GUIDELINES FOR INDIAN STUDENT ELIGIBILITY CERTIFICATION FORM
The following is what should be filled out on the form in the Counseling Office to become eligible for any ANE services.
WHO SHOULD FILL OUT THE FORM?
A parent, guardian, or foster parent of any student enrolled in the FNSBSD who is 1/16 or more Alaska Native or American Indian. This includes many children who have the appearance of belonging to other cultural groups.
NAME OF ELIGIBLE CHILD:
It is important to fill out the student's name as shown on school enrollment records. Include date of birth, present school and grade.
NAME OF TRIBE OR BAND:
Examples: Athabascan; Tlingit; EskimoYupik, Inupiaq; Aleut; Tsimpsian; Cherokee; Sioux; etc.
Identify the tribe or band as Federal or State Recognized, Terminated or Other Organized Group.
MEMBERSHIP INFORMATION:
Fill in the tribe member's name. If the individual named is the student and personally enrolled in the tribal organization with their own ID number, check the "Child" box.
If the student is not personally enrolled but the natural parent is, check the "Child's Parent" box.

The Alaska Department of Education and Early Development's Commissioner, Roger Sampson, notifies Superintendents of the Adequate Yearly Progress (AYP) final determinations. The determination for each school is based on data found in the Final AYP School designation sheets. These sheets can be accessed by clicking on the school name on the AYP results table. For those schools that did not make AYP, the consequences are listed below.
