services for Southcentral Foundation (SCF) at the Alaska Native Medical Center (ANMC) in Anchorage, Alaska, USA, a Native owned and managed 150-bed hospital and large medical center with more than 375,000 outpatient visits annually. The clinic serves the urban sprawl of Anchorage, as well as more than 50 remote villages in a 100,000-square mile catchment area that is so large it would be America’s seventh largest state. The story of ANMC is the story of remarkable vision, persistence, progress and change. It is the story of how this once federally-owned clinic — "a big, impersonal, ‘crank-‘em-through’ type place," says Eby, where most primary care was delivered in urgent care or emergency room settings — transformed itself into a customer-owned and directed system that provides same-day access to holistic, integrated, family-centered primary care.

School Health COORDINATED SCHOOL HEALTH PROGRAMS The coordinated approach to school health can address different aspects of health and education. These include: School Environment
  • EXAMPLES OF CURRICULA/PROGRAMS "Intervention specialists" or mental health counselors should be available at each school to provide training to teachers/school personnel on identifying early warning signs to negative or disruptive behavior, barriers to learning and student success, and provide guidance and intervention strategies. In addition, these specialists serve as a mediator/facilitator when conflicts between students do arise. Some schools have implemented "Solution Centers" where children can go/teachers can refer students during conflicts. The intervention specialists mediate the discussion of how to best solve the conflict. Initial reports indicate substantial improvement in student behavior in places where programs such as these have been implemented. Classrooms and buildings should receive the necessary funds to facilitate repairs/improvements necessary to provide a safe and healthy environment in which children can learn.

  • Alaska's children should have access to nutritious, child friendly meals and snacks in order to help them to be "ready to learn." School and child care environments should have policies regulating the sale of all foods sold. Those policies should be based on promoting children's health and not based on profit. Policies should include adequate time for meals, positive adult role modeling, healthy food choices and input from parents, students, and teachers. Nutrition education must be linked to the cafeteria to help children develop healthy eating beliefs and behaviors for a life time. Local, state, and federal commitment to child nutrition programs as an integral part of the educational day as evidenced by adequate financial resources and quality programs.

Appendix A: Examples of Federal Programs and Initiatives Programs on overweight and obesity span multiple departments, offices, and agencies in the Federal Government and promote valuable research and action in various settings. These programs are amplified by State, Tribal, local, and private-sector activities. Some examples of Federal initiatives on overweight and obesity, and the programs that support them, are listed below. For more information on a number of these programs, please see appendix B.
Setting 1: Families and Communities
  • The Centers for Disease Control and Prevention (CDC) has a community planning tool called the Planned Approach to Community Health (PATCH) . This tool can be valuable in the process of developing and sustaining action. The Federal Highway Administration, the Environmental Protection Agency, and the Georgia Department of Transportation have developed Strategies for Metropolitan Atlanta’s Regional Transportation and Air Quality , a document that provides a framework for assessing which factors of land use and transportation investment policies have the greatest potential to reduce the level of automobile dependence, which may consequently increase walking and bicycling activities while promoting the economic and environmental health of the Atlanta metropolitan region.
